Electromyographic comparison of various exercises to improve elbow flexion following intercostal nerve transfer.

We compared the quantitative electromyographic activity of the elbow flexors during four exercises (forced inspiration, forced expiration, trunk flexion and attempted elbow flexion), following intercostal nerve transfer to the musculocutaneous nerve in 32 patients who had sustained root avulsion brachial plexus injuries. Quantitative electromyographic evaluation of the mean and maximum amplitude was repeated three times for each exercise. We found that mean and maximum elbow flexor activity was highest during trunk flexion, followed by attempted elbow flexion, forced inspiration and finally fo

A 1-year prospective analysis of injuries in amateur and elite fistball.

The purpose of this study was to examine the incidence and mechanisms of acute injuries in the sport of fistball. No scientific studies on injury characteristics have yet been conducted in this traditional sport game. The study was conducted prospectively over the course of 12 months. During a total of 40.308 h of sport-specific exposure, 240 players reported 492 injuries, representing an overall injury rate of 12.2 injuries/1000 h of exposure. Most injuries were classified as bagatelle injuries (67.8%). The majority of the injuries were located in the knee (23.5%) followed by the elbow (11.9%

Non-Medial Ulnar Collateral Ligament Elbow Pathology in the Thrower: Valgus Extension Overload, Osteochondritis Dissecans, Olecranon Stress Fracture, and Ulnar Nerve.

Injury rates in baseball players reach as high as 5.8 per 1000 at the professional level. Recent data have singled out the elbow as the leading cause of injured list necessitating injuries and the costliest injury location in professional baseball. While the medial ulnar collateral ligament (MUCL) has gained the largest notoriety in this population, elbow injury in the thrower occurs as a spectrum of pathologies. These pathologies may encompass other injuries often resulting from or occurring in conjunction with MUCL insufficiency. These conditions include valgus extension overload syndrome, o

Thorough Debridement Facilitated by Timely Planning of One-Bone Forearm Surgery: A Report of 2 Cases.

CASES: Two patients with complex open forearm fractures underwent initial treatment with radioulnar fusion (arthrodesis of the radius to the ulna)-one patient sustained elbow and forearm injuries in a car accident and the other had an elbow injury caused by conveyor belt entanglement. CONCLUSIONS: Differentiating viable tissue from nonviable tissue is important in the management of potentially contaminated wounds. Arthrodesis of the radius to the ulna is generally considered a salvage option for complicated forearm instability. The decision to perform arthrodesis of the radius and ulna in the

Imaging acute trauma of the elbow.

Elbow injuries, both acute and chronic, continue to rise as both the young and elderly increase their participation in athletic activities. The role of imaging is to provide supportive data as to the cause of the patient's symptoms and to guide treatment options for the referring physician. Understanding the anatomy and biomechanics of this sophisticated joint, various injury patterns, and the implication of injury to the static and dynamic stabilizers will result in improvement in diagnostic accuracy. Each of these topics are discussed to provide a foundation and overview of key concepts nece…

Technique of the double nerve transfer to recover elbow flexion in C5, C6, or C5 to C7 brachial plexus palsy.

In C5, C6, or C5-to-C7 root injuries, many surgical procedures have been proposed to restore active elbow flexion. Nerve grafts or nerve transfers are the main techniques being carried out. The transfer of ulnar nerve fascicles to the biceps branch of the musculocutaneous nerve is currently proposed to restore active elbow flexion. Recovery of biceps muscle function is generally sufficient to obtain elbow flexion. However, the strength of elbow flexion is sometimes weak because the brachialis muscle is not reinnervated. Therefore, the transfer of 1 fascicle of the median nerve to the brachiali

Factors Influencing Time to Return of Elbow Motion in Completely Displaced and Flexion Type Pediatric Supracondylar Humeral Fractures.

BACKGROUND: Factors influencing the time to return of elbow motion have not been reported in extension-type III/IV and flexion-type supracondylar humeral (SCH) fractures. This study aims to identify factors influencing the time to return of elbow motion following operative pinning of these injuries. METHODS: An IRB-approved retrospective study was conducted of patients <18 years old treated at a large tertiary children's hospital from January 1, 2013 to July 31, 2022 for operatively pinned, completely displaced, and flexion-type SCH fractures. We excluded other SCH fracture types, patients un

The Relationship of Throwing Arm Mechanics and Elbow Varus Torque: Within-Subject Variation for Professional Baseball Pitchers Across 82,000 Throws.

BACKGROUND: Likely due to the high level of strain exerted across the elbow during the throwing motion, elbow injuries are on the rise in baseball. To identify at-risk athletes and guide postinjury return-to-throw programs, a better understanding of the variables that influence elbow varus torque is desired. PURPOSE: To describe the within-subject relationship between elbow varus torque and arm slot and arm rotation in professional baseball pitchers. STUDY DESIGN: Descriptive laboratory study. METHODS: A total of 81 professional pitchers performed 82,000 throws while wearing a motusBASEBALL…

Plate Osteosynthesis for Segmental Forearm Fractures: A Comparison of Outcomes in Floating Elbow and Isolated Forearm Fractures.

BACKGROUND: Segmental forearm fractures are challenging to treat due to their instability. A floating elbow, characterized by ipsilateral fractures of both the humerus and forearm, is a rare but severe injury often caused by high-energy trauma and associated with significant soft tissue damage. The objective of this study is to compare the surgical outcomes of plate osteosynthesis in segmental forearm fractures, with and without the concomitant presence of a floating elbow. METHODS: A total of 35 adult patients who underwent open reduction and internal fixation for segmental forearm fractures

[Isolated elbow dislocation in pediatric patients: non-operative treatment and complications associated with an infrequent pathology. Series of 4 cases].

Even though the elbow is the most often dislocated joint in children, this injury accounts for 3-6 % of elbow pathology. Dislocations without associated fractures are extremely rare. They result from a fall onto an outstretched hand. The patient is always referred with a painful joint, movement impairment and even clinical deformity. Acute treatment aims to achieve quick reduction and adequate joint stability, avoiding neurovascular injuries. We sought to analyze the functional outcomes and the complications after non-operative treatment. Our 4 patients Luxaciones puras de codo en pacientes pe

Elbow problems in the athlete.

Athletes expose their elbows to significantly high forces, in addition to multiple repetitive motions. Athletic injuries about the elbow include ligamentous, tendon, articular, and nerve injuries. The proper diagnosis requires a thorough knowledge of the anatomy, pathomechanics, and physical findings associated with these injuries. Treatment entails a methodical progression from nonoperative to operative intervention.

Open Reduction and Pinning of Lateral Condyle Fractures.

Lateral condyle fractures are the second most common elbow fracture in pediatric patients. Unlike supracondylar humerus fractures, these injuries involve the articular surface and present unique challenges. These injuries are graded based on the degree of displacement and this dictates your treatment. Both closed and open techniques are appropriate depending on the degree of displacement. Our preferred technique for open reduction and internal fixation is described and illustrated in the accompanying video. Key Concepts•Lateral condyle fractures are common elbow injuries in the pediatric popul…

Common pediatric elbow fractures.

Fractures of the elbow are a very common injury in children. The most common mechanism of injury is a fall on an outstretched upper extremity during play. Ranging in complexity from low-energy nondisplaced occult fractures to high-energy fractures with associated severe soft-tissue and neurovascular injuries, elbow fractures are a challenging problem for all pediatric healthcare providers. Because of the wide spectrum of fracture severity and associated bony and ligamentous injury, a very diverse spectrum of treatment modalities is necessary for optimal results. Management is based on fracture

[Inline skating--typical injuries and prevention].

In a field study we evaluated 500 inline skaters and documented the risk for injury as well as active and passive prophylactic methods. Inline-skating carries high risk for injuries. A total of 41% of the 500 examined subjects got injured. 73% of the injuries attacked soft tissue, 22% of the subjects suffered in distorsions and 5% fractures. 24% of the 222 injured people required specific therapy. Most of the injuries occur at the upper extremities. The fingers, wrist or elbow were involved in 78% of the fractures and 58% of the distorsions. Only every second skater has skating skills that all

Afferent Innervation, Muscle Spindles, and Contractures Following Neonatal Brachial Plexus Injury in a Mouse Model.

PURPOSE: We used an established mouse model of elbow flexion contracture after neonatal brachial plexus injury (NBPI) to test the hypothesis that preservation of afferent innervation protects against contractures and is associated with preservation of muscle spindles and ErbB signaling. METHODS: A model of preganglionic C5 through C7 NBPI was first tested in mice with fluorescent axons using confocal imaging to confirm preserved afferent innervation of spindles despite motor end plate denervation. Preganglionic and postganglionic injuries were then created in wild-type mice. Four weeks later,

Evaluation of the PhySens as a Wrist-Worn Wearable in Pitch Detection and Biomechanical Workload Estimation.

BACKGROUND: The volume and frequency of throwing activity are among the most significant risk factors for developing overuse injuries in youth athletes. Despite introducing systematic guidelines for 'pitch counts,' throwing injuries continue to rise. Using technology to create enhanced measures of workload exposure in this unique population of athletes may help generate more effective and personalized injury prevention strategies. HYPOTHESIS: The wrist-worn sensor system (PhySens) will: 1) accurately detect and differentiate throwing activity from other baseball movements, and 2) accurately p

Detection of traumatic elbow arthrotomies: computed tomography scan vs. saline load test.

BACKGROUND: Traumatic elbow arthrotomies are common injuries evaluated for by orthopedic services; however, failed identification of a traumatic arthrotomy leads to a high risk of developing septic arthritis. Currently these injuries are evaluated by either a saline load test or a computed tomography (CT) scan, yet there is little published evidence regarding detection of traumatic elbow arthrotomies. HYPOTHESIS: In our study, we hypothesized better sensitivity and specificity of detecting a traumatic elbow arthrotomy with a CT scan over a saline load test. STUDY DESIGN: Descriptive cadaveri

Distal Triceps Rupture with an Injury of the Medial Ulnar Collateral Ligament: a Result of a Fall Onto an Outstretched Arm - a Case Series.

Traumatic triceps tendon ruptures are rare and known to result in substantial disability unless appropriate surgical treatment is performed. A traumatic rupture can occur due to a fall onto the outstretched hand. Tearing of the triceps tendon results in a valgus load onto the elbow, which can lead to injuries to the radial head/neck and/or the ulnar collateral ligament. Hence, attention must be paid to associated pathologies after diagnosis of rupture to the distal triceps tendon. Our surgical procedure in these cases includes diagnostic arthroscopy to detect concomitant injuries. In the follo

Physical examination of the athlete's elbow.

Elbow injury is encountered less frequently than are other joint conditions. The bony architecture, muscle, ligament, and nerve anatomy are complex, and the forces leading to injury in the athlete's elbow are unique. Appreciating the pathomechanics leading to injury and a detailed knowledge of elbow anatomy are the foundation for conducting a directed history and physical examination that achieves an accurate diagnosis. Recent advances in physical examination have improved our ability to accurately diagnose and treat athletic elbow disorders. This article reviews general and focused physical e
