Clinical Decision-Making of Repair vs. Replacement of Defective Direct Dental Restorations: A Multinational Cross-Sectional Study With Meta-Analysis.

OBJECTIVES: This web-based survey, conducted across multiple countries, sought to explore the factors that impact the decision-making of clinicians when it comes to managing defective direct restorations. METHODS: A survey consisting of 14 questions was sent out to dentists in 21 countries through various online platforms. The survey consisted of two sections. The first contained five questions about demographic information, while the second involved eight clinical scenarios. In the second part, participants were tasked with deciding whether to repair or replace defective composite and amalga

Consensus for a primary care clinical decision-making tool for assessing, diagnosing, and managing low back pain in Alberta, Canada.

BACKGROUND: Low back pain (LBP) is a common condition causing disability and high healthcare costs. Alberta faces challenges with unnecessary referrals to specialists and long wait times. A province-wide standardized clinical care pathway based on evidence-based best practices can improve efficiency, reduce wait times, and enhance patient outcomes. Implementing such pathways has shown success in other areas of healthcare in Alberta. This study developed a clinical decision-making pathway to standardize care and minimize uncertainty in assessment, diagnosis, and management. METHODS: A systemat…

Decision-Making in Implantology-A Cross-Sectional Vignette-Based Study to Determine Clinical Treatment Routines for the Edentulous Atrophic Mandible.

This cross-sectional study aimed to investigate the influence of possible factors in the patient history on decision making in the therapy for a severely atrophied edentulous mandible. A vignette-based survey among 250 maxillofacial and oral surgeons was conducted. Determinants that could influence the therapy decision were patient age, smoking, fear of surgery, and radiotherapy in the head and neck area (the implant region is not in the direct radiation area). To achieve a suitable implant site, the options offered to the surgeons were bone split, bone block, augmentation with bone substitute

68 Ga-prostate-specific membrane antigen (PSMA) PET/CT as a clinical decision-making tool in biochemically recurrent prostate cancer.

OBJECTIVE: PSMA PET/CT has demonstrated superior sensitivity over conventional imaging in the detection of local and distant recurrence in biochemically relapsed (BCR) prostate cancer. We prospectively investigated the management impact of 68 Ga-PSMA PET/CT imaging in men with BCR, with the aim of identifying baseline clinicopathological predictors for management change. PATIENTS AND METHODS: Men with BCR who met eligibility criteria underwent 68 Ga-PSMA-11 PET/CT at Monash Health (Melbourne, Australia). Intended management plans were prospectively documented before and after 68 Ga-PSMA PET/C

Nurses' experiences of their decision-making process when participating in clinical trials during the 2014-2016 West African Ebola crisis.

OBJECTIVE: Due to the absence of a licenced vaccine or drug for treatment of Ebola patients during the 2014-2016 West Africa outbreak, frontline nurses were at increased risk of exposure. Hence, they were prioritized to participate in clinical trials to receive experimental therapeutics. To our knowledge no study has explored the nurses' experiences of their decision-making process when volunteering in clinical trials using unproven agents, which is the purpose of this qualitative study. METHODS: This study, part of a larger Ebola study, thematically analyzed the interview data of nine nurses

A novel Seizure Quality Index based on ictal parameters for optimizing clinical decision making in electroconvulsive therapy. Part 1: development.

Early identification of patients at high risk for an unfavorable outcome to ECT during the course might be beneficial because it provides an opportunity for timely intensification or optimization of stimulus conditions. We aimed to develop a new Seizure Quality Index (SQI) that delivers a clinical relevant outcome prediction early in the treatment course and can be used within common clinical setting. An observational study was conducted. Patients (n = 86) with a depressive episode and the clinical decision for ECT (right unilateral, brief pulse) were included, and several ictal parameters der

A novel seizure quality index based on ictal parameters for optimizing clinical decision-making in electroconvulsive therapy. Part 2: Validation.

Early identification of patients who are at a high risk for an unfavorable outcome to ECT during the treatment course might be beneficial because it provides an opportunity for timely intensification or optimization of stimulus conditions. We aimed to validate a previously developed seizure quality index (SQI) that delivers a clinically relevant outcome prediction early in the treatment course and can be used within common clinical setting. Therefore, a prospective study was conducted. Patients (n = 26) below the age of 65 years with a depressive episode and the clinical decision for ECT (righ

Use of antipsychotic blood levels in clinician decision making: A cross-over study using clinical vignettes of patients with schizophrenia.

The cause of treatment failure of antipsychotic medications is often difficult to determine in patients with schizophrenia. Evaluation of antipsychotic blood levels (ABLs) may aid clinicians in determining the cause of antipsychotic failure. The Clinical Assessment of the Schizophrenia Patient (CASP) was developed to evaluate clinical decision making during outpatient visits. The CASP assesses changes in medications, psychosocial treatments, and acute interventions along with factors influencing clinical decision making. Nine vignettes representative of clinical situations in patients with sch

Bypass versus Angioplasty in Severe Ischaemia of the Leg (BASIL) trial: A survival prediction model to facilitate clinical decision making.

BACKGROUND: An intention-to-treat analysis of the Bypass versus Angioplasty in Severe Ischaemia of the Leg (BASIL) trial showed that in patients with severe lower limb ischemia (SLI) due to infrainguinal disease who survived for 2 years after intervention, initial randomization to a bypass surgery (BSX)-first vs balloon angioplasty (BAP)-first revascularization strategy was associated with improvements in subsequent overall survival (OS) and amputation-free survival (AFS) of about 7 and 6 months, respectively. This study explored the value of baseline factors to estimate the likelihood of surv

Relationship between polyethylene wear, pelvic osteolysis, and clinical symptomatology in patients with cementless acetabular components. A framework for decision making.

The purpose of this study is to assess the relationship between acetabular component polyethylene wear, pelvic osteolysis, and clinical symptoms to determine when operative intervention should occur and to predict the degree of difficulty of the revision. Fifty-four revisions of failed cementless acetabular components were performed in 52 patients. All cases demonstrated polyethylene wear radiographically, and in 43 cases (80%), osteolysis of the pelvis was seen. Symptoms of groin or buttock pain were seen in 45 of 54 cases (83%). Preoperative staging of the disease process included one patien

Quality of Movement Assessment after ACL Reconstruction in Youth: A Clinical Framework to Guide Rehabilitation and Return-to-Sport Decision-Making.

Anterior cruciate ligament (ACL) injury is one of the most common knee injuries requiring surgical intervention in youth athletes. After ACL reconstruction surgery, youth athletes undergo prolonged rehabilitation which often concludes with return-to-sport (RTS) testing to determine the athlete's readiness to resume sports participation. Most rehabilitation and RTS literature have focused primarily on quantitative outcomes to guide these decisions. Given the relationship between trunk and lower extremity biomechanics with ACL injury risk, there has been a call to action for the inclusion of qua

Integrating Tumour Biology into Surgical Decision-Making for Metastatic Colorectal Cancer: A Real-World Cohort Analysis Adapted for Clinical Colorectal Cancer.

BACKGROUND: Metastatic colorectal cancer (mCRC) is biologically heterogeneous, resulting in variable outcomes despite similar disease burden and treatment. Although metastasectomy offers the only potentially curative option for selected patients, surgical selection remains largely anatomy based. Whether biological factors continue to influence outcomes after resection remains unclear. METHODS: We retrospectively analysed 290 consecutive patients with stage IV colorectal adenocarcinoma treated at a Brazilian tertiary oncology centre (2003-2019). Tumour sidedness, RAS mutational status, and met

Pillar pain after carpal tunnel release: an evidence-based review of pathophysiology, diagnostic strategies, and a structured clinical decision-making framework.

Pillar pain is a frequent, yet often overlooked, complication after carpal tunnel release (CTR). Although usually self-limiting, it can persist and interfere with recovery, hand function, and patient satisfaction. The pain typically localizes to the thenar and hypothenar regions and arises from a combination of inflammatory, biomechanical, and neurogenic factors. Diagnostic imaging and electrodiagnostic studies (EDS) mainly serve to rule out other conditions rather than confirm pillar pain directly. Conservative management through rehabilitation and individualized physical therapy remains the

Clinical value of surgical intervention and decision-making dilemmas for elderly patients of rib fractures: a systematic review and meta-analysis.

BACKGROUND: For elderly patients, when weighing the benefits and risks associated with surgery, the treatment strategy remains controversial. Our objective was to compare clinical outcomes between surgical and non-surgical management of rib fractures in patients aged ≥60 years. METHODS: PubMed, Embase, Cochrane Library, and Web of Science were searched for studies comparing surgical intervention with non-surgical treatment in elderly patients of rib fractures over the past decade. The mean differences (MDs) for continuous outcomes, the risk ratios (RRs) for binary outcomes, and the 95% confid

Conservative kidney management in a young patient with severe intellectual disability: clinical implications for kidney therapy decision-making - a case report.

BACKGROUND: Conservative kidney management (CKM) is increasingly recognized as a patient-centered treatment pathway for individuals with advanced chronic kidney disease (CKD). However, most CKM frameworks assume that patients can participate in shared decision-making, and little guidance exists for younger patients with a lifelong severe intellectual disability who lack decision-making capacity. CASE PRESENTATION: We report the case of a young adult with advanced CKD secondary to congenital anomalies of the kidney and urinary tract and severe intellectual disability who lacked decisional capa
