Serial Range of Motion and Grip Strength Measurements, Patient-Reported Outcomes, and Radiographic Thresholds Associated With Less Satisfactory Outcomes After Low-Energy Distal Radius Fracture in Women Aged 50 Years and Older.

PURPOSE: To document the change of clinical (wrist motion and grip strength) measurements and Patient-Rated Wrist Evaluation (PRWE) scores at 9 weeks and 3, 6, and 12 months after distal radius fracture (DRF) in women 50 years and older with exclusively isolated, displaced, and low-energy DRFs treated by either closed and/or open reduction and to relate these outcomes to their radiographic results. METHODS: In this retrospective single-institution cohort study, patients' post-DRF clinical measurements and PRWE scores were prospectively collected from December 2007 through September 2018 and s

A review of the treatment of hip dislocations associated with acetabular fractures.

Traumatic dislocation of the hip is an extremely severe injury. Although previously considered an uncommon lesion, it now is seen more often as a result of motor vehicle accidents. In most cases, dislocation of the hip is associated with fractures of the acetabulum, which ultimately can result in a higher incidence of complications than the complications observed in pure simple dislocations. Early recognition and prompt closed reduction of the dislocated hip constitute the cornerstone of proper treatment of this injury. Once the dislocation is reduced, definitive treatment of the acetabular fr

Operative Treatment of an Isolated, Comminuted, Biarticular Trapezium Fracture: A Case Report and Review of the Literature.

BACKGROUND: Isolated, highly comminuted trapezium fractures are extremely rare. While they can be initially diagnosed with plain radiographs, a computed tomography scan of the wrist will more clearly reveal the fracture details and aid in appropriate preoperative planning. Restoration of the fracture with a congruent reduction of the adjacent joints is mandatory for a favorable prognosis. CASE DESCRIPTION: A case of a highly comminuted, biarticular, trapezium fracture in a young male patient that was treated operatively with open reduction and internal fixation (ORIF) with a cannulated minisc

[Design and Analysis of a Rigid-Flexible Hybrid Fracture Reduction Robot].

To address the technical bottlenecks of minimally invasive closed reduction and intramedullary nailing (IMN), a rigid-flexible hybrid minimally invasive fracture reduction robot is proposed. The system leverages the compliant contact properties and continuous deformation capabilities of soft materials to ensure reduction accuracy while satisfying the compliance and safety requirements of the fracture reduction process. Additionally, based on the anatomical and physiological characteristics of the human lower limbs, a femoral shaft fracture model is developed, accounting for the presence of sof

Intelligent robot-assisted fracture reduction system for the treatment of unstable pelvic fractures.

BACKGROUND: Precise and minimally invasive closed reduction is the premise of minimally invasive internal fixation. This paper aims to explore the safety and efficacy of a robot-assisted fracture reduction system (RAFR) in the treatment of pelvic fractures and to analyze its clinical advantages and existing problems. METHODS: The RAFR system intelligently designed the optimal reduction path and target position based on a preoperative three-dimensional(3D) CT scan of the patient. The reduction robotic arm automatically reduced the affected hemipelvis according to the pre-planned reduction path

Lower Leg Fracture Irreducibility Resulting From Entrapment of the Fibula Within the Tibial Shaft.

The tibia is the most commonly fractured long bone. Tibia fractures are simple, and most are amenable to immediate closed reduction. Reported cases of irreducibility resulting from entrapment of soft-tissue structures are rare. We report the case of a 23-year-old man who, in a high-speed motor vehicle crash, sustained a closed lower leg fracture that was later found to have a unique pattern in which the fibula was entrapped in the medullary cavity of the tibia. Limited open reduction was performed, and the entrapped distal fibula was removed with use of a bone hook. The tibia was reduced, and

[Factors affecting the results of treatment in traumatic dislocation of the hip].

OBJECTIVES: We evaluated the long-term results of conservative and surgical treatment and the effect of prognostic factors on these results in patients with traumatic dislocation of the hip. METHODS: The study included 30 patients (27 males, 3 females; mean age 35 years; range 16 to 69 years) with traumatic hip dislocations. The most common cause was traffic accidents (80%). Fifteen patients underwent closed reduction for posterior (n=10), anterior (n=2), and central (n=3) dislocations, of which eight were isolated and seven were fracture dislocations. Open reduction was performed in 15 patie

Outcomes of surgical treatment of the tibial tuberosity fractures in skeletally immature patients: an update.

Tibial tubercle fractures in adolescents are uncommon injuries which typically occur in 12-16 year-old males involved in athletic activities. We hereby present our experience in the surgical treatment of such lesion. By reviewing all the tibial tuberosity fractures treated surgically at our institution between January 2012 and January 2016, we were able to identify 12 patients (14 fractures), of which 11 males and 1 female, whose average age at the time of the trauma was of 14.05. According to the Ogden classification, we identified two Type IIA fractures, one Type IIB fracture, one Type IIIB

Diagnosis and treatment of a closed and inverted metacarpal head fracture.

This case report documents a rare inversion of a closed metacarpal head fracture in the setting of polytrauma. Although rare, hemispherical articular bones can fracture and rotate 180°. Because of the symmetry of the bone and the rarity of an inverted, metacarpal head fracture, a delay in diagnosis and subsequent treatment can occur, which can lead to a poor outcome. This is particularly true in the setting of polytrauma. A 38-year-old male, polytrauma patient presented to the emergency department (ED) after falling off a bridge and being struck by an oncoming vehicle. He presented with multip

Acute compartment syndrome after intramedullary nailing of isolated radius and ulna fractures in children.

BACKGROUND: There exist varying reports in the literature regarding the incidence of compartment syndrome (CS) after intramedullary (IM) fixation of pediatric forearm fractures. A retrospective review of the experience with this treatment modality at our institution was performed to elucidate the rate of postoperative CS and identify risk factors for developing this complication. METHODS: In this retrospective case series, we reviewed the charts of all patients treated operatively for isolated radius and ulnar shaft fractures from 2000 to 2009 at our institution and identified 113 patients wh

Mini-Open Reduction and Extension Block Pinning for Irreducible Mallet Fractures: A Minimally Invasive Technique for Anatomical Reduction.

Background: Dorsal rotation or persistent displacement of the fracture fragment is frequently encountered in extension block pinning for mallet fractures. We reviewed nine irreducible mallet fracture patients treated with mini-open reduction and extension block pinning. Methods: A small V-shaped incision was made on the fracture gap when there was persistent displacement of fracture fragment despite closed maneuvers and percutaneous procedures while performing extension block pinning. Soft tissue or granulation tissue hampering reduction was removed through the incision. Anatomical reduction w

Match the Metatarsal Heads: A Case of All-Metatarsal Fractures After a Motorcycle Accident.

Metatarsal bone fractures are one of the most frequent foot injuries, especially after motor vehicle accidents in children. This case report briefly demonstrated a rare instance of pediatric all-metatarsal fractures of the left foot in an adolescent patient with polytrauma after a motorcycle accident. This case report illustrated the surgical procedure's potential for healing pediatric foot fractures in teenage patients after polytrauma. In the examination of a 16-year-old male patient brought to the emergency department after a motorcycle accident, we detected a right foot third finger proxim

Pucker sign in proximal humerus fracture: Case report and review of the literature.

INTRODUCTION: Proximal humerus fractures (PHFs) are among the most common upper limb fractures, particularly in elderly patients after low-energy falls. The pucker sign is a rare but clinically important finding caused by penetration of a fracture fragment through the deltoid muscle, leading to entrapment of subcutaneous tissue and visible skin dimpling. This sign indicates soft tissue interposition that may prevent closed reduction and threaten skin vascularity if unrecognized. CASE REPORT: A 31-year-old male with a history of multiple sclerosis presented with a proximal humerus fracture (AO

A rare but radiographically recognizable cause of an irreducible hip fracture-dislocation.

Hip dislocations are emergently reduced to decrease the chance of avascular necrosis of the femoral head. However, in some situations, the dislocation may be irreducible by a closed technique. Rarely, the fractured femoral head may become perched on an intact acetabulum, leading to an irreducible fracture-dislocation. This rare type of irreducible fracture-dislocation demonstrates unique physical and associated radiographic findings. These fractures are important to recognize early, as attempts at closed reduction can lead to femoral neck fractures.

Current Opinions on the Management of Non Thumb Metacarpal Fractures.

Metacarpals are unique bones that support the finger to aid hand function. Metacarpals are also the commonest bones to get fractured in the hand. Historically, most metacarpal fractures were managed conservatively. Due to increased patient expectations as well as advancements in diagnosis and osteosynthesis, various surgical options are now available for metacarpal fractures. The goal of operative management of metacarpal management is no longer limited to achieving clinical or radiological union. To restore hand function to a preinjury level, the surgeon must achieve adequate anatomical reduc

Ligamentum Teres Impingement in Valgus Impacted Femoral Neck Fracture: A Case for Hip Arthroscopy.

CASE: Seven years after hip arthroscopy for cam impingement, a 47-year-old female patient suffered a valgus impacted femoral neck fracture of her left hip that was stabilized with cannulated screws. After fracture healing, she experienced persisting sharp groin pain that could be elicited in flexion/internal rotation and extension/adduction. We suspected a ligamentum teres impingement at the border of the acetabular fossa due to a post-traumatic fovea alta and performed an arthroscopic ligament resection. Symptoms completely resolved thereafter. CONCLUSION: Valgus impacted femoral neck fractu
