Indications for clinical islet transplantation today and in the forseeable future--the diabetologist's point of view.

The treatment of patients with type 1 diabetes mellitus has to focus on short-term and long-term risks of the disease which means to avoid hyperglycemic or hypoglycemic coma as well as late complications. As we know from the DCCT study metabolic control substantially lowers the risk for retinopathy, nephropathy and neuropathy. We also know, that keeping the blood glucose in a nearly normal range inevitably is connected with a marked increase of severe hypoglycemia, an event which occurs more frequently when normoglycemia has been reached and the further slow decline of blood glucose is not rec

Therapeutic effectiveness of sodium bicarbonate in diabetic ketoacidosis: A retrospective cohort analysis using the marginal structural Cox model.

The treatment effect of sodium bicarbonate in diabetic ketoacidosis (DKA) remains controversial, as clinical guidelines recommend its use only in extreme acidemia (pH < 6.9) owing to inconsistent evidence and concerns over potential risks such as paradoxical central nervous system acidosis, delayed ketone clearance, and electrolyte imbalance. This study aimed to evaluate the real-world effectiveness and safety of sodium bicarbonate therapy in critically ill patients with DKA in a large clinical database. This is a retrospective cohort study following the STROBE guidelines on the MIMIC-IV datab

Solitary Primary Central Nervous System Lymphoma Mimicking Third Ventricular Colloid Cyst-Case Report and Review of Literature.

BACKGROUND: Primary central nervous system lymphoma is a rare malignant tumor of the central nervous system. It is associated with poor prognosis and accounts for 0.7%-0.9% of all lymphomas and only 0.3%-1.5% of intracranial tumors. Typically, these lesions are in the cerebral white matter near the corpus callosum, the central gray matter, the basal ganglia-thalamus-hypothalamic region, the posterior fossa and the periventricular region. Only 2 cases with pure third ventricular lymphoma have been reported in the literature. CASE DESCRIPTION: A 72-year-old female patient known to have type II

Improvement in renal prognosis with prompt hemodialysis in hyperosmolar hyperglycemic state-related rhabdomyolysis: A case report.

RATIONALE: Acute kidney injury is common and correctable in patients with a hyperosmolar hyperglycemic state (HHS). Nevertheless, hyperglycemic crisis may also contribute to the development of rhabdomyolysis, which can worsen renal function and lead to high mortality in such patients. PATIENT CONCERNS: Herein, we report a case of hyperosmolar hyperglycemic state-related rhabdomyolysis and acute renal failure with an excellent outcome. DIAGNOSIS: A 26-year-old Asian female with underlying paranoid schizophrenia presented with newly diagnosed type 2 diabetes mellitus complicated with HHS. Her

EXPERIMENTAL STUDIES ON DIABETES : SERIES I. PRODUCTION AND CONTROL OF DIABETES IN THE DOG. 5. VARIOUS FAILURES OF DIETETIC TREATMENT, AND THEIR CAUSES.

1. Practically every detail of clinical diabetes can be reproduced in partially depancreatized animals. The resemblance is made still more exact by the susceptibility of such dogs to acidosis and coma, as will be shown in later papers, and also by the similarity of the anatomic changes in the islands of Langerhans. These animals are therefore useful test objects for a therapeutic investigation. 2. These animals at first show considerable tendency to regain assimilation, comparable to that in the early stages of most human cases of diabetes; and in some instances they recover so as to be able t

Incidence and Outcomes of Non-ST Elevation Myocardial Infarction in Patients Hospitalized with Decompensated Diabetes.

Single center studies suggested that non-ST elevation myocardial infarction (NSTEMI) in patients admitted with acute decompensated diabetes is associated with poor long-term prognosis. We hypothesize that acute decompensated diabetes is also associated with worse early morbidity and mortality in patients admitted with NSTEMI. Adult patients with a primary discharge diagnosis of diabetic ketoacidosis (DKA) or hyperosmolar hyperglycemic state (HHS) were identified in the national inpatient sample. We then assessed contemporary trends in the incidence and management patents of NSTEMI in patients

Stress Hyperglycemia Is Associated With Similar Mortality and Complication Rates Compared to Trauma Patients With Diabetes.

INTRODUCTION: Diabetes mellitus (DM) is one of the most common diseases worldwide and is a known significant cause of morbidity and mortality. Specifically, DM and stress-induced hyperglycemia (SH) significantly increase morbidity and mortality in trauma patients. It is currently unknown how complication rates compare between trauma patients with SH and DM as well the rate of undiagnosed diabetes in trauma patients. METHODS: The institutional review board at our institution reviewed and approved the following study. A practice management guideline was established calling for the collection of

Episodic ventriculomegaly due to hypernatremia mimicking shunt malfunction: case report.

Patients with shunted hydrocephalus presenting with altered mental status and ventriculomegaly are generally considered to be in shunt failure requiring surgical treatment. The authors describe a case of shunted hydrocephalus secondary to a disseminated neuroectodermal tumor in a pediatric patient in whom rapid fluctuations in sodium levels due to diabetes insipidus repeatedly led to significant changes in ventricle size, with invasively confirmed normal shunt function and low intracranial pressure. This clinical picture exactly mimics shunt malfunction, requires urgent nonsurgical therapy, an

Chronic tophaceous gout in a 72-year-old male presenting with fatal hypertensive hemorrhagic stroke: a case report.

BACKGROUND: Chronic tophaceous gout is a well-recognized consequence of long-standing hyperuricemia. Although epidemiological evidence increasingly links hyperuricemia with elevated cerebrovascular risk, the co-occurrence of advanced tophaceous gout and fatal hemorrhagic stroke is infrequently reported, particularly from South Asian populations. CASE PRESENTATION: A 72-year-old Nepali male with a 15-year history of hypertension and a 12-year history of poorly controlled gouty arthritis presented following a fall with decreased consciousness. He was in hypertensive crisis (200/100 mmHg) with a

First Ecuadorian Pediatric Case of Multisystem and Neurological Involvement Associated with Influenza A-H5N1 Virus-Case Report.

Influenza A (H5N1) is a highly pathogenic zoonotic virus with a human fatality rate of approximately 60%. Pediatric cases and associated neurological manifestations remain poorly documented in Latin America. This report describes the first confirmed Ecuadorian pediatric case of H5N1-associated encephalitis and multisystem organ failure in a previously healthy 9-year-old female following direct contact with infected poultry. The clinical course was characterized by an atypical initial presentation of bilateral periorbital edema and headache, progressing to acute encephalitis, cerebral ischemia,

Solitary neurocysticercosis mimicking tuberculoma and glioblastoma: A diagnostic challenge in a low-resource setting.

BACKGROUND: Neurocysticercosis (NCC) is the most common parasitic infection of the central nervous system and a major cause of acquired epilepsy in endemic regions. Solitary parenchymal lesions frequently mimic tuberculoma or high-grade glioma, posing a significant diagnostic challenge, particularly in tuberculosis-endemic and resource-limited settings. CASE DESCRIPTION: We report the case of a 60-year-old male farmer with diabetes mellitus who presented with a 1-week history of focal seizures with secondary generalization and progressive left-sided weakness. Neurological examination revealed
