Optimal electrode position for transvenous defibrillation: a prospective randomized study.

OBJECTIVES: This study was performed to determine the optimal position for the proximal electrode in a two-electrode transvenous defibrillation system. BACKGROUND: Minimizing the energy required to defibrillate the heart has several potential advantages. Despite the increased use of two-electrode transvenous defibrillation systems, the optimal position for the proximal electrode has not been systematically evaluated. METHODS: Defibrillation thresholds were determined twice in random sequence in 16 patients undergoing implantation of a two-lead transvenous defibrillation system; once with the

Multifactorial determinants of auditory outcomes in pediatric cochlear implantation: A long-term longitudinal study.

INTRODUCTION: Early cochlear implantation (CI) is considered a key determinant of auditory development in children with severe-to-profound sensorineural hearing loss. However, long-term outcomes remain highly variable and still incompletely understood. This study aimed to evaluate the long-term impact of age at implantation and other modifying variables in a large pediatric CI cohort. METHODS: A retrospective longitudinal cohort study was conducted including 588 implanted ears from 494 children aged ≤10 years. Speech recognition scores (SRS) at 65 dB SPL were assessed longitudinally up to 10

Stimulation-Stability and Preclinical Bench Testing of a New Auditory Nerve Interface.

OBJECTIVE: Cochlear implants are the flagship sensory neural prosthetic, enabling improved hearing and speech perception in >1 million patients with hearing loss \cite{zengCelebratingOneMillionth2022}.Despite over three decades of active work and innovations in cochlear implants, hearing performance for complex tasks such as speech-in-noise and music appreciation are still quite limited, putatively due to the challenge of selectively stimulating the auditory nerve with intracochlear electrodes through the bony cochlear wall. Our approach is to more selectively evoke hearing percepts through the

[Appropriate procedure for pacemaker and ICD malfunction].

Malfunctions in implanted pacemaker and defibrillator systems can affect all components of the device (battery, capacitor, central processing unit (CPU), connector, soldering points, electrodes). The resulting problems can manifest themselves as inadequate therapy, a lack of therapy despite the need for stimulation or shock release in case of ventricular tachyarrhythmia, or as an ineffective delivery of therapy. The high number of implanted pacemaker and defibrillator systems, the increased life expectancy and the technical progress of cardiac "device" therapy mean that the treating physician

Fence-post-like Stereoelectroencephalography Electrode-guided Focus Resection Technique: Technical Note.

Stereoelectroencephalography electrodes are widely used to identify the epileptogenic zone. When performing resection of the epileptogenic zone identified by intracranial electroencephalography using stereoelectroencephalography electrodes, accurate delineation of the resection boundaries is critical for complete removal while preserving neurological function. However, intraoperative brain shifts often make it difficult to identify the resection boundaries. To address this challenge, we aimed to develop a novel surgical approach, the fence-post-like stereoelectroencephalography electrode-guide

In vivo microstimulation stability of electrodeposited platinum-iridium on carbon fiber electrodes.

Carbon fiber electrodes (CF) have proven effective at recording neural activity at single cell resolution. CF elicit a minimal foreign body reaction due to their small size, thus the longevity of recording is extended vs. traditional electrodes. However, microstimulation with carbon fibers is not possible chronically, due to the limited ability of carbon to transfer stimulus charge without degradation. Electrodeposited platinum iridium alloy (PtIr) has been applied to carbon to enable microstimulation. Studies to date have shown durability of PtIr in vitro. Here, we show that PtIr is robust to

Chronic measurement of the stimulation selectivity of the flat interface nerve electrode.

The flat interface nerve electrode (FINE) is an attempt to improve the stimulation selectivity of extraneural electrodes. By reshaping peripheral nerves into elliptical cylinders, central fibers are moved closer to the nerve-electrode interface, and additional surface area is created for contact placement. The goals of this study were to test the hypothesis that greater nerve reshaping leads to improved selectivity and to examine the chronic recruitment properties of the FINE. Three FINEs were developed to reshape peripheral nerves to different degrees. Four electrodes of each type were implan

Strategy of pacemaker and electrode replacement for superior vena cava stenosis.

Cardiac implantable devices are commonly used for superior vena cava stenosis, but there have been few reports of electrode replacement in the stenosed superior vena cava. A 73-year-old man was diagnosed with second-degree type II atrioventricular block and a permanent dual-chamber, rate-modulated pacing pacemaker was implanted 10 years previously. Because of depletion of the pacemaker battery and an increase in the ventricular pacing threshold, replacement of the pacemaker and ventricular electrode was required. During the operation, we found that the patient had severe superior vena cava ste

Minimal access cochlear implant fixation: temporalis pocket with a plate.

OBJECTIVE: Minimal access approaches for cochlear implants have recently gained popularity, offering a smaller incision and minimal hair shave. The current generation of thinner internal receiver stimulators is adaptable for these approaches. However, conventional bony fixation of the internal receiver stimulator is difficult through this limited exposure, and some minimal access techniques rely on soft tissue fixation only. Inadequate fixation can result in device migration, extrusion, and electrode migration. We compare wound complications and electrode migration for a group undergoing conve

Chronic electrocorticography for sensing movement intention and closed-loop deep brain stimulation with wearable sensors in an essential tremor patient.

Deep brain stimulation (DBS) has become a widespread and valuable treatment for patients with movement disorders such as essential tremor (ET). However, current DBS treatment constantly delivers stimulation in an open loop, which can be inefficient. Closing the loop with sensors to provide feedback may increase power efficiency and reduce side effects for patients. New implantable neuromodulation platforms, such as the Medtronic Activa PC+S DBS system, offer important data sources by providing chronic neural sensing capabilities and a means of investigating dynamic stimulation based on symptom

Central effects of occipital nerve electrical stimulation studied by functional magnetic resonance imaging.

OBJECTIVE: To study the central effects of occipital nerve stimulation (ONS) using functional magnetic resonance imaging (fMRI). MATERIALS AND METHODS: After phantom measurements, blocked design fMRI scanning was performed during intermittent ONS in a healthy volunteer with implanted electrodes connected to an external generator. To assess the effect of frequency and stimulation mode, seven different frequencies in either tonic or burst mode were generated by a neurostimulator. RESULTS: A qualitative analysis of the main effect of ONS demonstrated significantly decreased activity within the

Intracochlear factors contributing to psychophysical percepts following cochlear implantation.

The performance of cochlear implant patients may be related to intracochlear, histopathological factors. We have performed detailed post-mortem examinations of five human, implanted cochleas and for each electrode correlated the psychophysical threshold, comfortable level and dynamic range with spiral ganglion cell survival, presence of fibrous tissue and/or new bone, and distance between the centers of the electrode bands and Rosenthal's canal. The psychophysical parameters were strongly interrelated. Threshold and comfort levels correlated with the distance between the electrodes and Rosenth

Comparative assessment of power dynamics of gastric electrical activity.

This study was undertaken to investigate the correlation between power dynamics of gastric electrical activity (GEA) assessed with different recording techniques. A total of seven eight-channel 1-hr combined recordings were obtained from three subjects in five consecutive postoperative days. Four channels were recorded from bipolar electrodes implanted into the gastric antral wall, and four channels were electrogastrographic (EGG). Six pairs of bipolar electrodes were inserted into the antral wall (three anterior; three posterior) of 16 anaesthetized dogs. Fourteen-channel (six internal GEA an

Electrical Interaction between Implantable Vagus Nerve Stimulation Device and Implantable Cardiac Rhythm Management Device.

BACKGROUND: Autonomic regulation therapy via vagus nerve stimulation (VNS) was recently approved as a therapy for chronic heart failure, and will likely be utilized in patients who are also indicated for cardiac rhythm management device implantation. This study is designed to assess the degree to which VNS is likely to cause interference in the cardiac sensing of an implantable cardiac rhythm management device. METHODS: A VNS stimulation lead and a cardiac sensing lead were placed in a simulated biological medium. A nonconductive carrier frame was used to position the leads at a precise elect

Subcutaneous mechano-electrocardiogram (MECG) sensor for complementary cardiac diagnosis.

Since the heart pumps out the blood through the excitation-contraction coupling, simultaneous monitoring of the electrical and mechanical characteristics is beneficial for comprehensive diagnosis of cardiac disorders. Currently, these characteristics are monitored separately with electrocardiogram (ECG) and medical imaging techniques. This work presents a fully implantable device named mechano-electrocardiogram (MECG) sensor that can measure mechanocardiogram (MCG) and ECG together. The key to the success is fabrication of permeable electrodes on a single low-modulus porous nanofiber mat, whic

Temporary gastric electrical stimulation with orally or PEG-placed electrodes in patients with drug refractory gastroparesis.

BACKGROUND: Gastric electrical stimulation (GES) has been shown to be efficacious for drug refractory gastroparesis, but GES requires surgery. Placement of temporary GES electrodes endoscopically (ENDOstim) or via a PEG (PEGstim) is feasible, thereby allowing rapid assessment and comparison of temporary use (TEMP) with permanent (PERM) implantation. METHODS: Twenty consecutive patients with gastroparesis had TEMP electrodes placed (6 ENDOstim, 14 PEGstim). TEMP alone and TEMP vs. PERM placement of GES devices in 13 of 20 patients were compared via the following: average vomiting frequency sco

Neurobiological Mechanisms of Metacognitive Therapy - An Experimental Paradigm.

INTRODUCTION: The neurobiological mechanisms underlying the clinical effects of psychotherapy are scarcely understood. In particular, the modifying effects of psychotherapy on neuronal activity are largely unknown. We here present data from an innovative experimental paradigm using the example of a patient with treatment resistant obsessive-compulsive disorder (trOCD) who underwent implantation of bilateral electrodes for deep brain stimulation (DBS). The aim of the paradigm was to examine the short term effect of metacognitive therapy (MCT) on neuronal local field potentials (LFP) before and
