Broad approaches to cholera control in Asia: Water, sanitation and handwashing.

Cholera has been eliminated as a public health problem in high-income countries that have implemented sanitation system separating the community's fecal waste from their drinking water and food supply. These expensive, highly-engineered systems, first developed in London over 150 years ago, have not reached low-income high-risk communities across Asia. Barriers to their implementation in communities at highest risk for cholera include the high capital and operating costs for this technological approach, limited capacity and perverse incentives of local governments, and a decreasing availabilit

Echinococcus multilocularis management by fox culling: An inappropriate paradigm.

With the ongoing spread of Echinococcus multilocularis in Europe, sanitary authorities are looking for the most efficient ways of reducing the risk for human populations. Fox culling is one particular tool that has recently shifted from predation control to population health management. Our study aims to assess the effectiveness of this tool in limiting E. multilocularis prevalence in fox populations in France. During four years, a culling protocol by night shooting from cars was implemented around the city of Nancy (eastern France) representing ∼1700h of night work and ∼15,000km driven. The 7

Getting a GRiPP on everyday schistosomiasis: experience from Zimbabwe.

Schistosomiasis, commonly known as bilharzia, is a parasitic disease prevalent in Africa, Asia and South America. The majority of the cases occur in Sub-Saharan Africa where schistosomiasis is a major public health problem impacting on child health and development as well as adult health when infections become chronic. Control of schistosomiasis is by treatment of infected people with the antihelminthic drug praziquantel. Current schistosome control programmes advocated by the World Health Assembly in 2001 are aimed at regular school-based integrated deworming strategies in order to reduce dev

Analyzing transmission dynamics of cholera with public health interventions.

Cholera continues to be a serious public health concern in developing countries and the global increase in the number of reported outbreaks suggests that activities to control the diseases and surveillance programs to identify or predict the occurrence of the next outbreaks are not adequate. These outbreaks have increased in frequency, severity, duration and endemicity in recent years. Mathematical models for infectious diseases play a critical role in predicting and understanding disease mechanisms, and have long provided basic insights in the possible ways to control infectious diseases. [Determination of key interventions for the transition from control to elimination of malaria in China].

OBJECTIVE: To determine the key interventions transferring from the control to elimination of malaria in China so as to provide the basic information for achieving malaria elimination. METHODS: Based on the data collected from the document entitled of The National Annual Report on Schistosomiasis, Malaria and Echinococcosis, published by the National Institute of Parasitic Diseases of Chinese Center for Disease Control and Prevention, the malaria incidence and intervention data were selected only in the typical endemic provinces during the period of 2004-2010. The correlation between the inci

Lifelong vaccination as a key disease-prevention strategy.

Vaccination is traditionally considered as a measure addressed to infants and children. Indeed, in natural conditions, vaccine-preventable infections are mainly spread at a young age. The implementation of routine and mass vaccination programmes has led to the eradication of smallpox and to the elimination of poliomyelitis in many regions of the world, together with the control of once life-threatening diseases like diphtheria and tetanus. In more recent times, the development of new generation vaccines and the changing epidemiological profile of many vaccine-preventable diseases have greatly

[Basic reproduction rate and its application in schistosomiasis].

A key parameter in the transmission dynamics of schistosomiasis, namely basic reproduction rate (R0) is introduced in this paper, as well as its epidemiological significance, formulas in mathematical models of schistosomiasis transmission dynamics, and its application in the evaluation of schistosomiasis control measures. It suggests that the studies on transmission thresholds such as R0 are of important theoretical guiding significance to the establishment of a schistosomiasis elimination standard in China.

[Laboratory diagnosis of cholera: analysis and prospects for improvement].

Microbiological monitoring of the circulation of Vibro cholerae remains one of the key factors contributing to optimization of epidemiological surveillance in a specific area and the laboratory diagnosis of cholera is a basic applied tool for the detection and characterization of isolated cultures. The quality of etiological identification of the pathogen, the competent use of procedures, and the observance of a laboratory diagnosis scheme increases the likelihood of the cholera pathogen being detected in the samples taken from human beings and environmental objects, which allows to timely not

Discontinued drugs in 2005: anti-infectives.

This perspective is the fifth in a series discussing drugs dropped from development in 2005, of which 11 were being developed for infectious diseases. Of these, eight were antivirals and were dropped in Phase II or III: Medivir's alovudine, Ono Pharmaceuticals' aplaviroc hydrochloride and Excite's immunotherapeutic Xcellerate for HIV; Boehringer Ingelheim's ciluprevir, ViroPharma's HCV-086, Isis Pharmaceuticals' antisense oligonucleotide ISIS-14803, Japan Tobacco's JTK-003 and Rigel's R803 for hepatitis C virus. The remaining discontinued anti-infective drugs were an antibacterial vaccine (Vic

Effective measures for controlling trypanosomiasis.

African trypanosomiasis, otherwise known as sleeping sickness in humans and 'Nagana' in cattle, is a disease that is resurgent in Africa. Research on the disease suggests that the development of a vaccine is still far away; even existing drugs are becoming ineffective on account of the emergence of drug-resistant trypanosomes. All this contributes to heavy economic losses and a sociopolitical crisis in the continent, thus underscoring the pressure to intensify research for inexpensive, less toxic and affordable trypanocides. This review discusses the current treatment of trypanosomiasis and th

Trachoma.

Trachoma is the most common infectious cause of blindness. It is caused by ocular serovars of Chlamydia trachomatis. Transmission is favoured in poor communities, where crowding is common and access to water and sanitation inadequate. Repeated reinfection over many years causes dense scarring of the upper eyelid. The resultant inversion of the lashes abrades the eyeball, and the abrasion leads to corneal opacification and visual impairment. The host immune response is probably at least partly the cause of this process. The "SAFE" strategy is used for the control of trachoma: surgery for in-tur
