BACKGROUND: Thrombocytosis can be present in patients with myeloproliferative disorders or can accompany various conditions, in particular chronic inflammatory diseases, namely chronic bowel diseases, rheumatoid arthritis, and nephritis.
OBJECTIVE: We report our experience in 55 patients younger than 45 years of age with increased platelet counts (over 500 X 10(9)/L). Thirty-three were affected by essential thrombocytemia in agreement with polycytemia vera study group criteria and 22 by reactive thrombocytosis. Serotonin concentration has been determine in all the patients.
RESULTS: Serotoni

BACKGROUND: Bursal fluid accumulations are not usually associated with skin disease; however, several cases have been observed in a large atopic dermatitis (AD) clinic.
OBJECTIVE: We sought to characterize the clinical features of bursitis in AD and to assess the evidence for a mechanistic association between these conditions.
METHODS: We retrospectively evaluated the clinical presentation of bursitis in six adults with lifelong severe AD. The course of the bursitis and possible causative factors were assessed for each patient.
RESULTS: Ten episodes of bursitis were documented in six patien

The authors present the results of allergologic investigations in 54 infants and children with atopic dermatitis. To demonstrate the food and inhalative allergies, which are most important etiologic factors in this disease, different methods have been administered. Besides these in vitro and in vivo methods oral food challenges were done to verify the real causative role of the food in question. Food allergy only was verified in 16 cases, 12 patients were suffering in inhalative allergy. Both kind of allergens produced reaction in 19 cases. Among the foods hen egg caused sensibilization most f

Contact sensitivity and immediate hypersensitivity to extracts from Pityrosporum ovale were studied in patients with atopic dermatitis (AD). In a chamber-scarification patch test, 75 (64%) of 118 patients with AD responded positively, compared with 1 (3%) of 35 healthy volunteers. However, no significant statistical correlations were found between contact sensitivity to P ovale in patients with AD and any of the following factors: age, sex, distribution of skin lesions, presence of pruriginous papules, history of infantile seborrheic dermatitis, or concomitance of other atopic diseases. Lympho

Sensitization trials with dinitrochlorobenzene (DNCB) were performed in 150 patients with atopic dermatitis (AD) (24 severe, 86 moderate, and 40 mild cases). Dinitrochlorobenzene challenge tests were positive in 33% (8/24) of severe cases, in 95% (82/86) of moderate cases, and in 100% (40/40) of mild cases, indicating that DNCB contact sensitivity is diminished only in patients with AD who have extensive skin lesions. Of the 20 patients who were nonreactive on the first DNCB challenge tests, 18 became reactive on the second challenge tests that were performed at the time when the dermatitis wa

The use of the nonsedating antihistamine terfenadine (60 mg twice daily) in the treatment of chronic idiopathic urticaria and contact urticaria associated with atopic dermatitis was assessed. In patients with chronic idiopathic urticaria, terfenadine was found to be as effective as the traditional antihistamine clemastine in reducing the number of wheals and the severity of itch, without causing drowsiness. A separate study showed terfenadine to be of value in some patients with atopic dermatitis and a history of contact urticaria.

It has recently been suggested that hyperlinear palms seen in atopic dermatitis are actually a manifestation of concomitant ichthyosis vulgaris. Patients with atopic dermatitis were examined in our clinic for the presence or absence of ichthyosis vulgaris and/or hyperlinear palms. While 70 percent of the patients with atopic dermatitis and ichthyosis did have hyperlinear palms, only 29 percent of the patients with atopic dermatitis and hyperlinear palms had concomitant ichthyosis vulgaris. These findings do not support the suggestion that hyperlinear palms in atopic dermatitis are a manifestat

Twenty-six children with atopic dermatitis and markedly elevated serum IgE concentrations were evaluated for clinical evidence of hypersensitivity to foods with double-blind placebo-controlled food challenges. Selection of foods for challenges was based on positive prick skin tests (greater than 3 mm wheal) or a convincing history. At least one positive skin test to a food antigen was found in 24/26 patients. A total of 111 double-blind placebo-controlled challenges were performed in these children after suspect foods were eliminated from their diets for 10 to 14 days. There were 23 positive c

UNLABELLED: Psoriasis and atopic dermatitis (AD) are traditionally viewed as distinct inflammatory skin diseases driven by type 17 and type 2 immune pathways, respectively. Increasing use of targeted therapies has revealed a treatment-associated phenotype switch between these conditions, the so-called 'flip-flop' phenomenon, characterized by sustained emergence of eczematous features in patients treated for psoriasis or psoriasiform disease in those treated for AD. Robust real-world data on this entity remain limited.
OBJECTIVES: To characterize treatment-associated phenotype switching betwee

BACKGROUND: Atopic dermatitis (AD), which causes eczema, dryness, and itching, is one of the most prevalent skin diseases in children. Along with topical corticosteroids and the liberal application of emollients, monoclonal antibodies have recently been used for systemic treatment in moderate to severe cases. This study aims to review the current literature on the application of monoclonal antibodies (mAbs) in treating children with AD.
METHODS: To conduct a narrative review of the literature, we searched for the keywords (monoclonal antibodies OR biologics) AND (pediatric OR children) AND (a

Atopic dermatitis (AD) is the most common chronic inflammatory skin disease and has traditionally been viewed as a disorder confined to the skin. However, emerging epidemiologic, genetic, and mechanistic evidence increasingly supports the concept that AD represents a systemic disease associated with a broad spectrum of immune-related comorbidities. This review synthesizes current literature on the comorbidities associated with AD, including dermatologic, respiratory, gastrointestinal, rheumatologic, neurologic, ophthalmologic, and endocrine disorders. Well-established associations include othe

Atopic dermatitis (AD) patients usually have elevated serum IgE that is thought inducing inflammation upon binding to allergen. However, the role of IgE-producing B cells and other isotypes of immunoglobulin, such as IgG, in AD are not clear and rarely explored. This study aimed to investigate the role of IgE-producing B cells and other isotypes of immunoglobulin, particularly IgG, in skin lesion of AD. BCR repertoires were analysed using mRNA prepared from skin lesions and peripheral blood mononuclear cells (PBMCs) from AD patients and non-allergic healthy subjects. Single-cell RNA sequencing

Atopic dermatitis (AD) and autoimmune diseases exhibit epidemiological comorbidity, yet the shared genetic architecture remains incompletely understood. We investigated the genetic overlap between AD and three autoimmune disorders including inflammatory bowel disease (IBD), rheumatoid arthritis (RA), and vitiligo, leveraging genome-wide association data. Despite modest evidence for global genetic correlations, we found 113 independent pleiotropic loci shared among AD and autoimmune diseases, with 11 displaying a concordant effect across all 3 pairwise comparisons. Gene-set and tissue enrichmen

The clinical manifestations of atopic dermatitis (AD) and chronic nodular prurigo (CNPG) include pruritus and eczema/lesions, posing significant challenges for patients. Th2 cells and ILC2, marked by cytokine production-particularly IL-4/13-are crucial therapeutic targets. Despite displaying a dose-dependent lack of pruritus induction post-injection, IL-13 acts through the IL-13Rα1 and IL-13Rα2 receptor system. Our study focused on investigating ex vivo skin biopsies in AD (n = 17), CNPG (n = 14) and healthy controls (HC; n = 10), examining the gene expression landscape of interleukins linked

IMPORTANCE: Previous studies suggest that atopic dermatitis (AD) is associated with cognitive impairment in children, but these studies have relied primarily on neurodevelopmental diagnoses (rather than symptoms) as proxy measures of cognitive function. It remains unknown if certain subpopulations of children with AD are at greater risk of cognitive impairment.
OBJECTIVE: To examine the association of AD with symptoms of cognitive impairment (difficulty in learning or memory) among US children and whether this association varies according to the presence or absence of neurodevelopmental comor
