Auricular reconstruction with a postauricular myocutaneous island flap: flip-flop flap.

Surgical defects of the concha-helix part of the ear larger than 2 cm may pose a reconstructive challenge. Split- or full-thickness skin grafts or local flaps may be used, and a number of these have been described. Yet cosmetic results are often unsatisfactory. Our experience with a postauricular myocutaneous island flap is described. Eleven patients (12 ears), aged 48 to 89 years, underwent the procedure under local anesthesia following excision of conchal bowl malignant tumors that included the cartilage underlying the skin. The surgical technique is described in detail. Few complications we

[Cholesteatoma in childhood. Surgical treatment and results].

261 children with 281 cholesteatoma operations performed between 1970 and 1982 in the ENT-Clinic of the University of Tübingen were followed-up. In 77 ears an open technique (attico-antrotomy with removal of the posterior canal wall) was employed. In 103 ears a combined approach with posterior tympanotomy and preservation of the posterior canal wall was performed. In 47 ears an atticotomy with reconstruction of the lateral attix wall defect with tragal cartilage and perichondrium was done. The cholesteatoma was limited to the middle ear in 54 patients; treatment consisted of a transcanal tympa

Induction in vivo of cartilage grafts for craniofacial reconstruction.

In the craniofacial region, defects of cartilage structures are preferably reconstructed with autologous cartilage. Donor-site morbidity related to the creation of a new defect elsewhere, and a lack of growth potential of the graft--mandatory in children--have stimulated investigators to find other ways to generate new "extra" cartilage. Several biomaterials have been tested as a matrix for the ingrowth of (peri)chondroblasts in experimental animals. In young (growing) rabbits we have developed a process of heterotopic cartilage induction with the use of a demineralized (bovine) bone matrix wh

A Case with Thyroid Cartilage Fracture after Sneezing.

Non-traumatic laryngeal injuries are unusual occasions. In the medical literature we found only six reports of cases that had laryngeal injury after sneezing. We report a case of a 34-year-old man diagnosed with thyroid cartilage fracture after a strong sneeze. In physical examination, edema and hematoma were seen in the right vocal cord and the right band. Computed tomography scan revealed an anterior thyroid cartilage fracture without separation. Antibiotics and steroids were administered. This is a very rare entity and the seventh case reported in the literature. Ear Nose Throat specialist

Endoscopic Transcanal Type 1 Tympanoplasty with Cartilage Perichondrium Graft; A Prospective Study.

INTRODUCTION: Recently use of endoscopes for ear surgeries is being popular among the surgeons as it gives a wide view of the middle ear. Also, tympanoplasty by transcanal approach can be done as a day care procedure and so it reduces the cost of hospitalization. The study was aimed to evaluate the results of type 1 tympanoplasty done by endoscopic transcanal approach with cartilage perichondrium graft, in relation to successful graft take-up rate and improvement in hearing. MATERIALS AND METHODS: Total 50 patients were included in the study and all of them underwent type 1 tympanoplasty by e

Percutaneous adjustable closed otoplasty for prominent ear deformity.

OBJECTIVE: The aim of this study is to follow longitudinally the prominent ears treated by percutaneous adjustable closed otoplasty (PACO) and evaluate this procedure in terms of technical efficiency, recurrence, complications, and patient satisfaction. MATERIALS AND METHODS: Percutaneous adjustable closed otoplasty was used to treat 28 ears in 15 patients presenting with prominent ear deformities. To determine the success of each operation, distances between the helical rim and scalp were measured at 4 levels preoperatively, at the end of the surgery and again postoperatively at the first we

Better late than never: effective air-borne hearing of toads delayed by late maturation of the tympanic middle ear structures.

Most vertebrates have evolved a tympanic middle ear that enables effective hearing of airborne sound on land. Although inner ears develop during the tadpole stages of toads, tympanic middle ear structures are not complete until months after metamorphosis, potentially limiting the sensitivity of post-metamorphic juveniles to sounds in their environment. We tested the hearing of five species of toads to determine how delayed ear development impairs airborne auditory sensitivity. We performed auditory brainstem recordings to test the hearing of the toads and used micro-computed tomography and his

Doc, Just Cut My Ears Off-A Case Report of Actinomyces in Bilateral Chronic Auricular Chondritis.

Ear piercings can lead to infectious complications, commonly involving Staphylococcus aureus and Pseudomonas aeruginosa . This is the first documented case of bilateral chronic auricular chondritis caused by Actinomyces odontolyticus following cartilage piercings. This case highlights a uniquely morbid case necessitating interdisciplinary care and aggressive surgical intervention for resolution.

Prevention of meatal stenosis in conchal setback otoplasty.

The conchal setback is a useful technique for correcting many prominent ear deformities. A disadvantage of the technique in some cases is meatal stenosis of the external auditory canal. By excising a portion of meatal cartilage, this problem is prevented. The technique is illustrated and post-operative result is shown.

Microbiome of Acute Otitis Externa.

BACKGROUND: Ear canal skin is directly attached to bone or cartilage, and is also connected to the eardrum. Acute otitis externa is cellulitis of the ear canal skin and subdermal tissue associated with acute inflammation and variable edema. We characterized the microbiome of the normal ear canal and ear canal with otitis externa. METHODS: In total, 28 samples (14 each from the ear canal skin of patients with acute otitis externa and normal healthy controls) were collected using swabs. DNA extraction and bacterial microbiome analysis via 16S rRNA gene sequencing were performed. RESULTS: The d

Ear reconstruction with porous polyethylene implants.

This article describes a surgical technique using porous polyethylene as the framework material for ear reconstruction. In comparison to the use of rib cartilage, porous polyethylene - first described by Berghaus in 1982 - provides better definition and projection as well as congruency with the opposite side. Hospitalization time is significantly shorter. There are less surgical interventions than with traditional microtia operations that use rib cartilage, and the patient is spared the additional procedure needed to remove the rib cartilage, with all the associated complications as well as th

A 2-stage ear reconstruction for microtia.

OBJECTIVE: To introduce our 2-stage reconstruction of microtia method, which results in a natural-looking contour of the reconstructed ears, one of the most demanding challenges in facial plastic surgery. METHODS: In the first stage, the 3-dimensional cartilage framework is fabricated. The skin flap and retroauricular fascial flap are elevated in the mastoid area. Then the framework is wrapped by the fascial flap from behind and covered by the skin flap from front. In the second stage the crus, the tragus, and the conchal cavity are reconstructed. So almost all of the fine structures of ear a

Cost analysis of microtia treatment in the Netherlands.

INTRODUCTION: Ear reconstruction for microtia is a challenging procedure. Although analyzing esthetic outcome is crucial, there is a paucity of information with regard to financial aspects of microtia reconstruction. This study was conducted to analyze the costs associated with ear reconstruction with costal cartilage in patients with microtia. METHODS: Ten consecutive children with autologous ear reconstruction of a unilateral microtia were included in this analysis. All patients had completed their treatment protocol for ear reconstruction. Direct costs (admission to hospital, diagnostics,

High-resolution computational modeling of the current flow in the outer ear during transcutaneous auricular Vagus Nerve Stimulation (taVNS).

BACKGROUND: Transcutaneous auricular Vagus Nerve Stimulation (taVNS) applies low-intensity electrical current to the ear with the intention of activating the auricular branch of the Vagus nerve. The sensitivity and selectivity of stimulation applied to the ear depends on current flow pattern produced by a given electrode montage (size and placement). OBJECTIVE: We compare different electrodes designs for taVNS considering both the predicted peak electric fields (sensitivity) and their spatial distribution (selectivity). METHODS: Based on optimized high-resolution (0.47 mm) T1 and T2 weighted

Dysmorphogenesis of the mandible, zygoma, and middle ear ossicles in hemifacial microsomia and mandibulofacial dysostosis.

A review of the anatomical changes in patients with various "first arch" syndromes shows that some anomalies (e.g., micrognathia, ear defects) generally appear together. This study tested the hypothesis that the mandible, zygomatic arch, and middle ear ossicles are a developmental field (i.e., when any of these structures is anomalous, the other two will be also). The hypothesis was tested using data from 25 patients with mandibulofacial dysostosis (MFD) and 40 patients with hemifacial microsomia (HFM). Analysis of the pooled data showed that the hypothesis of character association was general

Total auricular reconstruction after traumatic total amputation of the auricle.

We present a case of successful total auricular reconstruction after total amputation of the auricle. Owing to the severe damage to the auricular cartilage, replantation was not attempted; instead, for the next step of the reconstruction, severed superficial temporal artery and vein were anastomosed. At the second stage, total auricular reconstruction was planned. A huge temporoparietal fascial flap was used (based on STA/STV), which was used to cover the costal cartilage ear framework. This case suggests that, even for the total loss of the auricle, autogenous reconstruction can still be succ

Relapsing polychondritis: an unusual cause of painful auricular swelling.

Auricular pain, redness, and swelling are usually the result of infectious cellulitis. However, relapsing polychondritis is another cause of this symptom complex and is the result of an autoimmune phenomenon. It presents with inflammation and destruction of both articular and nonarticular cartilage, with the external ear and joint cartilage most often involved. Although usually indolent with multiple acute exacerbations, relapsing polychondritis can be rapidly fatal if the airway or cardiovascular system is involved. We present a case of relapsing polychondritis initially mistaken for auricula

[Atypical relapsing polychondritis].

Relapsing polychondritis is a disease of unknown etiology whose main characteristic is the chronic inflammation and destruction of the different cartilaginous structures of the body. A rare case is presented, the rareness being the fact that ear cartilage was not affected and that the destructive arthropathy was similar to that produced by rheumatoid arthritis.
