Optimizing Neurological Documentation in the Army Medical Evaluation Board Process: A Practical Framework for Military Physicians.

The Narrative Summary (NARSUM) authored by the referring physician is the clinical basis for the Integrated Disability Evaluation System (IDES) and impacts Physical Evaluation Board (PEB) determinations of fitness, disability rating, and disposition for soldiers undergoing Medical Evaluation Board (MEB) review. Neurological conditions-post-traumatic epilepsy, residual traumatic brain injury, migraine and post-traumatic headache, multiple sclerosis, and peripheral neuropathy-account for a substantial share of MEB referrals in the active and reserve component Army, but there is no published cons

Genetics impact risk of Alzheimer's disease through mechanisms modulating structural brain morphology in late life.

BACKGROUND: Alzheimer's disease (AD)-related neuropathological changes can occur decades before clinical symptoms. We aimed to investigate whether neurodevelopment and/or neurodegeneration affects the risk of AD, through reducing structural brain reserve and/or increasing brain atrophy, respectively. METHODS: We used bidirectional two-sample Mendelian randomisation to estimate the effects between genetic liability to AD and global and regional cortical thickness, estimated total intracranial volume, volume of subcortical structures and total white matter in 37 680 participants aged 8-81 years

Memory, Executive, and Intellectual Functions in Adults with Moyamoya Disease.

OBJECTIVE: Cognitive function can decline in adults with moyamoya disease (MMD). Memory, which is an essential but complex and multifaceted function, underpins executive and intellectual functions. However, the relationship between memory and executive or intellectual functions in adults with MMD has not been well studied. The relationship between memory and cerebral blood flow has also not been elucidated. This study investigated correlations between memory, executive function, and intellectual function, and associations between cerebral blood flow and memory in adults with MMD. METHODS: Mem

Association of Physical Frailty and Cognitive Function in a Population-Based Cross-Sectional Study of American Older Adults.

INTRODUCTION: Cognitive impairment and frailty are prevalent in older persons. Physical frailty is associated with cognitive decline; however, the role of effect modifiers such as age, sex, race/ethnicity, and cognitive reserve is not well understood. METHODS: Cross-sectional data from the National Health and Nutrition Examination Survey (2011-2014) were obtained for participants aged ≥60 years. Complete availability of cognitive scores was an inclusion criterion. Physical frailty was defined by the presence of exhaustion, weakness, low body mass, and/or low physical activity, and categorized…

Cognitive dysfunction after polytrauma in the absence of traumatic brain injury: A systematic review of incidence.

BACKGROUND: Polytrauma patients frequently experience long-term health impacts, including cognitive impairments. While traumatic brain injury (TBI) is a recognized nonmodifiable cause, polytrauma patients are likely to face cognitive challenges potentially linked to systemic inflammation and multiple surgical interventions even in the absence of TBI. This review aims to describe the incidence and identify factors associated with cognitive dysfunction in adult multiple injury patients without Frank TBI. METHODS: A systematic search was conducted across MEDLINE, CINAHL, EMBASE, and Scopus datab

Exercise training for cardiovascular prevention in patients with cancer.

Patients with cancer usually report limitations of their functional capacity, which may range from subclinical impairment of cardiopulmonary exercise reserve to poor quality of life with physical, cognitive, or psychosocial consequences. Exercise training is a potent multi-targeted approach to control pre-existing and new risk factors. Preliminary evidence shows that it is associated with a lower risk of cancer therapy-related cardiotoxicity and increased self-reported well-being in patients with cancer. Current evidence demonstrates that supervised exercise therapy, including high-intensity i

Regional brain aging: premature aging of the domain general system predicts aphasia severity.

Premature brain aging is associated with poorer cognitive reserve and lower resilience to injury. When there are focal brain lesions, brain regions may age at different rates within the same individual. Therefore, we hypothesize that reduced gray matter volume within specific brain systems commonly associated with language recovery may be important for long-term aphasia severity. Here we show that individuals with stroke aphasia have a premature brain aging in intact regions of the lesioned hemisphere. In left domain-general regions, premature brain aging, gray matter volume, lesion volume and…

Effects of dual tasking on postural and gait performances in children with cerebral palsy and healthy children.

Simultaneous execution of motor and cognitive tasks is embedded in the daily life of children. 53 children of 7-12 years and 22 adults (study 1), 20 healthy children and 20 children of 7-12 years with cerebral palsy (study 2) performed a Stroop-animal task simultaneously with a standing or a walking task in order to determine the attentional demand of postural control and locomotion. Dual-task cost decreased with advancing age in healthy children during balance. CP and healthy children were similarly affected by dual-task constraints during standing and walking. Children with diplegia were mor

A neural index reflecting the amount of cognitive resources available during memory encoding: A model-based approach.

Humans have a limited amount of cognitive resources to process various cognitive operations at a given moment. The Source of Activation Confusion model of episodic memory proposes that resources are consumed during each processing, and once depleted, they need time to recover gradually. This has been supported by a series of behavioral findings in the past. However, the neural substrate of the resources is not known. In the present study, over an existing electroencephalogram data set of a free recall task (Kahana et al., 2022), we provided a neural index reflecting the amount of cognitive res

What predicts expertise in Baduk? Intelligence, practice, and a natural test of the range restriction hypothesis.

The range restriction hypothesis suggests that intelligence-performance correlations may be underestimated in domains like chess, where most players have above-average cognitive ability. We test this hypothesis using Baduk (Go), a game of even greater complexity but, apparently, without the range restrictions in intelligence typically observed in chess. In Study 1A, experts reported significantly more serious practice alone than amateurs (d = 1.42), but scored lower in fluid intelligence (d = -1.78). In Study 1B, we invited participants back to complete a revised matrix reasoning task. We prer

Reexamining gender differences and the transdiagnostic boundaries of various conceptualizations of perseverative cognition.

Research examining gender differences in perseverative cognition (repetitive, negative, and difficult-to-control thoughts) has focused on depressive rumination and internalizing syndromes. This study examines the transdiagnostic role of depressive rumination, anger rumination, and repetitive negative thinking across gender on internalizing and externalizing symptoms. Utilizing an ethnoracially diverse sample (33% Black, 35% Latinx, 32% White non-Hispanic) of n = 1,187 young adults (49.5% women), we found equivalent instrument functioning across gender for depressive rumination (specifically br

Anemia and the frail elderly.

Frailty engenders a recognizable clinical syndrome of vulnerability to stressors related to impaired physiologic reserve that primarily occurs among older adults. Features of frailty include weight loss, exhaustion, muscle weakness, performance impairment, and cognitive slowing and can be identified independent of comorbid conditions. Among frail older adults, anemia prevalence is markedly increased. The interaction between anemia and frailty is complex and confidently separating cause and effect may not be possible. Nevertheless, anemia functions as a powerful prognostic factor for the develo
