Embolic ST-Elevation Myocardial Infarction in Atrial Fibrillation: Report of Two Cases and Review of Literature.

Coronary artery plaque rupture, erosion, thrombosis, and dissection account for nearly all acute myocardial infarction (AMI). However, coronary artery embolism remains a significant cause of AMI that is essentially unaccounted for. In this report, we present two cases of acute coronary syndrome caused by coronary embolism. Both cases illustrate that patients with atrial fibrillation are at an increased risk of thromboembolic events of the coronary circulation. We highlight the clinical characteristics of atrial fibrillation associated with coronary embolism and present the therapeutic interven…

Coronary Embolus: An Underappreciated Cause of Acute Coronary Syndromes.

Coronary embolism is the underlying cause of 3% of acute coronary syndromes but is often not considered in the differential of acute coronary syndromes. It should be suspected in the case of high thrombus burden despite a relatively normal underlying vessel or recurrent coronary thrombus. Coronary embolism may be direct (from the aortic valve or left atrial appendage), paroxysmal (from the venous circulation through a patent foramen ovale), or iatrogenic (following cardiac intervention). Investigations include transesophageal echocardiography to assess the left atrial appendage and atrial sept

Surgical repair of coronary artery fistula combined with coronary artery ectasia in adults.

OBJECTIVES: Coronary artery fistula (CAF) is rare in patients undergoing coronary angiography. Coronary artery ectasia (CAE) is found in 1.2% to 4.9% of patients at autopsy or during angiographic studies. CAF combined with CAE is a extremely rare clinical condition. This study aimed to summarize a treatment strategy for this complex disorder. METHODS: Ten consecutive patients who underwent surgical repair of CAF combined with CAE between 2000 and 2012 are reported. The main outcome measure was death. Secondary outcome measures included surgical technique, the extracorporeal circulation time,

Coronary Microcirculatory Dysfunction in Human Cardiomyopathies: A Pathologic and Pathophysiologic Review.

Cardiomyopathies are a heterogeneous group of diseases of the myocardium. The term cardiomyopathy involves a wide range of pathogenic mechan … esearch, the relation between the structural and functional abnormalities of the myocardium and the coronary circulation are still not well understood in the various forms of cardiomyopathy. The main pathological characteristics of cardiomyopathies and the coronary microcirculation develop in a progressive manner due to (1) genetic-immunologic-systemic factors; (2) co …

Endothelin 1 mediates ex vivo coronary vasoconstriction caused by exogenous and endogenous cytokines.

Treatment of rats with cytokines has been associated with an increase in the circulating levels of endothelin 1 (ET-1). Here we show that administration of tumor necrosis factor alpha (TNF-alpha; 4 micrograms.kg-1) to anesthetized rats caused within 15 min a strong elevation in the circulating levels of ET-1. This was associated with a striking coronary vasoconstriction in hearts from these animals when they were removed and perfused in vitro by the Langendorff technique. This vasoconstriction was largely overcome by treatment with either the endothelin type A (ETA) receptor antagonist FR 1393

Reconstruction of a left main coronary artery aneurysm using a radial artery patch.

A left main coronary artery aneurysm is a rare anomaly. There are no standardized treatment guidelines given the infrequency of reported cases. A 70-year-old African American female with an enlarging distal left main coronary artery aneurysm was taken to the operating room for surgical intervention. The patient underwent a successful open surgical repair of the aneurysm with reconstruction of the distal left main using a radial artery patch. No coronary bypasses were necessary. Aneurysm ligation with concomitant coronary artery bypass grafting is commonly reported but reconstruction may be pre

Cholangitis and choledocholithiasis after repair of duodenal atresia: a case report.

BACKGROUND: C1q tumor necrosis factor (TNF) related proteins 9 (CTRP9) is a novel adipocytokine that has been shown to have a cardioprotective effect in coronary artery disease (CAD). However, there are conflicting results on circulating levels of CTRP9 in patients with and without CAD. This meta-analysis was conducted to investigate the association between circulating CTRP9 levels and CAD. OBJECTIVE: The aim of this meta-analysis was to re-examine the relationship between circulating CTRP9 levels and CAD. METHODS: We searched PubMed, Web of Science, Embase, Cochrane Library, CNKI, VIP, Wanf

Coronary artery ostial atresia: diagnosis of conotruncal anastomotic collateral rings using CT angiography.

In this paper we discuss 2 unprecedented examples of coronary ostial atresia in adults. The first case documents a case of left main ostial agenesis with intercoronary communication through a retroaortic Kugel anastomotic collateral. The second case describes a right coronary artery (RCA) ostial agenesis with a preconal intercoronary anastomosis in association with supravalvular pulmonary stenosis. While atresia of the left coronary artery ostium is a rare anatomic variant of the coronary circulation, atresia of the right coronary ostium is exceedingly rare and not reported in adults (1). Beyond Coronary Artery Disease: Assessing the Microcirculation.

Ischemic heart disease (IHD) affects more than 20 million adults in the United States. Although classically attributed to atherosclerosis of the epicardial coronary arteries, nearly half of patients with stable angina and IHD who undergo invasive coronary angiography do not have obstructive epicardial coronary artery disease. Ischemia with nonobstructive coronary arteries is frequently caused by microvascular angina with underlying coronary microvascular dysfunction (CMD). Greater understanding the pathophysiology, diagnosis, and treatment of CMD holds promise to improve clinical outcomes of p

Gelanxinning capsule improves coronary microvascular dysfunction by inhibiting inflammation and restoring endothelial function.

OBJECTIVE: Gelanxinning capsule (GXSC) is a Chinese medicine to cure coronary artery disease (CAD) and a compound of Pueraria lobata, hawthorn extract, and gypenosides. However, whether GXSC could improve coronary microvascular dysfunction (CMD) is unknown. We aimed to demonstrate the therapeutic effect of GXSC on CMD and its underlying mechanisms in CAD patients. PATIENTS AND METHODS: This was a single-center, randomized control trial. A total of 78 patients diagnosed by selective coronary angiography (CAG) participated in this study. Patients' demographics, medical history, medications, and

Ischaemia without obstructive coronary artery disease: the pathophysiology of microvascular dysfunction.

PURPOSE OF REVIEW: Nearly one-third of patients presenting with angina have unobstructed epicardial coronary arteries and evidence of coronary microvascular disease. Up until recently, the pathophysiology of coronary microvascular disease has been poorly understood, resulting in limited effective therapeutic options in these patients. As a result, patients with coronary microvascular disease continue to suffer from a poor quality of life and adverse cardiovascular outcomes. RECENT FINDINGS: Recent mechanistic studies have improved our understanding of the pathophysiology underlying coronary m

[Functionality of mammary-coronary bypass grafts according to the data of intraoperative flowmetry].

Improvement of surgical treatment for ischaemic heart disease is one of the main trends in modern medicine. After the operation of coronary bypass grafting, further functioning of blood flow in the grafts largely depends upon its domination over the native blood flow in the target coronary arteries. Therefore, intraoperative diagnosis of functional competence of coronary bypass grafts by means of flowmetry is currently of special importance. The purpose of this study was flowmetric quantitative assessment of the curves of blood flow through the coronary bypass grafts, depending on the degree o

Normalisation of abnormal coronary fractional flow reserve associated with myocardial bridging using an intracoronary stent.

Although intracoronary stenting procedures have been advocated for the successful treatment of myocardial ischaemia associated with myocardial bridging, the physiological rationale for this approach remains unexplored. The case of a 70 year old man with symptoms of cardiac ischaemia associated with a left anterior descending coronary artery bridge is described, where use of an intracoronary stent abolished the angiographic abnormalities and also restituted pronounced abnormalities of coronary fractional flow reserve.

Defining Metrics and the Framework for the Invasive Assessment of Patients With Presumed Coronary Microvascular and Vasomotor Dysfunction.

The objective of diagnostic investigations should be to confirm or refute the presence of abnormal coronary function and to identify patients who might benefit from specific therapeutic interventions. In the field of coronary microvascular and vasomotor dysfunction, progress toward this goal is hampered if diagnostic pathways differ among sites, the meaning of the results are unclear, or the potential benefits of current therapies are not well established. In this second companion paper, we define the metrics that can be derived from the invasive assessment of the coronary function assessed in
