Expression analysis suggests a potential cytoprotective role of Birc5 in the inner ear.

Hearing impairment is a worldwide health problem. Employing semi-quantitative immunological detection methods, we found that the apoptosis inhibitor protein Birc5 is expressed in cell types critical for hearing perception. In the guinea pig model, moderate noise exposure causing only a temporary mean hearing impairment of 33dB significantly enhanced Birc5 expression in the spiral ligament, nerve fibers and the organ of Corti. In contrast, intratympanic gentamicin injection inducing permanent cell damage and mean hearing loss of 24dB correlated with a significant Birc5 downregulation in the lig

Cochlear implant and inner ear malformation. Proposal for an hyperosmolar therapy at surgery.

The objective of this retrospective study is to evaluate the efficiency of hyperosmolar therapy for cerebrum spinal fluid (CSF) leakage in cochlear implant (CI) surgery in children with inner ear malformations. Between 1991 and 2006, 490 cochlear implantations were performed in Armand Trousseau Children's Hospital. Thirty-seven patients (7.5%) had inner ear malformation. They were classified as isolated enlargement of the vestibular aqueduct (EVA) (18 cases), incomplete partition (IP) (11 cases), common cavity (CC) (1 case) and variable canal and vestibular malformations (VSCC) (7 cases). A hy

Mumps virus may damage the vestibular nerve as well as the inner ear.

We report a case of mumps deafness with acute vestibular symptoms secondary to both retrolabyrinthine and inner ear dysfunction. To our knowledge, no such case has been reported elsewhere. The patient was a 6-year-old girl in whom mumps deafness was initially diagnosed. Two days after onset of the hearing impairment, severe vertigo developed. Neurotologic examinations revealed spontaneous right-beating nystagmus, left canal paresis, absence of vestibular-evoked myogenic potentials in the left ear, and absence of responses to a left galvanic body sway test (GBST). The vertigo resolved 7 days af

The role of bone morphogenetic protein 4 in inner ear development and function.

Bone Morphogenetic Protein 4 (BMP4) is a member of the TGF-beta superfamily and is known to be important for the normal development of many tissues and organs, including the inner ear. Bmp4 homozygous null mice die as embryos, but Bmp4 heterozygous null (Bmp4(+/-)) mice are viable and some adults exhibit a circling phenotype, suggestive of an inner ear defect. To understand the role of BMP4 in inner ear development and function, we have begun to study C57BL/6 Bmp4(+/-) mice. Quantitative testing of the vestibulo-collic reflex, which helps maintain head stability, demonstrated that Bmp4(+/-) mi

Efficacy of an ototoxic aminoglycoside (gentamicin) on the differentiation of the inner ear of cichlid fish.

Previous investigations revealed that the growth of fish inner ear otoliths depends on the amplitude and the direction of gravity, thus suggesting the existence of a (negative) feedback mechanism. In the course of these experiments, it was shown that altered gravity both affected otolith size (and thus the provision of the proteinacious matrix) as well as the incorporation of calcium. It is hitherto unknown, as of whether sensory hair cells are involved either in the regulation of otolith growth or in the provision of otolithic material (such as protein or inorganic components) or even both. [A patient with acoustic neuroma--The reason for the functional disorder of his inner ear?].

BACKGROUND: Acoustic neuroma is one of the classic causes of retrocochlear hearing disorders. Auditory brainstem response (ABR) testing with a sensitivity of about 90 % in the diagnosis of acoustic neuroma plays an important role as a screening method. CASE REPORT: A patient suffering from an acute, unilateral sensorineural hearing loss with tinnitus recovered for a short period of time after being treated with low density lipoprotein (LDL-) apheresis improving hemorheology. Surprisingly, the reason for the sudden hearing loss was a tumor in the cerebellopontine angle with a maximum diameter

Basic and clinical physiology of the inner ear receptors and their neural pathways in the brain.

The six receptors of the inner ear (cochlea, two otolith organs and three semicircular canals) share a common transduction unit made up of a sensory hair cell, a first order sensory neuron and the synapse between them. Displacement of the stereocilia in a particular direction leads to excitation of the hair cell and activation of the neuron. Electrical and mechanical reflections of these stages of transduction can be recorded non-invasively in humans and in animals. These include cochlear microphonic potentials, otoacoustic emissions, auditory and vestibular evoked potentials. The ability to r

Role of S-100beta as potential autoantigen in an autoimmune disease of the inner ear.

In Lewis rats, adoptive transfer of T cells specific for the calcium-binding protein S-100beta mediates experimental autoimmune encephalomyelitis, but surprisingly also induced a marked labyrinthitis associated with impairment of hearing. This suggests that transfer of S-100beta-specific T cells into susceptible animals could be a novel model to study autoimmunity of inner ear diseases. The investigation demonstrated in detail an inner ear involving, central nervous system (CNS)-specific autoimmune disease in order to identify a putatively shared autoantigen(s) in these pathologies. In fact, t

An alternative method for dealing with cerebrospinal fluid fistulae in inner ear deformities.

OBJECTIVE: This study aimed to review the management of cerebrospinal fluid (CSF) fistulae in the setting of developmental inner ear deformity. STUDY DESIGN: The study design was a case review, close examination of preoperative radiology, and corresponding intraoperative images. TECHNIQUE: A definitive method of CSF fistula closure is described using previously known techniques used commonly in skull base surgery. CONCLUSIONS: The use of a multiple-level, reinforced wound closure technique is necessary to definitively close CSF fistulae in extreme inner ear deformity and to prevent further

Identification and localization of type IV collagen chains in the inner ear cochlea.

Mutations in the genes encoding the alpha3(IV), alpha4(IV) and alpha5(IV) chains of type IV collagen have been implicated in the pathogenesis of Alport's syndrome, a hereditary disorder characterized by progressive nephropathy and sensorineural deafness. The known expression of these chains in kidney basement membranes supports the contention that they play a crucial role in the ultrafiltration function. Whether they play a role in auditory signal transduction remains unknown as heretofore, they have not been identified in the inner ear. In the present study, the expression of type IV collagen

Clinical Experiences of Steroid Targeting Therapy to Inner Ear for Control of Tinnitus.

We summarize our long-term clinical experiences with an inner-ear drug delivery system (DDS) for control of tinnitus. The inner-ear DDS consist of transtympanic perfusion of 2 or 4 mg dexamethasone to the round window via the middle ear. Therapeutic results of steroid targeting therapy to the inner ear were evaluated in 3,978 ears of 3,041 patients. Tinnitus improved in 75% of these patients immediately after treatment and in 68% after 6 months. The effects of such therapy on tinnitus varied depending on the underlying diseases. The improvement of tinnitus in the presence of labyrinthine hydro

Jean Marquet Award. Regeneration of the neurosensory structures in the mammalian inner ear.

Regeneration of the neurosensory structures in the mammalian inner ear. Motivated by the absence of treatment of neurosensory deafness apt to restore auditory function or alter the course of progressive hearing loss, we developed two different experimental strategies to approach these diseases: one is otoprotection, designed to prevent further degradation of auditory function; the other is regeneration which is defined as the replacement of hair cells in the deaf ear and their reconnection to the central nervous system through primary auditory neurons. In this paper, we summarize our data on t

Clinical study of the role of melanocytes in the inner ear of patients with Harada's disease.

Harada's disease is an autoimmune disorder involving melanocytes. Twenty patients with Harada's disease underwent otoneurological examination to investigate the inner ear involvement in Harada's disease and the function of the melanocytes in the inner ear. All 20 exhibited clinical and/or laboratory abnormal inner ear findings. The incidence of inner ear involvement in Harada's disease is very high. The melanocytes in the inner ear play important roles in both auditory and equilibrium function.

Effect of hand-arm vibration on inner ear and cardiac functions in man.

To evaluate distant effects of hand-arm vibration we studied Finnish forestry workers using chain saw during the years 1972 through 1990. The hearing was tested annually and individual regression curves for sensorineural hearing loss (SHL) were calculated. Robinson's model was used in prediction of SHL. The heart rate variation (HRV) indexes at rest and during deep breathing test were analyzed to measure autonomic nervous function. In Robinson's model the measured SHL (17.8 dB) respected the predicted SHL (17.2 dB). The subjects with VWF had on average, 10 dB greater hearing loss than those wh

Intermediate filaments in the inner ear of normal and experimentally damaged guinea pigs.

The hypothesis that proteins known to occur in glial cells in the central nervous system may be present in inner-ear supporting cells was investigated. Immunocytochemical techniques were used to look for the existence of two classes of intermediate filaments, vimentin and glial fibrillary acidic protein (GFAP), in cellular elements of the inner-ear epithelium in normal and experimentally damaged guinea-pig cochleas. Vimentin is present in two types of supporting cells in the normal organ of Corti: Deiters' cells and inner pillar cells. Differences in intensity and distribution of vimentin immu

Invasion of the inner ear by Haemophilus influenzae type b in experimental meningitis.

Bacterial interstrain variation for cochlear invasion was studied by intraperitoneal inoculation of infant rats with Haemophilus influenzae type b. Eight pairs of CSF isolates from children with or without deafness due to meningitis were injected into half of each litter in separate experiments. At 48 h, quantitative CSF culture results and CSF white blood cell counts were equivalent for the two groups. Organisms within the cochlea were detected in four of eight animals in each group. There was no difference between the deaf and nondeaf isolates in the degree or frequency of inner ear inflamma

The role of acute and latent virus infections in the pathogenesis of inner ear disturbances.

The possible role of herpesviral infections of the inner ear in suddenly appearing inner ear disturbances was investigated. Experimental pseudorabies virus (PRV, Herpes sui 1) infection of mice and swine was used as a model system. Infected mice represented the productive cycle of PRV infection (acute phase), whereas the latent phase of infection could be tested in swine. From the acutely infected mice the virus could be reisolated from perilymphatic fluid and various parts of the brain. Massive histopathologic alterations and signs of total cell damage to the organ of Corti and the vestibular
