THE CONTROL OF STRYCHNINE CONVULSIONS BY INTRASPINAL INJECTIONS OF MAGNESIUM SULPHATE.

We feel that the results obtained in the human case and in our animal experiments justify the supposition that magnesium sulphate may be of use in controlling cases of poisoning by strychnine. It is a method easily available not only in large hospitals but in private practice, and requires no elaborate technique. The amount of magnesium sulphate to be used should follow the advice of Meltzer (19) in tetanus cases: intraspinally 1 cc. of a 25 per cent solution to each 20 pounds of body weight in adults, and one-half the dose in young children. Should this not control the convulsions a small amo

Focal hypermetabolism in suspected non-convulsive status epilepticus.

OBJECTIVE: To evaluate ¹⁸F-fluorodeoxyglucose (18F-FDG) positron emission tomography (PET) as a biomarker in suspected non-convulsive status epilepticus (NCSE) by characterizing electroclinical-metabolic correlations and quantifying focal hypermetabolism. METHODS: We included adult patients with rhythmic and periodic patterns (RPPs) undergoing 18F-FDG PET for suspected NCSE. Two neurologists reviewed patient electroencephalography (EEG) recordings (0-30 min post-injection) using 2021 American Clinical Neurophysiology Society (ACNS) criteria. 18F-FDG PET images were blindly assessed by two nuc

Case Report: A man with septic shock developed convulsions.

Embolism has variable manifestations in infective endocarditis (IE) and might be misinterpreted in clinical settings. In this study, we reported the case of a 67-year-old man with erysipelas and septic shock who had developed a new onset of blurred vision and subsequently convulsions and multiple cerebral infarctions. Valve vegetations and patent foramen ovale were negative on the first echocardiography. After the neurologic symptom occurred, repeated echocardiography was performed and revealed vegetations on the mitral valve. As the patient had immunological complications, we suggested prolon

Efficacy of intravenous clonazepam for paediatric convulsive status epilepticus.

AIM: To compare the efficacy of intravenous clonazepam (CLZ) for the initial management of convulsive status epilepticus (CSE) in children as a function of the first-line in-hospital dose used. METHOD: This monocentric retrospective study included children who received a first dose of CLZ for CSE at Montpellier University Hospital, France, between January 2016 and June 2019. Data from medical records (clinical, treatment, course) were collected and compared as a function of the first CLZ dose used. RESULTS: Among the 310 children treated for CSE, 105 received at least one CLZ dose (median ag

The electro-convulsive therapy story of Africa, a systematic review.

Over the years, the use of Electroconvulsive therapy (ECT) has gained increasing acceptance as a viable treatment for managing treatment-resistant mental health conditions, and it is known to deliver more rapid therapeutic benefits than most treatment modalities in psychiatry. However, the practice of ECT exhibits significant variability both within and between countries. This review aimed to shed light on the status of ECT in the African context and its implications for mental health care in the region. In July 2023, databases (Ovid, PubMed, Web of Science, etc.) were searched for articles ab

Current Management of Generalized Convulsive Status Epilepticus in Children.

Generalized convulsive status epilepticus (GCSE) in pediatric patients is an emergency condition with high morbidity and mortality and potentially irreversible brain damage, leading to cognitive deterioration, psychomotor retardation, chronic epilepsy with recurring seizures, and other complications. Treatment must be initiated in the impending GCSE phase, within five minutes of the onset of a generalized convulsive seizure. Early initiation of treatment and adequate therapy is a prerequisite for a good patient outcome.

Convulsions in children with COVID-19 during the Omicron wave.

AIM: Most children with COVID-19 have mild symptoms, but data on the Omicron variant are rare. This paper describes unexpected cases with convulsions during 1 week in January 2022. METHODS: Four children with COVID-19 were admitted with convulsions to the paediatric department in Örebro, Sweden, when Omicron accounted for more than 98% of the country's COVID-19 cases. Three children tested positive for the virus, and one had clinical COVID-19. I was able to contact the parents of three boys, who gave consent for these case studies. RESULTS: Two boys aged 3 and 21 months tested positive for t

Recent Updates on Electro-Convulsive Therapy in Patients with Depression.

OBJECTIVE: Electro-convulsive therapy (ECT) has been established as a treatment modality for patients with treatment-resistant depression and with some specific subtypes of depression. This narrative review intends to provide psychiatrists with the latest findings on the use of ECT in depression, devided into total eight sub-topics. METHODS: We searched PubMed for English-language articles using combined keywords and tried to analyze journals published from 1995-2020. RESULTS: Pharmacotherapy such as antidepressants or maintenance ECT is more effective than a placebo as prevention of recurre

Epileptic convulsions probably induced by desloratadine: a case report.

Desloratadine, a second generation H1-antihistamine, is generally considered to be safe. We found only one article reporting four children with a family or disease history of epilepsy who developed the condition after desloratadine treatment, with all four patients recovering well. Here we describe a healthy boy who developed left-arm convulsions on day 68 after taking desloratadine, at which point the desloratadine treatment was immediately stopped. Investigations were completed on day 83 and the patient was diagnosed with epilepsy. He was prescribed sodium valproate combined with oxcarbazepi

Perioperative Dystonia Mimicking Convulsion: Awareness Is the Key to Diagnosis.

Dystonia, a variant of acute extrapyramidal symptoms (EPS), is a known side effect of neuroleptics. It is a rare but well-recognized complication in perioperative settings. Most of the reported cases have been linked to the perioperative use of metoclopramide. Dystonic reactions typically present as repetitive movements or abnormal posturing of the head, neck, and body. These reactions usually last only for a few minutes and are rarely lethal. However, occasionally they may present as sustained laryngopharyngeal muscle spasms, potentially leading to fatal airway obstruction. In this report, we

Effect of fenfluramine on convulsive seizures in CDKL5 deficiency disorder.

CDKL5 deficiency disorder (CDD) is an X-linked pharmacoresistant neurogenetic disorder characterized by global developmental delays and uncontrolled seizures. Fenfluramine (FFA), an antiseizure medication (ASM) indicated for treating convulsive seizures in Dravet syndrome, was assessed in six patients (five female; 83%) with CDD whose seizures had failed 5-12 ASMs or therapies. Median age at enrollment was 6.5 years (range: 2-26 years). Mean FFA treatment duration was 5.3 months (range: 2-9 months) at 0.4 mg/kg/day (n = 2) or 0.7 mg/kg/day (n = 4; maximum: 26 mg/day). One patient had valproate
