Dermoscopic Examination of Generalized Lichen Nitidus in a Patient With Down Syndrome.

Down syndrome, also known as trisomy 21, is the most frequently identified chromosomal disorder. Patients with Down syndrome frequently present with a broad spectrum of dermatological conditions, including atopic dermatitis, nonspecific dermatitis, folliculitis, seborrheic dermatitis, and cutaneous infections. Several cases of generalized lichen nitidus in individuals with Down syndrome have been reported. However, dermoscopic findings have not been previously described in these cases. We report a case in which dermoscopy contributed significantly to the clinical diagnosis, illustrating its po

[Current eczema therapy].

The diagnostic term eczema refers to a group of skin diseases defined by the morphological criteria of erythema, papules, papulovesicles and, in the chronic stage, lichenification and desquamation. While the diagnostic workup should lead to a more specific diagnosis (irritant or allergic contact dermatitis, atopic, seborrheic or nummular dermatitis), the therapeutic approach is similar for most forms of eczema. In the present review, traditional and innovative therapeutic options are presented and discussed.

Case report: Diaper dermatitis presenting as pustules.

Diaper dermatitis is the most common dermatologic disorder of infancy. Its cause can often be determined clinically based on the clinical presentation. Primary diaper dermatitis is associated with irritants and spares the deep skin folds. Secondary diaper dermatitis is most often caused by Candida yeast overgrowth and typically presents as a well-defined area of beefy red erythema covering the diaper area and including the deep folds of skin with hallmark satellite pustules. Other causes include seborrheic dermatitis, psoriasis, acrodermatitis enteropathica, allergic contact dermatitis, Langer

Diaper dermatitis. Factors to consider in diagnosis and treatment.

Diaper dermatitis is a common problem in infants and young children. Although the pathogenesis of diaper dermatitis is not well defined, many associated factors have been identified, including individual predisposition to atopic or seborrheic dermatitis, occlusion and friction caused by the diaper, and overgrowth of bacteria and yeast. General treatment goals involve keeping the diaper area as dry as possible, washing the area as infrequently as possible, and avoiding tight-fitting diapers. Petrolatum, talc, baking soda, and fluorinated topical steroidal medications should be avoided. If all e

Old drug--in a new system--revisited.

Sodium sulfacetamide, penetrating antibacterial, in combination with hydrocortisone and sulfur, has enjoyed twenty years of remarkable safety, with outstanding efficacy and patient acceptance, in the prescription treatment of pustular acne and severe, refractory seborrheic dermatitis. Recently, this combination has been reported to be highly effective concomitant therapy for perioral dermatitis. Almost paradoxically, it achieves these desired goals without the excessive erythema and discomforting irritation associated with retinoic acid and benzoyl peroxide.

Perioral dermatitis.

Perioral dermatitis is a relatively common inflammatory facial skin disorder that predominantly affects women. It is rarely diagnosed in children. A typical perioral dermatitis presentation involves the eruption of papules and pustules that may recur over weeks to months, occasionally with fine scales. The differential diagnosis includes seborrheic dermatitis, systemic lupus erythematosus, acne vulgaris, lupus miliaris disseminatus faciei, polymorphous light eruption, steroid-induced rosacea, granulomatous perioral dermatitis, contact dermatitis (allergic and irritant), and even basal cell car

Histopathological and immunohistochemical studies of infective dermatitis associated with HTLV-I.

Infective dermatitis associated with HTLV-I (IDH) is a chronic, recurrent, exudative eczema occurring in childhood which is considered to be a risk factor for the development of lymphoma and HTLV-I-associated myelopathy/tropical spastic paraparesis. Skin biopsies from 19 patients with IDH were studied histologically and immunohistochemically using the following antibodies: anti-CD3, CD45RO, CD20, CD79a, CD4, CD8, CD56, CD57, TIA-1, granzyme-B, and perforin. A chronic dermatitis similar to atopic and seborrheic dermatitis was observed in 15 cases, whereas architectural aspects mimicking mycosis

Allergic contact dermatitis from ketoconazole.

Ketoconazole is a widely used imidazole antifungal agent. True contact allergy to topical ketoconazole is rare, and few cases of patients with contact allergy to ketoconazole have been reported. We present the case of a patient with a history of undiagnosed recurrent dermatitis who developed acute facial swelling and pruritus after using ketoconazole cream and shampoo for the treatment of seborrheic dermatitis. Patch testing revealed true contact allergy to ketoconazole without cross-reactivity to 4 other imidazole antifungals. Review of the patient's medical record suggested that prior incide

Diagnosis and Management of Common Inflammatory Skin Diseases in Older Adults.

Inflammatory skin conditions affect people of all ages, genders, and races. These common conditions are frequent causes of visits to the dermatologist. The geriatric population is often afflicted by these conditions because many are chronic and relapsing diseases. These inflammatory conditions include but are not limited to psoriasis, atopic dermatitis, contact dermatitis, seborrheic dermatitis, rosacea, and Grover disease. Chronic inflammatory skin conditions place a large burden on the health care system in the United States and have many associated comorbidities. This article discusses thes…

Clinical Relevance of the Microbiome in Pediatric Skin Disease: A Review.

The human microbiome encompasses the microorganisms that live in and on the body. During the prenatal and infantile periods, foundations for the cutaneous and gut microbiomes are being established and refined concurrently with the development of immune function. Herein, we review the relevance of the microbiome to 5 conditions commonly encountered in pediatric dermatology: acne, alopecia areata, atopic dermatitis, psoriasis, and seborrheic dermatitis. Understanding the role microbes play in these conditions may establish the groundwork for future therapeutic interventions.

Possible side effects of topical steroids.

When used properly in controlled situations, topical corticosteroids provide dramatic relief in distressing disorders such as eczema, psoriasis and seborrheic dermatitis. The fluorinated preparations are associated with a variety of side effects, including acne, striae, premature aging of the skin, hypertrichosis, perioral dermatitis, glaucoma and even adrenal suppression. Except for the severe, acute dermatoses, a nonfluorinated steroid is the better choice because of the relative lack of side effects and lower cost.
