[Surgical treatment of hemifacial spasm: a systematic review].

UNLABELLED: The annual incidence of hemifacial spasm is 8-15 cases per 100.000. This disease is not life-threatening, but it significantly impacts the quality of life. Primary treatment is vascular decompression of the facial nerve. Thus, improvement of appropriate technique is crucial for higher efficacy and safety. OBJECTIVE: To determine the optimal algorithm for vascular decompression according to available international experience in surgical treatment of hemifacial spasm. MATERIAL AND METHODS: A systematic review was conducted in accordance with the PRISMA guidelines. To identify relev

Lateral Lumbar Interbody Fusion with Percutaneous Pedicle Screw in Combination with Microendoscopic Laminectomy in the Lateral Position for Lumbar Canal Stenosis.

A minimally invasive surgical (MIS) procedure is an ideal surgical procedure. Many MIS techniques have been reported in spinal surgery. In clinical practice, we often encounter two-level canal stenosis cases, in which one level shows instability and the other does not. In such a case, fusion surgery for one level and decompression surgery for the other level is ideal. LLIF/OLIF approached from the lateral side has been reported effective. MIS decompression techniques in the lateral decubitus position have never been reported. We devised a surgical method that can accomplish both fixation and d

Treatment Concept and Technical Considerations of Biportal Endoscopic Spine Surgery for Lumbar Spinal Stenosis.

Decompression is a major component of surgical procedures for degenerative lumbar spinal stenosis (LSS). In addition to sufficient decompression to guarantee the relief of neurological pain, compensating surgical instability after wider laminectomy and foraminotomy and instrumentation with caging and fusion with grafting are performed to secure or restore the foraminal dimension and correct coronal/sagittal imbalance for longer survival of the adjacent segment. Endoscopic spinal surgery (ESS) has been developed under the flag of successful decompression while preserving structural integrity as

Endoscopic-Assisted Extended Nerve Decompression with Radical Artery Elimination Yields Superior Relief for Refractory Occipital Migraines.

BACKGROUND: Surgical decompression of the greater occipital nerve (GON) is a recognized treatment for occipital migraines that are resistant to medical management. Recent evidence suggests that vascular compression by the occipital artery (OA) contributes to occipital migraine pathophysiology. This study compared postoperative outcomes between endoscopic-assisted radical GON decompression along its entire course with complete OA elimination and standard open GON decompression with limited proximal OA elimination. METHODS: A retrospective review was performed on 85 patients who underwent endos

Craniocervical spinal instability after type 1 Arnold Chiari decompression: a case report.

To present and describe an unusual case of spinal instability after craniocervical spinal decompression for a type-1 Chiari malformation. Type-1 Chiari malformation is a craniocervical disorder characterized by tonsillar displacement greater than 5 mm into the vertebral canal; posterior fossa decompression is the most common surgical treatment for this condition. Postoperative complications have been described: cerebrospinal fluid leak, pseudomeningocele, aseptic meningitis, wound infection, and neurological deficit. However, instability after decompression is unusual. A 9-year-old female pres

Order of surgical intervention in double-crush syndrome.

BACKGROUND: Double-crush syndrome involves concurrent nerve compression at two points, typically in the cervical spine and peripherally in the upper extremity, such as the ulnar nerve at the elbow or the median nerve at the wrist. Surgical management of this condition, including which decompression to perform first, remains controversial. We aim to explore the effect of surgical technique, timing, and relative order on outcomes in patients with combined cervical and peripheral upper extremity nerve compression. METHODS: This retrospective study included adult patients treated at a Level 1 trau

Meta-Analysis of Simultaneous versus Staged Decompression of Stenotic Regions in Patients with Tandem Spinal Stenosis.

BACKGROUND: Tandem spinal stenosis (TSS) refers to simultaneous spinal canal stenosis of noncontiguous regions. There is no consensus in the surgical strategies for patients with symptomatic TSS because of the confusing clinical manifestations. This meta-analysis aimed to compare the outcomes of 1-stage and 2-stage decompression of all stenotic regions (cervical and lumbar/thoracic segments) in patients with TSS. METHODS: A systematic review was conducted, and a comprehensive literature search with the Japanese Orthopedic Association score, was carried out using MEDLINE, Web of Science, and P

Emergency laparoscopic approach without sufficient preoperative decompression for intersigmoid hernia: A case report.

INTRODUCTION: In acute care surgery, an increasing number of patients operatively treated for small bowel obstruction undergo laparoscopic procedures. However, intersigmoid hernia is a rare condition. In some reports, surgeons have successfully operated on patients with an intersigmoid hernia via a laparoscopic approach. The laparoscopic approach has the advantage of facilitating simultaneous diagnosis and surgical intervention for intersigmoid hernias. In the laparoscopic approach, sufficient decompression of the small bowel is preoperatively performed in most cases. PRESENTATION OF CASE: We

Curcumin modulates chronic myelogenous leukemia exosomes composition and affects angiogenic phenotype via exosomal miR-21.

BACKGROUND: Posterior fossa decompression (PFD) has been widely accepted for the surgical treatment for Chiari malformation type I (CM1). However, inadequate decompression causes surgical mortality and complications such as cerebrospinal fluid leakage, meningitis, or progression of syrinx and symptoms. The authors report a novel technique of PFD under navigation. METHODS: Five female patients with CM1 who developed severe symptoms and underwent surgical treatment were evaluated (mean age 14.0 years; mean follow-up 1.3 years). Surgical outcomes, surgical time, intraoperative blood loss, and op

Evaluation of the degenerative lumbar spondylolisthesis instability classification (DSIC) system as a guide to surgical technique selection.

PURPOSE: Degenerative spondylolisthesis (DS) is addressed with a wide range of surgical techniques, though controversy exists regarding surgical technique selection. Given the lack of high-quality evidence to guide surgical technique selection in DS, appropriateness criteria and classification systems have been developed based on expert opinion. The DSIC System uses imaging and patient characteristics to predict stability. The purpose of this study was to evaluate the DSIC system as a guide for technique selection by determining if patients within each DSIC Type have different outcomes when tr

Ten-Step 3-Dimensional-Navigated Single-Stage Lateral Surgery With Microtubular Decompression: A Case Series.

BACKGROUND: Single-stage lateral lumbar interbody fusion is a safe and effective procedure that relies on indirect decompression and fusion to treat various lumbar pathologies. This technique, however, has an overall 9% rate of indirect decompression failure, which may require additional surgery to achieve adequate direct decompression. To address this concern, we modified this technique by adding a minimally invasive, direct tubular decompression in lateral position when indicated. No study has described the technical nuances of incorporating a microtubular decompression into the single-stage

Exoscopic microvascular decompression for hemifacial spasm and trigeminal neuralgia.

The 4K 3D exoscope system is becoming increasingly used in neurosurgery. Its 3D ultra-high-definition image is valuable in identifying and dissecting the delicate neural and vascular structures during microvascular decompression. In this video, the authors describe several nuances and details to perform the exoscopic microvascular decompression, including the exoscope layout and the modified supine position. Several illustrative case presentations highlight the benefits of exoscopic surgery. The authors' exoscopic microvascular decompression series of 159 patients showed noninferior surgical o

Functional outcomes and complications in refractory dysthyroid optic neuropathy management: Experience with 3 different surgical protocols.

INTRODUCTION: Dysthyroid optic neuropathy (DON) is the most severe complication of Graves' orbitopathy (GO) and its management may require decompression surgery. Clear recommendations do not exist about which surgery should be performed and how extended the decompression should be. In this paper we present our experience regarding the management of DON via 3 different surgical protocols: a modified extended orbital apex decompression, a 2 walls decompression (inferior and lateral) and a 3 walls decompression (inferior, lateral and medial) and evaluate the functional outcomes. METHODS: Retrosp
