Protecting the brain from post-stroke cognitive impairment and dementia with multimodal exercise training: study protocol for a Bayesian adaptive trial (PROTECT).

INTRODUCTION: Stroke triggers acute vascular and inflammatory mechanisms that predispose the brain to rapid neurodegeneration. Up to 52% of stroke survivors develop cognitive impairment within 6 months and 20% receive a clinical diagnosis of dementia within 5 years. The subacute phase (<6 months) represents a critical window in which the brain may be most responsive to neuroprotective interventions. Multimodal aerobic and resistance training improves cognition in chronic stroke, but whether it improves cognition, neuroimaging markers and blood biomarkers of dementia risk when delivered during

Engagement in computerized cognitive training instructions by older people. A within-subject design to evaluate comprehension and acceptability of serious games instructions.

This article emphasizes the advantages of using a within-subject experimental design to assess the impact of salient visual cues on the comprehension and acceptability of computerized cognitive training (CCT) instructions among older adults. The study would involve participants aged 65 and above, who will engage in an online experiment presenting two sets of instructions for serious games: one with salient visual cues and one without. This within-subject design eliminates the need for random assignment, improves internal consistency, and enhances statistical power. Participants serve as their …

Safety, adherence, and compliance in a RCT of supervised exercise training among persons with multiple sclerosis who have slowed cognitive processing speed.

BACKGROUND: Safety, adherence, and compliance have been poorly-characterized in randomized controlled trials (RCTs) of exercise training within multiple sclerosis (MS) research. The MoXFo initiative recently proposed standard criteria for defining exercise safety, adherence, and compliance, yet these criteria have infrequently been applied within an exercise training RCT involving people with MS. OBJECTIVE: This study applied published criteria for characterizing safety, adherence, and compliance within a RCT that compared 12-weeks of supervised treadmill walking exercise (TMWX) training (int

Effects of 6-Week Motor-Cognitive Agility Training on Football Test Performance in Adult Amateur Players - A Three-Armed Randomized Controlled Trial.

Agility, defined as the ability to rapidly respond to unforeseen events, constitutes a central performance component in football. Existing agility training approaches often focus on change of direction that does not reflect the complex motor-cognitive demands on the pitch. The objective of this study is to examine the effects of a novel motor-cognitive dual-task agility training (Multiple-object tracking integrated into agility training) on agility and football-specific test performance parameters, compared to agility and a change of direction (COD) training. Adult male amateur football player

Cognitive-motor dual-task training improves dynamic stability during straight and curved gait in patients with multiple sclerosis: a randomized controlled trial.

BACKGROUND: Multiple Sclerosis (MS) is a chronic inflammatory, demyelinating, degenerative disease of the central nervous system and the second most frequent cause of permanent disability in young adults. One of the most common issues concerns the ability to perform postural and gait tasks while simultaneously completing a cognitive task (namely, dual-task DT). AIM: Assessing cognitive-motor dual-task training effectiveness in patients with Multiple Sclerosis (PwMS) for dynamic gait quality when walking on straight, curved, and blindfolded paths. DESIGN: Two-arm single-blind randomized contr

The effects of computerised metacognitive cognitive bias modification training on the development of adaptive metacognitive beliefs and post-traumatic stress disorder symptoms.

BACKGROUND AND OBJECTIVES: Maladaptive metacognitive beliefs are associated with the development of post-traumatic stress disorder (PTSD) symptoms following trauma, however it remains unclear whether training people to adopt healthy metacognitive beliefs helps to protect against the development of PTSD symptoms. We developed and tested a new cognitive bias modification training protocol (CBMMetacog) that aimed to prevent analogue PTSD by training people to adopt healthy metacognitive beliefs prior to watching a distressing film. METHODS: Participants (N = 135) received CBMMetacog or a control

Combining attention training with cognitive-behavior therapy in Internet-based self-help for social anxiety: study protocol for a randomized controlled trial.

BACKGROUND: Guided Internet-based cognitive-behavioral therapy (ICBT) has been found to be effective for social anxiety disorder (SAD) by several independent research groups. However, since the extent of clinically significant change demonstrated leaves room for improvement, new treatments should be developed and investigated. A novel treatment, which has generally been found to be effective, is cognitive bias modification (CBM). This study aims to evaluate the combination of CBM and ICBT. It is intended that two groups will be compared; one group randomized to receiving ICBT and CBM towards t

Activation and effective connectivity changes following explicit-memory training for face-name pairs in patients with mild cognitive impairment: a pilot study.

BACKGROUND: Mild cognitive impairment (MCI) is often a precursor to Alzheimer disease. Little research has examined the efficacy of cognitive rehabilitation in patients with MCI, and the relevant neural mechanisms have not been explored. The authors previously showed the behavioral efficacy of cognitive rehabilitation using mnemonic strategies for face-name associations in patients with MCI. Here, the authors used functional magnetic resonance imaging (fMRI) to test whether there were training-specific changes in activation and connectivity within memory-related areas. METHODS: A total of 6 p

Differential effects of cognitive training and aerobic exercise on regional gray matter volume and inter-regional covariance in community-dwelling older adults.

Previous studies have shown that both cognitive training and aerobic exercise benefit cognition and brain structure in healthy older adults, yet direct comparisons are limited. This study aimed to compare the effects of three months of cognitive training and aerobic exercise on regional gray matter volume and inter‑regional covariance in community‑dwelling older adults. Structural magnetic resonance imaging (MRI) was conducted at baseline and 12-month follow-up. Repeated-measures analysis of covariance (ANCOVA) revealed nominally significant time-by-group interactions in the left posterior orb…
