Delirium in older adults in the emergency department: a scoping review of current research and identified knowledge gaps.

BACKGROUND: Delirium is a common neurocognitive syndrome among older adults, associated with increased morbidity, mortality, and healthcare costs. Emergency departments (EDs) represent a critical point for early detection and prevention, yet delirium often remains under-recognized in this setting. Contributing factors include overcrowding, sensory overload, and lack of orientation aids, combined with demographic shifts leading to more acutely admitted older adults with complex needs. This scoping review aimed to map existing research on delirium in elderly patients in EDs and identify knowledg

Emergency medicine updates: Pediatric brief resolved unexplained event.

INTRODUCTION: Brief resolved unexplained events (BRUEs) can occur in a large number of pediatric patients, with most having a benign etiology. However, in many cases, these can result in unnecessary evaluations and admissions among low-risk patients. Therefore, it is critical for clinicians to be aware of the current evidence regarding BRUE in the emergency department (ED) setting. OBJECTIVE: This paper evaluates key evidence-based updates concerning pediatric BRUE for the emergency clinician. DISCUSSION: A BRUE is defined as a clinical entity occurring in an infant less than 1 year of age t

Emergency medicine updates: Managing the patient with return of spontaneous circulation.

INTRODUCTION: Patients with return of spontaneous circulation (ROSC) following cardiac arrest are a critically important population requiring close monitoring and targeted interventions in the emergency department (ED). Therefore, it is important for emergency clinicians to be aware of the current evidence regarding the management of this condition. OBJECTIVE: This paper provides evidence-based updates concerning the management of the post-ROSC patient. DISCUSSION: The patient with ROSC following cardiac arrest is critically ill, including a post-cardiac arrest syndrome which may include hyp

Leveraging Artificial Intelligence and Data Science for Integration of Social Determinants of Health in Emergency Medicine: Scoping Review.

BACKGROUND: Social determinants of health (SDOH) are critical drivers of health disparities and patient outcomes. However, accessing and collecting patient-level SDOH data can be operationally challenging in the emergency department (ED) clinical setting, requiring innovative approaches. OBJECTIVE: This scoping review examines the potential of AI and data science for modeling, extraction, and incorporation of SDOH data specifically within EDs, further identifying areas for advancement and investigation. METHODS: We conducted a standardized search for studies published between 2015 and 2022,

All the Better to Taste You With, My Dear: ED Management of Mammalian Bites.

Mammal bites account for over 5 million visits to Emergency Departments (EDs) annually. Nurse Practitioners (NPs) need to stay abreast of current guidelines, changes to antibiotic regimens that are now most effective, and understand in what circumstances collaboration with other specialists is indicated. It is not enough to care for the wound, itself, but rather understand in what presentations additional care may be needed despite the fact that there is no clear evidence at the time of evaluation of the need for advanced care. Additionally, NPs should understand what resources are available w

Using Virtual Emergency Medicine Clinicians as a Health System Entry Point (Virtual First): Cross-Sectional Survey Study.

BACKGROUND: The COVID-19 pandemic accelerated the use and acceptance of telemedicine. Simultaneously, emergency departments (EDs) have experienced increased ED boarding. With this acceptance of telemedicine and the weighty increase in patient boarding, we proposed the innovative Virtual First (VF) program to leverage emergency medicine clinicians' (EMCs) ability to triage patients. VF seeks to reduce unnecessary ED visits by connecting patients with EMCs prior to seeking in-person care rather than using traditional ED referral systems. OBJECTIVE: The goal of this study is to investigate how p

Severity grading of dermatological emergencies based on comorbidities and systemic involvement: An observational study.

Dermatological emergency is defined as any urgent/immediate care. Dermatological conditions compromise about 5-8% of all cases presenting to the emergency department. A grading system can help dermatologist's and allied medical personnel to triage a patient accordingly. Currently no severity grading for dermatological emergencies is available. All patients seen in OPD for dermatological consultations requiring urgent interventions were included. Detailed history and clinical examination were done. Patients were assessed according to the onset, symptoms, distribution, examination, body surface

Deviation From National Dosing Recommendations for Children Having Out-of-Hospital Emergencies.

BACKGROUND: Previous evaluations of medication dosing variance for children in the prehospital setting have been limited regionally or to specific conditions. We sought to describe pediatric dosing deviations from nationally recommended guidelines for commonly administered medications from a registry of prehospital encounters. METHODS: We evaluated prehospital patient care records for children (<18 years) from approximately 2000 emergency medical services agencies from 2020 to 2021. We investigated dosing deviations (defined as being ≥20% of the weight-appropriate dose from national guideline

Intersectional characterization of emergency department (ED) staff experiences of racism: a survey of ED healthcare workers for the Disrupting Racism in Emergency Medicine (DRiEM) Investigators.

INTRODUCTION: The impact of racism on patient outcomes in Emergency Medicine has been examined but there have been few studies exploring the experiences of racism in health care workers. This survey aims to explore the experience of racism by interdisciplinary staff in a tertiary ED. By characterizing the staff experience of racism in the ED, we hope to inform the design of strategies to disrupt racism and ultimately improve the health and wellness of both staff and patients. METHODS: We conducted a self-administered, cross-sectional survey to explore the reported experience of racism by heal

Prognostication in Acute Neurological Emergencies.

BACKGROUND: For patients with acute, serious neurological conditions presenting to the emergency department (ED), prognostication is typically based on clinical experience, scoring systems and patient co-morbidities. Because estimating a poor prognosis influences caregiver decisions to withdraw life-sustaining therapy, we investigated the consistency of prognostication across a spectrum of neurology physicians. METHODS: Five acute neurological presentations (2 with large hemispheric infarction; 1 with brainstem infarction, 1 with lobar hemorrhage, and 1 with hypoxic-ischemic encephalopathy) w

Opioid-Related Risk Screening Measures for the Emergency Care Setting.

Efforts to minimize the impact of prescribed opioids on future adverse outcomes are reliant on emergency care providers' ability to screen and detect opioid use disorder (OUD). Many prescriptions are initiated in the emergency department (ED) for acute pain; thus, validated measures are especially needed. Our systematic review describes the available opioid-related screening measures identified through search of the available literature. Measures were categorized by intent and applied clinical setting. We found 44 articles, identifying 15 screening measures. Of these, nine were developed to sc

Approaching acute pain in emergency settings: European Society for Emergency Medicine (EUSEM) guidelines-part 1: assessment.

Pain management is a vital component of patient care, particularly in the emergency setting. Pain can hinder the opportunities to treat and manage pain-causing conditions and remains one of the primary reasons patients seek emergency medical care, yet despite this, pain often remains under-acknowledged, under-assessed and undertreated. Despite the importance of effective management of acute pain, there are currently no well-defined emergency medicine guidelines at a European level to support healthcare professionals in achieving this goal. Rickettsialpox in a Pediatric Patient.

Rickettsialpox is a rare vector-borne rickettsiosis that manifests in hosts living in urban populations, particularly in New York City. The following is a case discussion of a 12-year-old girl with history of fever and right-ankle swelling. She was initially evaluated and treated for cellulitis. She later developed a maculopapular rash completing the classic triad for rickettsialpox: black eschar, papular rash, and fever. Her convalescent rickettsial titers returned positive, confirming the diagnosis of rickettsialpox.

A study of bedside ocular ultrasonography in the emergency department.

UNLABELLED: The use of ocular ultrasonography for the evaluation of emergency patients has recently been described in the emergency medicine (EM) literature. There are a number of potential uses that may greatly aid the emergency physician (EP) and avoid lengthy consultation or other diagnostic tests. OBJECTIVE: To examine the accuracy of bedside ultrasonography as performed by EPs for the evaluation of ocular pathology. METHODS: This prospective, observational study took place in a high-volume, suburban community hospital with an EM residency program. All patients arriving with a history of

Procedural sedation and analgesia in the emergency department. Canadian Consensus Guidelines.

Procedural sedation and analgesia are core skills in emergency medicine. Various specialty societies have developed guidelines for procedural sedation, each reflecting the perspective of the specialty group. Emergency practitioners are most likely to embrace guidelines developed by people who understand emergency department (ED) skills, procedures, conditions, and case mix. Recognizing this, the Canadian Association of Emergency Physicians (CAEP) determined the need to establish guidelines for procedural sedation in the ED. In March, 1996, a national emergency medicine (EM) working committee,

Anorectal disorders.

Anorectal disorders are commonly encountered in the practice of emergency medicine. Most can be diagnosed and treated in the emergency department setting. Almost all anorectal disorders once diagnosed and treated in the emergency department need appropriate follow-up to ensure adequacy of treatment, for further possible diagnostic procedures (e.g., endoscopy, biopsy), or for definitive treatment. Hemorrhoids are the most prevalent anorectal disorder and are the most common cause of hematochezia. Treatment is dependent on the degree of hemorrhoid prolapse and symptoms. Most cases can be treated
