Activation of the inspiratory intercostal muscles by electrical stimulation of the spinal cord.

Electrical stimulation of the spinal cord was evaluated as a method of activating the inspiratory intercostal muscles. Studies were performed in anesthetized dogs after hyperventilation-induced apnea. A stainless steel electrode, rubberized along its entire length except for 2 to 3 mm at the distal tip, was introduced epidurally onto the dorsal surface of the thoracic spinal cord. Stimulating electrodes were also placed in each hemidiaphragm. Intercostal electromyograms, inspired volume, and thoracoabdominal movements were monitored. The inspiratory capacity was determined in each animal as th

Transcutaneous electrical nerve stimulation following appendicectomy: the placebo effect.

A controlled trial was undertaken to compare the efficacy of transcutaneous electrical nerve stimulation (TENS) with standard intramuscular opiate analgesia in the management of postoperative pain following appendicectomy. Consecutive patients undergoing emergency appendicectomy were randomised into control, sham TENS and active TENS groups. There was a significant decrease in pain severity and analgesic intake in both active and sham TENS groups when compared with the control group (P less than 0.01). No difference was demonstrated in pain severity between active and sham TENS groups but the

Electrical stimulation applied to bone and nerve injuries in the upper extremity.

In conclusion, electrical stimulation of bone has advanced from the laboratory to clinical reality. Despite the lack of good double-blind clinical studies, it is impossible to ignore the excellent results reported from numerous multicenter trials. Doubts and controversies will and should continue. Electrical stimulation has a definite place in the treatment of scaphoid nonunion as well as other failures of osteogenic biology in the upper extremity. The future may realize the enormous potential of electrical stimulation in areas of nerve repair, wound healings, or osteoporosis. The hand surgeon

Evaluation of cutaneous sensation in myelodysplastic children, using electrical stimulation.

A new method, cutaneous electrical stimulation, was compared with the standard neurological examination for its effectiveness in detecting cutaneous sensation. Both methods provided clear and congruous results for 45 per cent of the patients. However, for the other 55 per cent, cutaneous electrical stimulation frequently detected sensation at sites which were classified as anaesthetic on standard neurological examination. Electrical stimulation also was able to indicate sensation in a group of infants too young to be evaluated by standard neurological evaluation.

Respiratory rhythmically regulated electrical stimulation of paralyzed laryngeal muscles.

Electrical stimulation of the posterior cricoarytenoid muscle, synchronized with inspiration, was achieved in dogs, utilizing a radio frequency stimulus triggered by a chest wall expansion transducer. This system brings about the abduction of the paralyzed vocal cord for the entire duration of inspiration, which allows a normal flow of air through the larynx. The stimulation system could be tested successfully in actual experiments in dogs with artificial paralysis of the recurrent laryngeal nerve. Its effectiveness could be proved through observation of the vocal cord movements (photographic

Motor effects of electrical and cholinergic stimulation of the cat's dorsal hippocampus.

The effect of electrical stimulation of the dorsal hippocampus was studied in 17 adult cats with implanted electrodes and those effects of carbachol and dioxolane in a group of ten adult cats with a cannula and electrodes implanted in the above cited structure. Electrical stimulation induced a contralateral head-eye-body turning response in 3 cats (17.6%), only when it was associated with afterdischarge. On the other hand the cholinergic agonists evoked contralateral head-eye-body turning in nine out of ten cats in whom the injections were administered into the hippocampus. The fact that dioxo

Influence of electrical vagal stimulation and acetylcholine on the function of the feline gallbladder.

The effects of electrical stimulation of the vagus nerves on the function of the feline gallbladder and hepatic bile outflow were studied with a perfusion technique in vivo. After elimination of the muscarinic receptors with atropine, efferent stimulation of the cut vagus nerve in the neck relaxed the gallbladder, reduced the net water absorption rate across its wall, and increased the bile outflow from the liver. The results imply that the concentrating function of the gallbladder and the bile formation in the liver are under regulatory control by noncholinergic, nonadrenergic nerve fibres in

Combined electrical stimulation of the periaqueductal gray matter and sensory thalamus.

11 patients with chronic intractable pain of at least 3 years' duration underwent a morphine infusion test, the results of which suggested a syndrome of superimposed somatogenic and neurogenic pain components. They then underwent stereotactic implantation of a dual-channel brain stimulation system with two brain electrodes, one in the left periaqueductal gray matter (PAG) and the other in the sensory thalamus contralateral to the neurogenic pain. Using this system, all patients have obtained excellent simultaneous relief of both pain components (follow-up 12-36 months). The findings support a

Temporary tinnitus suppression in man through electrical stimulation of the cochlea.

While investigating the possibilities of electrically induced hearing in cases of profound deafness, we observed that very often an appropriate stimulation of the cochlea could result in tinnitus cancellation. We present here a progress report on that phenomenon, including 15 subjects. Such a cancellation was restricted to tinnitus subjectively localized in the ear under test, we observed no effect on noises localized in the other ear, or centrally. This suppression occurred so long as the cochlea was stimulated and stopped when the stimulation ceased. This phenomenon was very reproducible and

The declining electrical response of muscle to repetitive nerve stimulation in myotonia.

The electrical response of muscle to repetitive nerve stimulation was studied in patients with various myotonic disorders. A decrementing response was common but not invariable finding, and was unrelated to the severity or diagnosis. The decrement either continued throughout the period of stimulation or "leveled off", sometimes being followed by an increment. If it occurred at low rates of stimulation, a greater decrement occurred at higher rates, usually after a shorter latent period. It was not related consistently to the presence of weakness, but in patients with myotonia congenita it was m

Neurogenic cerebral vasodilation from electrical stimulation of the cerebellum in the monkey.

The effect of stimulation of fastigial nucleus (FN) on cerebral blood flow (CBF) was examined in anesthetized, paralyzed monkeys. CBF was measured with a flow transducer chemically implanted on the left common carotid artery. The left external carotid artery and its branches were ligated. Electrical stimulation of the fastigal nucleus produced rapid (within two seconds) increases in arterial pressure (AP) and heart rate. Mean carotid blood flow (MCBF) rose 77% (+/- 40% SEM) above control and followed the increases in mean arterial pressure (MAP). Calculated cerebrovascular resistance decreased

Selection of patients with urinary incontinence for application of functional electrical stimulation.

Contradictory data from the literature along with our own experience using functional electrical stimulation (FES) have facilitated the determination of objective criteria for the application of FES in correcting urinary incontinence. Simultaneous urodynamic, neurophysiological, and radiological examinations employed during our studies enabled us to determine changes in these parameters due to FEX. Among a large number of patients with urinary incontinence (for example: paraplegics, patients with spina bifida, stress incontinence following prostate operations, or resistant enuresis) we have de

Modification of blood flow to the extremities by electrical stimulation of the nervous system.

Sixteen patients who had electrical stimulation applied to various portions of the nervous system were examined for increase in blood flow to the extremities. Clinical observations and a one-channel plethysmograph were used to measure arterial dilatation. Seven patients had transcutaneous stimulation applied over the cervical or thoracic spinal cord, peripheral nerves, or low lumbar region; eight had electrical stimulators implanted over the spinal cord in attempts to relieve intractable pain or some of the symptoms of multiple sclerosis; and one patient had electrical stimulators implanted ov

Control factors in the release of gastrin by direct electrical stimulation of the vagus.

The release of gastrin by direct electrical stimulation of the vagus was studied together with the relative effects on the response of antral and duodenal acidification. As expected, gastrin levels increased to three times the normal simulated response following antral neutralization. In contrast, duodenal acidification failed to influence the vagal release of gastrin when the antrum was neutralized although it had a minor effect when the antrum was acidified. Thus the antral pH dominates over duodenal pH as a factor in controlling gastrin release. Surprisingly, atropine in doses which blocked

Peroneal nerve conduction velocity after chronic electrical stimulation.

The effects of electric stimulation on nerve conduction velocity of the peroneal nerve via surgically implanted electrodes was evaluated in a group of nine hemiplegic patients. After a mean duration of 25 months nerve conduction velocity averaged 47.3 meters/sec in the uninvolved and 46.1 meters/sec in the stimulated (paretic) extremity. In a control group of ten hemiplegic subjects who did not receive stimulation, nerve conduction velocity averaged 43.1 meters/sec in the hemiplegic leg, and 41.3 meters/sec in the uninvolved leg. These differences were not statistically significant. Maximum do…
