Comparison of pulse field ablation versus cryoballoon ablation in atrial fibrillation: systematic review and meta-analysis on safety and efficacy.

BACKGROUND: As the cornerstone of ablation, cryoballoon Ablation (CBA) still has certain limitations, whereas Pulse Field Ablation (PFA) offers a non-thermal and tissue specific alternative. This review aims to compare the safety and efficacy of PFA and CBA. METHODS: A systematic search for eligible studies was conducted in trial registries and databases until August 31, 2024. A total of 1090 patients from eight studies were included. RESULTS: Most studies reported higher 1-year free recurrence rate in PFA, but did not yield statistical significance (RR 1.03, 95%CI: 0.94, 1.12, p = 0.55, I2

Cryoablation Within the Evolving Paradigm of Multidisciplinary Breast Cancer Treatment De-Escalation.

Treatment de-escalation is increasingly shaping the management of breast cancer, aiming to reduce treatment intensity, minimize toxicity, and improve quality-of-life outcomes without compromising effectiveness and survival. Breast cancer cryoablation, a minimally invasive image-guided percutaneous technique that destroys tumor cells through controlled freezing and thawing, has emerged as an alternative to surgery for select patients with early-stage disease. Support for this approach has grown following recent prospective trials demonstrating its safety and efficacy, US Food and Drug Administr

Preoperative cryoneurolysis for peri- and postoperative pain in total knee arthroplasty: a systematic review and pooled analysis.

BACKGROUND: Cryoneurolysis is seeing increased implementation in total knee arthroplasty (TKA), providing postoperative analgesia through preoperative cryoneurolysis of peripheral nerves. Thus, we are conducting this systematic review and meta-analysis to determine cryoneurolysis' efficacy in providing analgesia and improvements in recovery times for patients undergoing TKA. METHODS: We conducted a comprehensive search using five databases including PubMed, Cochrane Library, Web of Science, Embase, and Scopus. The screened articles were assessed for inclusion of primary outcomes: pain scores

Cryoneurolysis as a Neuroregenerative Intervention for Chronic Painful Mononeuropathies: A Four-Patient Case Series and Discussion.

Posttraumatic painful mononeuropathy involves a permanent structural change in the affected nerve. Clinical and animal models have shown that exposure of nerves to specific cold temperatures for distinct time periods can induce: cessation of signal transduction, Wallerian degeneration, and predictable axonal regeneration. It is hypothesized that these processes may return a structurally altered painful nerve to a non-pathologic or normal state. We present four patients in which percutaneous cryoneurolysis was employed with the intent of inducing regeneration and repair. Primary outcomes includ

MR Imaging-Guided Cryoablation of Small Renal Masses.

Cryoablation has become a widely adopted thermal ablation modality for the treatment of renal tumors. Magnetic resonance (MR) imaging offers several distinct advantages over computed tomography (CT) for procedural guidance, including superior lesion characterization, multiplanar imaging capability, enhanced soft-tissue contrast resolution, and improved visualization of intraparenchymal lesions. Moreover, MR imaging allows for more precise delineation and real-time monitoring of the ice ball during ablation, while eliminating ionizing radiation exposure. Despite the logistical challenges associ

Spray Cryotherapy for Benign Large Airway Stenosis: A Multicenter Retrospective Cohort Study of Safety and Practice Patterns.

BACKGROUND: Benign airway stenosis (BAS) represents a significant burden on patients, providers, and healthcare systems. Spray cryotherapy (SCT) has been proposed as an adjunctive treatment to reduce BAS recurrence. We sought to examine safety and practice variations of the latest SCT system when used for BAS. METHODS: We conducted a retrospective multicenter cohort study in seven academic institutions within the Interventional Pulmonary Outcomes Group. All patients who underwent at least one SCT session with a diagnosis of BAS at the time of procedure at these institutions were included. Dem

Cryoprobe biopsy versus mechanical biopsies in pulmonary diagnostics.

PURPOSE OF REVIEW: Biopsy tools have been essential in improving the diagnostic accuracy of bronchoscopic procedures. Of these tools, cryobiopsy has emerged as a promising technique for diagnosing thoracic diseases. This review summarizes the existing data comparing cryobiopsies to other mechanical biopsy methods for sampling endobronchial, parenchymal, and mediastinal targets. RECENT FINDINGS: Initially adopted for managing airway stenoses, the use of cryoprobes has expanded to diagnosing endobronchial lesions, parenchymal opacities, and mediastinal lymph node pathologies. Studies have demon

First-in-Human Experience With Ultra-Low Temperature Cryoablation for Monomorphic Ventricular Tachycardia.

Ultra-low temperature cryoablation (ULTC) using near-critical nitrogen (-196ºC) has been shown to produce durable, contiguous, transmural lesions in ventricles of animal models. This report summarizes acute experience with ULTC in the first-ever 13 patients with recurrent monomorphic ventricular tachycardias (VTs) of both ischemic cardiomyopathy and nonischemic etiologies enrolled in the CryoCure-VT (Cryoablation for Monomorphic Ventricular Tachycardia; NCT04893317) clinical trial. After an average of 9.6 ± 4.6 endocardial ULTC lesions per patient, no clinical ventricular tachycardias were ind

The Impact of Surgical Posterior Nasal Nerve Cryoablation on Symptoms and Disease-Specific Quality of Life in Patients With Chronic Rhinitis.

OBJECTIVE: Preliminary data have demonstrated long-term efficacy of posterior nasal nerve (PNN) cryoablation in reducing rhinitis symptoms for patients with allergic rhinitis (AR) and nonallergic rhinitis (NAR). We sought to evaluate the impact of procedural cryoablation of the PNN on quality of life (QOL) in patients with AR and NAR. METHODS: Adult patients undergoing PNN cryoablation for AR or NAR after appropriate medical therapy were included for analysis. Demographics, medical therapies, baseline rhinitis symptom (total nasal symptom score [TNSS]), and disease-specific QOL (mini-rhinocon

Feasibility and safety of cryoablation on the porcine ureter using a new balloon cryoprobe.

Cryoablation, as a well-characterized technology, has multifarious clinical applications. But few are reported on ureteral endometriosis. Thus, we examined the feasibility and safety of cryotherapy with a novel balloon cryoprobe on ureter in a live porcine model. During the freezing and thawing cycle, temperature was recorded in different parts of the ureter in real-time. Two-minute cryoablation could induce necrosis within range in 1 cm diameter on serosa layer. The temperature respectively dropped to -136.5 °C, -96.1 °C and -17.5 °C at the cryotherapy center, 1 cm and 1.5 cm from center on s

Pleural Cryobiopsy: A Systematic Review and Meta-Analysis.

BACKGROUND: Pleural biopsy using either video-assisted thoracoscopic surgery or medical pleuroscopy is the current diagnostic criterion standard for pleural pathology with a high, yet imperfect, diagnostic yield. Cryobiopsy may provide greater tissue, increase depth of sampled tissue, and/or reduce crush artifact. However, its impact on diagnostic yield remains uncertain, and there are potential concerns regarding its safety too. We performed a systematic review and meta-analysis to investigate the same. METHODS: We performed a systematic search of MEDLINE, Embase, and Google Scholar for stud

Is achievement of pulmonary vein isolation the only effect of cryoballoon ablation in long-standing persistent atrial fibrillation?

A case of long-standing persistent atrial fibrillation (LPAF) that underwent hybrid treatment combining transvenous cryothermal and radiofrequency (RF) energy is described in the present report. Complex fractionated atrial electrogram (CFAE) maps before and after cryoablation revealed not only pulmonary vein isolation (PVI), but also a large antral ablation of CFAEs. Following cryoablation, a severe reduction in remote left atrial CFAE area was observed.

[Catheter ablation of typical atrial flutter].

Atrial flutter is one of the most common supraventricular arrhythmic diseases and occurs with an incidence of 88/100,000. The high risk for thromboembolic events is similar to that for atrial fibrillation and also has the risk for fast conduction to the ventricle. Pharmacological treatment is not significantly effective. Catheter ablation of common atrial flutter offers an alternative with a high quality outcome ranging from 90-96% and a low recurrence rate of 2-6%. Radiofrequency catheter ablation of the cavotricuspidal isthmus is an established treatment for common atrial flutter. Modern tec
