Autologous clavicle bone graft for anterior cervical discectomy and fusion with titanium interbody cage.

A variety of donor-site complications have been reported for anterior cervical discectomy and fusion (ACDF) using autologous iliac bone graft. To minimize such morbidities and to obtain optimal bony fusion at the ACDF surgery, a novel technique was used to harvest cancellous bone from the autologous clavicle instead of the popular iliac crest graft. After a routine cervical discectomy of the affected level, a 1.5-cm linear skin incision was made over the clavicle within 2.5 cm of the sternoclavicular joint on the medial one-third portion. This portion is known as an anatomically safe zone, wit…

Functional Outcomes of Retrograde Intramedullary Screw Nailing for Displaced Midshaft Clavicle Fractures in Adults.

Background  Midshaft clavicle fractures are quite common among clavicle injuries in adults. Although traditionally managed conservatively, displaced fractures often lead to nonunion, malunion, and shoulder dysfunction. While plate fixation offers strong stabilization, it requires a large incision and extensive soft-tissue dissection. Intramedullary fixation has become a minimally invasive alternative, but earlier devices such as Kirschner wires and elastic stable intramedullary nails (ESIN) have shown limitations, including implant migration, irritation, and inadequate rotational stability. Th…

Non-surgical treatment for displaced midshaft clavicle fractures: a systematic review and meta-analysis.

BACKGROUND: This review aims to synthesize current evidence by pooling patient-reported outcome measures (PROMs) and reporting pooled data on pain and complication rates after non-surgically treated displaced midshaft clavicle fractures. MATERIALS AND METHODS: PubMed (Medline), Web of Science, Scopus, and the Cochrane Central Register of Controlled Trials databases were searched. Studies were eligible for inclusion if they were randomized controlled trials (RCTs) involving adult patients (≥ 18 years) with displaced midshaft clavicle fractures treated non-surgically. The primary outcome was to…

Radiological age assessment based on clavicle ossification in CT: enhanced accuracy through deep learning.

BACKGROUND: Radiological age assessment using reference studies is inherently limited in accuracy due to a finite number of assignable skeletal maturation stages. To overcome this limitation, we present a deep learning approach for continuous age assessment based on clavicle ossification in computed tomography (CT). METHODS: Thoracic CT scans were retrospectively collected from the picture archiving and communication system. Individuals aged 15.0 to 30.0 years examined in routine clinical practice were included. All scans were automatically cropped around the medial clavicular epiphyseal cart…

Ipsilateral Rockwood type V acromioclavicular joint dislocation and midshaft clavicle fracture: A case report.

BACKGROUND: Clavicle fractures are among the most common fractures seen in the emergency department. While acromioclavicular (AC) joint injuries are much less common. However, ipsilateral combinations of these injuries are quite rare with only a few cases reported in the literature. CASE SUMMARY: A 29-year-old man who sustained a combination of ipsilateral AC joint dislocation and midshaft clavicle fracture. He underwent open reduction and plate fixation of the clavicle fracture, as well as semi-rigid surgical implants used to restore both the AC ligaments and the coracoclavicular joint. one …

Dual- Versus Single-Plate Fixation of Clavicle Fractures: Understanding the Rationale Behind both Approaches.

Clavicle fractures are a common injury resulting from a high-energy force, such as a fall onto the shoulder, motor vehicle accident, or sporting activity. Although some clavicle fractures may be treated nonoperatively, operative treatment results in higher union rates and faster return to activity. Here we discuss the operative treatment options for plating of clavicle fractures; specifically, a single plate placed either superiorly or anteriorly or two plates placed orthogonally. Because both techniques provide adequate stability, fracture and patient characteristics should guide the surgical…

Dual Versus Single Plate Fixation of Displaced Midshaft Clavicle Fractures: A Cost-Effectiveness Analysis.

BACKGROUND: Prior studies have highlighted lower rates of reoperation if fixation of a displaced midshaft clavicle fracture is performed with dual plating (DP) compared with single plating (SP). Despite higher initial costs associated with the DP construct, the observed reduction in secondary surgeries compared with the SP construct may make it a more cost-effective treatment option. The objective of this study was to assess the cost-effectiveness of DP compared with SP in patients with operatively indicated displaced midshaft clavicle fractures. METHODS: We developed a decision tree to model…

Utility of Novel Clavicle-lifting Technique in Mediastinal Thyroid Surgery: A Case Series Study.

BACKGROUND/AIM: Symptomatic mediastinal goitre requires surgery and is usually resectable using the cervical approach alone; however, sternotomy is occasionally required. Sternotomy is a highly invasive procedure, and its complications, including mediastinitis and osteomyelitis, can be critical. To date, there have been no reports of non-invasive techniques to avoid sternotomy for mediastinal thyroid tumours. We investigated the safety and efficacy of thyroidectomy using the clavicle lifting technique with a paediatric Kent hook. PATIENTS AND METHODS: This was a retrospective study of 8 patie…

Acute clavicle fixation after blunt chest trauma: effect on pulmonary outcomes and patient disposition.

PURPOSE: Clavicle fractures are common in patients who sustain blunt chest trauma (BCT). Recently, surgical fixation of rib fractures in patients with BCT has been shown to improve pulmonary and clinical outcomes. Therefore, the purpose of this study is to assess the role of early clavicle fixation (ECF) versus non-operative (NO) treatment for midshaft clavicle fractures in this same population. METHODS: A retrospective chart review was performed in patients with midshaft clavicle fractures and BCT at a Level I Trauma Center between 2007 and 2017. Patients with pre-existing pulmonary conditio…

Low-profile dual mini-fragment plating of diaphyseal clavicle fractures. A biomechanical comparative testing.

BACKGROUND: Implant removal rates after clavicle plating are high. Recently, low-profile dual mini-fragment plate constructs have revealed lower implant removal rates following fixation of diaphyseal clavicle fractures. However, they have not been subject to a biomechanical investigation. AIMS: To: (1) investigate thebiomechanical competence of different dual plate designs and (2) compare them against single superoanterior plating. METHODS: Twelve artificial clavicles with a simulated AO/OTA 15.2C unstable diaphyseal clavicle fracture were assigned to 2 groups and instrumented with dual tita…

Comparative Study of Postoperative Outcomes of Clavicle Midshaft Fracture Treated by Nailing vs. Plating.

Background A midshaft clavicle fracture is a prevalent form of injury of the upper extremity that affects one's quality of life. Several treatment modalities facilitate fixation of the displaced midshaft clavicle to decrease nonunion and malunion of the clavicle fracture. Still, numerous factors influence choosing an optimal surgical intervention. Thus, this study investigates the functional outcome of two standard fixation techniques, titanium elastic nails (TENs) and locking plates, as a prospective comparative study for surgical management of displaced midshaft clavicle fractures. Methods W…

Risk factors for complications and reoperation following operative management of displaced midshaft clavicle fractures.

BACKGROUND: Optimal management of a displaced midshaft clavicle fracture remains controversial. This study assessed demographic factors, fracture pattern, and surgical technique as potential predictors of surgical complications. Smoking, diabetes, obesity, polytrauma, high-energy mechanism, inpatient status, transverse or comminuted fractures, and single-plating technique were hypothesized to be associated with an increased risk of complications following clavicle fracture open reduction internal fixation (ORIF). METHODS: Consecutive patients with minimum 12-week follow-up from the trauma and…

Clavicular osteotomy to assess subclavian vein injury during internal fixation of midshaft clavicle fractures.

Midshaft clavicle fractures are commonly fixed with locking plates. The subclavian vein risks injury during this procedure and the consequences can be fatal. The purpose of this present study is to describe a clavicular osteotomy technique in order to equip orthopaedic surgeons with a means of rapidly accessing a subclavian vein injury. The osteotomy should only be performed following an urgent intraoperative vascular surgery assessment. There must be shared consensus from both orthopaedic and vascular surgery that direct repair of the subclavian vein is necessary, and further exposure is requ…

Residual coracoclavicular separation after plate fixation for distal clavicle fractures: comparison between fracture patterns.

BACKGROUND: Plate fixation is an established treatment for Neer type II and V distal clavicle fractures; however, residual coracoclavicular (C-C) separation after osteosynthesis for unstable distal clavicle fractures has rarely been discussed. This study aimed to reveal the extent of postoperative C-C separation after plate fixation for distal clavicle fractures and to evaluate the relationship between residual C-C separation and the risk of postoperative complications. METHODS: We retrospectively reviewed 60 patients with a displaced distal clavicle fracture that was treated with a Scorpion …
