Insulin-dependent diabetes mellitus: a risk factor in anorexia nervosa or bulimia nervosa? An empirical study of 116 women.

A number of recent studies suggest that diabetes mellitus confers a high risk for the development of anorexia nervosa or bulimia nervosa. In order to test this hypothesis, 56 women with IDDM and 60 non-diabetic female controls were studied. All subjects completed the Eating Attitudes Test (EAT), and the Bulimic Investigatory Test, Edinburgh (BITE). The subjects were interviewed in order to obtain clinical and demographic information as well as to determine test validity. The DSM-III-R criteria of anorexia nervosa and bulimia nervosa were used. Four items were removed from the original EAT in o

The effects of gluten-free diet versus hypocaloric diet among patients with fibromyalgia experiencing gluten sensitivity symptoms: protocol for a pilot, open-label, randomized clinical trial.

BACKGROUND: Fibromyalgia is a chronic musculoskeletal pain syndrome characterized by a broad spectrum of manifestations. Patients with fibromyalgia frequently suffer from manifestations similar to those experienced by patients with gluten-related disorders raising the possibility that some patients with fibromyalgia could suffer from underlying gluten sensitivity. OBJECTIVE: This study aims to assess whether avoiding gluten among patients with fibromyalgia and gluten sensitivity is beneficial. METHODS: Adult patients with fibromyalgia presenting gluten sensitivity symptoms are randomly alloc

Severe Perioperative Lactic Acidosis and Hyperglycemia Responsive to Thiamine in a Patient With Prior Semaglutide Use and Restrictive Dieting: A Case Report.

Thiamine deficiency is a rare but potentially fatal cause of type B lactic acidosis, classically associated with alcoholism or severe malnutrition. Restrictive dieting and weight-loss regimens, including glucagon-like peptide-1 (GLP-1) receptor agonist use, may unmask thiamine deficiency under perioperative stress. We report a young, nondiabetic woman on a restrictive diet who developed severe lactic acidosis and hyperglycemia after total abdominal hysterectomy, with worsening lactate despite hemodynamic stability and adequate hydration. Empiric intravenous thiamine produced rapid normalizatio

Effect of visceral manipulation on menstrual complaints in women with polycystic ovarian syndrome.

CONTEXT: Research is lacking regarding osteopathic approaches in treating polycystic ovary syndrome (PCOS), one of the prevailing endocrine abnormalities in reproductive-aged women. Limited movement of pelvic organs can result in functional and structural deficits, which can be resolved by applying visceral manipulation (VM). OBJECTIVES: This study aims to analyze the effect of VM on dysmenorrhea, irregular, delayed, and/or absent menses, and premenstrual symptoms in PCOS patients. METHODS: Thirty Egyptian women with PCOS, with menstruation-related complaints and free from systematic disease

Mental health and physical activity in women with polycystic ovary syndrome: a brief review.

This review was designed to consider the available literature concerning mental health and physical activity in women with polycystic ovary syndrome (PCOS). A systematic approach was taken and two electronic databases (PubMed and EBSCO Research articles published between 1970 and 2013) were searched in 2013 to inform a narrative review. Inclusion criteria encompassed requirements for the research to involve a physical activity intervention and assessment of mental health outcomes in women with PCOS. Seven articles considered mental health outcomes and physical activity interventions for women

Psychologic and physiologic effects of dieting in adolescents.

Obesity in adolescents has increased by 75% in the past three decades. Cross-sectional and prospective surveys have shown that a large percentage of adolescents, particularly females and even those of normal weight, diet at some time. While moderate changes in diet and exercise have been shown to be safe, significant psychologic and physiologic consequences may occur with extreme or unhealthy dieting practices. Moderate dieting has been shown to be associated with negative self-esteem in some adolescents. The very act of starting any diet increases the risk of eating disorders in adolescent gi

Differences between binge eating disorder and nonpurging bulimia nervosa.

OBJECTIVE: To compare the pathway to binge eating and clinical characteristics of binge eating disorder patients (BED) and nonpurging bulimics. METHODS: The subjects were 45 nonpurging bulimics and 45 BED patients who consecutively sought treatment in an outpatient eating disorders unit. The subjects underwent a clinical interview and replied to the Eating Disorders Inventory and the Hopkins Symptom Checklist. RESULTS: In most of the nonpurging bulimics (89%), binge eating is preceded by dieting and weight loss, whereas among BED patients the pathway to binge eating is more variable. Previou

The relationship of dieting severity and bulimic behaviors to alcohol and other drug use in young women.

Patients with bulimia nervosa frequently have problems with alcoholism and other substance abuse. The goal of this study was to assess whether this relationship between eating abnormalities and substance abuse extends to subthreshold levels of dieting and substance use. A self-administered questionnaire assessing dieting and substance use (alcohol, cigarettes, and marijuana) was completed by 1,796 women prior to their freshman year in college. Using a scale derived from DSM-III-R criteria for bulimia nervosa and previous research in this population, subjects were categorized as nondieters, cas

Cholelithiasis in patients treated with a very-low-calorie diet.

One hundred seventy-nine obese patients (mean body mass index = 36.3) were retrospectively evaluated for the development of cholelithiasis associated with the use of a 2530-kJ/d (605-kcal) very-low-calorie diet (VLCD). Nine percent of patients had preexisting gallstones and 11% of patients developed gallstones either during or within 6 mo of completing the diet. Six percent had subsequent cholecystectomy. Ursodeoxycholic acid administered to one patient resulted in spontaneous stone dissolution whereas spontaneous dissolution occurred in three patients. Surveys of patients at three other progr

Gallstone formation during weight-reduction dieting.

We investigated the development of gallstones over an 8-week period from the onset of dieting in 51 obese men and women and 26 nondieting control subjects. Gallbladder examinations were performed by abdominal real-time ultrasonography for the detection of gallstones. Initial sonography was performed prior to dieting and only those subjects in whom initial sonograms showed no gallstones or sludge were included in the study. Repeated sonography was performed at 4-week intervals for 8 weeks while they remained on a 2100-kJ/d diet. Initial weight of subjects prior to dieting averaged 105.9 +/- 3.8

Longitudinal examinations in the course of dietotherapy of mentally retarded obese in-patients.

Mentally retarded obese in-patients were fed by low-energy diet (4.2-4.6 MJ) for 9 months. During this period, an average of 13 +/- 4.5 kg loss of body mass occurred in men and 16 +/- 2.7 kg in women. Anthropometric measurements were performed before starting the dietotherapy and in the ninth month. Changes of body fat could be followed well when calculated according to BMI. Less reliable results were obtained with skinfold thickness measurements, presumably due to body deformities. Results of clinical laboratory tests, which were carried out before starting the dietotherapy and in the fourth

Do behavioral treatments of obesity last? A five-year follow-up investigation.

Although the long-term maintenance of therapy induced behavior changes and the resulting weight loss represent critical issues in the treatment of obesity, there is a paucity of available data. The present study assessed the durability of treatment induced weight losses 5 years after treatment and the long-term adherence to treatment strategies. Thirty-six of the original 44 subjects who participated in a 10-week behavioral weight control program were interviewed in person (28), by mail (5) or reported their weights by phone (3). Results indicated that most subjects gained back a major portion

Adaptation to hypocaloric feeding: physiologic significance of the fall in serum T3 as measured by the pulse wave arrival time (QKd).

We have investigated the physiologic significance of the decline in serum triiodothyronine (T3) occurring during hypocaloric feeding by measurement of changes in cardiovascular function. The QKd interval, the interval between the Q wave of the electrocardiogram and the onset of Korotkoff sounds at diastolic pressure at the brachial artery, is the sum of the preejection period and pulsetransmission time, and has proven to be a sensitive and effective measure of the effect of thyroid hormones on the cardiovascular system. Fifteen euthyroid obese volunteers underwent successive 2 wk periods of hy

Effect of glycerol on weight loss and hunger in obese patients.

The effectiveness of oral glycerol as a dietary component or as a supplement to a 1000-kcal/day diet was examined in two studies involving obese patients. Glycerol did not differ from an equicaloric dose of glucose in its effect on hunger ratings, diet compliance or overall weight loss. We conclude that oral glycerol is not a useful adjunct to weight reduction programs.
