Anterior spinal artery syndrome complicated by the ondine curse.

BACKGROUND: Anterior spinal artery (ASA) syndrome results in motor palsy and dissociated sensory loss below the level of the lesion, accompanied by bladder dysfunction. When the cervical spine is involved, breathing disorders may be observed. OBJECTIVE: To describe the polysomnographic findings in a patient with cervical ASA syndrome complicated by a sleep breathing disorder. SETTING: Unit of neurology at a sleep center. Patient A 30-year-old man had an ischemic lesion that affected the anterior cervical spinal cord (C2-C6) bilaterally because of an ASA thrombosis. He developed ASA syndrome

Retinal pigment epithelial dysfunction in patients with pigment dispersion syndrome: implications for the theory of pathogenesis.

OBJECTIVE: To test the hypothesis that the retinal pigment epithelial/photoreceptor complex is affected in patients with pigment dispersion syndrome and/or in patients with pigmentary glaucoma. METHODS: Electro-oculograms were recorded from patients with pigment dispersion syndrome, pigmentary glaucoma, ocular hypertension, and primary open-angle glaucoma and from control subjects. Electro-oculograms were recorded during 15 minutes of dark adaptation followed by 15 minutes of light adaptation. For each subject, dark-trough amplitudes, dark-trough latencies, light-peak amplitudes, light-peak l

Adaptive changes of saccadic eye-head coordination resulting from altered head posture in torticollis spasmodicus.

We asked whether and how the abnormal head posture in torticollis patients affects saccadic gaze shifts and impairs the associated head movements. We wanted to learn to what extent observed changes directly result from the disease or reflect compensatory mechanisms, secondary to the altered head posture. We compared the results of patients with those of normal subjects. When patients viewed a centric target, their heads were a priori deviated in the direction of the torticollis, with orbital eye position showing a compensatory offset in the opposite direction. These abnormal eye and head posit

Electrophysiological correlates of human intrasaccadic processing.

Visual discrimination performance is thought to be suppressed during saccades in order to contribute to space constancy. However, under certain experimental conditions, visual inhibition may not take place, suggesting a more complex underlying mechanism. We tested the discrimination ability of 20 healthy subjects during visually guided horizontal saccades and recorded simultaneously the evoked brain activity from 30 channels over occipital, parietal, and temporal areas. During the execution of saccadic eye movements, visual stimuli were presented for 30 ms. In order to prevent retinal afterima

Electrophysiological abnormalities in age-related macular degeneration.

BACKGROUND: Results from psychophysical tests indicate that in age related macular degeneration (ARMD) retinal function is globally impaired. Observations in single ARMD cases showing abnormalities of the electrooculogram (EOG) and electroretinogram (ERG) encouraged us to systematically examine ARMD patients by means of electrophysiological tests to confirm a global retinal dysfunction in ARMD. METHODS: Sixty-six patients with ARMD were examined clinically and by means of EOG, ERG, triple scotopic ERG, and fluorescein angiography (FA). RESULTS: EOG data were considerably reduced in ARMD. Sco

Neuronal activity in the cholinoceptive basal forebrain of freely moving narcoleptic dobermans.

Cholinergic stimulation in the basal forebrain (BF) triggers cataplexy in canine narcolepsy. Extracellular single unit recordings in the BF were carried out in freely moving narcoleptic dogs to study the neuronal mechanisms mediating cataplexy induction in the BF. Among the 64 recorded neurons, 12 were wake-active, three were slow wave sleep (SWS)-active, 17 were wake-/REM-active, 11 were REM sleep-active, three were cataplexy-active, and the other 18 were state-independent. Systemic administration of physostigmine, a cholinesterase inhibitor, induces status cataplecticus, decreases SWS and in

Vertical gaze palsy induced by midbrain lesions and its structural imaging.

We experienced four cases of vertical gaze palsy induced by midbrain lesions. Lesions commonly covered the rostral midbrain, including the rostral interstitial nucleus, dorsomedial to the red nucleus. Two of the four cases resulted from vascular insult, in which a single, unpaired perforator is supposed to innervate the rostral midbrain and medial thalamus bilaterally. One case showed vertical gaze palsy accompanied by bilateral ptosis. The findings agree with recent experimental evidence that a neural substrate in eyelid control lies in the supraoculomotor area immediately dorsal to the oculo

Arousals and nocturnal respiration in symptomatic snorers and nonsnorers.

The purpose of the present investigation was to examine the relationship among upper airway resistance, snoring, and arousals, all measured simultaneously, in nonapneic snorers complaining of excessive daytime sleepiness (EDS). To accomplish this task, we selected a group of nine nonapneic snorers who presented because of snoring, EDS, tiredness, or fatigue. Ten healthy, alert, nonapneic, nonsnoring subjects recruited from among the hospital personnel acted as a comparison group. All subjects had nocturnal polysomnography, which included measurements of snoring, nasal and pharyngeal airway pre

Centrally administered ouabain aggravates central sleep apneas.

The presence of endogenous digitalis-like factors in the central nervous system suggests their functional significance in the central nervous system. Three-day infusions of three-stepped doses of the digitalis agent ouabain (1-100 ng.kg body wt-1.h-1) into the lateral cerebral ventricle of freely moving rats caused a dose-dependent increase in the number of central-apneic episodes during rapid-eye-movement sleep without affecting the time spent in rapid-eye-movement sleep or basic respiratory rate. These results suggest that endogenous digitalis-like factors may be involved in the genesis of c

Electroencephalographic sleep and mood during cocaine withdrawal.

We report on nine patients between the ages of 21 and 39 years who were admitted to an inpatient substance abuse treatment unit for cocaine treatment. The patients' sleep was studied in the laboratory for 4 nights during the first week, and 2 nights during the second and third weeks of their hospitalization. Daily mood ratings, cocaine craving scores and sleep logs were also recorded on each patient. During the first week of withdrawal, these patients had a markedly shortened REM latency, an increased REM sleep percentage, a very high REM density and a long total sleep period time. During the

Dissociations between the effects of hallucinogens on behavior and raphe unit activity in behaving cats.

The hypothesis that hallucinogenic drugs exert their behavioral effects by an action at pre- or postsynaptic serotonin receptors was evaluated by co-administering various drugs that possess either serotonin agonist or antagonist properties, while concurrently monitoring behavior and the electrophysiological activity of serotonin-containing dorsal and median raphe neurons in freely moving cats. Co-administration of the serotonin receptor blockers, metergoline or mianserin, with lysergic acid diethylamide (LSD) produced no change in the inhibitory effects of LSD on raphe neurons, but produced a

[Effects of cyaninoside chloride and Heleniene on mesopic and scotopic vision in myopia and night blindness].

A controlled, clinical trial, comparing cyaninoside chloride and Heleniene , was conducted on 31 out-patients suffering from functional disturbances of vision in low-luminance conditions. The evolution of photopic and mesopic visual acuities, electro- oculograms and adapto -electroretinograms was assessed for both treatment groups and controls. Both agents significantly improved photopic visual acuity (p less than 0.05). Only cyaninoside chloride treatment improved visual functions related to mesopic and scotopic vision (p less than 0.01). There were also significant differences between the tw

Effective Diagnosis of Various Sleep Disorders by LEE Classifier: LightGBM-EOG-EEG.

A novel method is proposed for diagnosing the sleep disorders of insomnia, narcolepsy, periodic leg movement syndrome, nocturnal frontal lobe epilepsy, rapid eye movement behavior disorder, and sleep-disordered breathing. We use the light gradient boosting decision tree model (LightGBM) for the classification of healthy controls and different sleep disorders. The proposed approach is evaluated on the publicly available CAP dataset of 108 subjects. The LightGBM classifier using only an electrooculogram (EOG) channel (L-EOG) achieves a seven-class classification accuracy of 83.3%. This performan

Automatic sleep staging: from young adults to elderly patients using multi-class support vector machine.

Aging is a process that is inevitable, and makes our body vulnerable to age-related diseases. Age is the most consistent factor affecting the sleep structure. Therefore, new automatic sleep staging methods, to be used in both of young and elderly patients, are needed. This study proposes an automatic sleep stage detector, which can separate wakefulness, rapid-eye-movement (REM) sleep and non-REM (NREM) sleep using only EEG and EOG. Most sleep events, which define the sleep stages, are reduced with age. This is addressed by focusing on the amplitude of the clinical EEG bands, and not the affect
