We treated a patient with birdshot retinochoroidopathy, an autoimmune eye disease. An autoimmune sensorineural hearing loss developed, probably due to endolymphatic hydrops. To our knowledge, the association of these two conditions has not been recorded previously.

Ménière's disease (MD) is a complex inner ear disorder characterized by a range of symptoms, with its pathogenesis linked to immune-related mechanisms. Our previous research demonstrated that IL-1β maturation and release can trigger cell pyroptosis, exacerbating the severity of the endolymphatic hydrops in a mouse model; however, the specific mechanism through which IL-1β influences MD symptoms remains unclear. This study conducted on patients with MD examined changes in protein signatures in the vestibular end organs (VO) and endolymphatic sac (ES) using mass spectrometry. Gene ontology and p…

OBJECTIVE: To investigate the vestibular endolymphatic hydrops in patients with semicircular canal malformation.
METHODS: We searched 7864 patients who underwent MR Imaging after contrast injection and found 21 patients with semicircular canal malformations. Another 9 non-malformed patients with unilateral hearing loss were randomly included. We asked patients about their medical history and measured the volume of total vestibular fluid space and endolymphatic space. The vestibular volume ratio = endolymphatic space/total fluid space × 100.
RESULTS: Hearing loss was observed in 18 of 30 malf…

PURPOSE: Intratympanic injection of gadolinium diethylenetriaminepentaacetic acid (Gd-DTPA) has been reported as a procedure to visualize endolymphatic hydrops of Meniere's disease. We frequently noted that cerebrospinal fluid (CSF) in the internal auditory canal (IAC) was also enhanced after this procedure. The purpose of this study was to evaluate how frequently this occurs and to investigate the specific features of patients who lack this communication.
MATERIALS AND METHODS: A total of 25 patients with clinically suspected endolymphatic hydrops underwent the procedure. After 24 h, three-d…

To evaluate the therapeutic effect of middle ear pressure pulse on endolymphatic hydrops in humans, Meniett20k was used for 12 months in two severe elderly patients with definite Meniere's disease. The average number of vertigo occurrences in the 6 months before treatment and in the 12 months after treatment started was compared. Numeric values (NV) among patients were calculated and categorized into an improved group (NV = 1-40). For hearing function, the pure-tone average (PTA) before and 12 months after treatment started was compared. PTA was calculated based on the following equation: PTA …

The pathophysiology of the characteristic episodic symptoms of vertigo, low-frequency hearing loss, and tinnitus in Menière's disease remains poorly understood. It is likely that the manifestation of this condition may be multifactorial and related to elements affecting the inner ear beyond the underlying pathology of endolymphatic hydrops. We have identified a subgroup of female patients with Menière's disease in which the symptoms of this disorder are correlated with the late luteal phase of the menstrual cycle (premenstrual period). Through audiometric and vestibular testing, we have docume…

The right vestibular aqueduct was obliterated in guinea pigs, chinchillas, and monkeys, and the right cochlear aqueduct and both the right vestibular and right cochlear aqueducts were obliterated in guinea pigs and chinchilas. Changes in auditory acuity were monitored by determining averaged temporal-response thresholds, and temporal-bone histologic studies were performed. Obliteration of the vestibular aqueduct or both the vestibular and cochlear aqueducts consistently produced endolymphatic hydrops in guinea pigs. In these animals, the auditory acuity gradually deteriorated. The low tones co…

INTRODUCTION: Although the proportion of Ménière disease (MD) patients who meet candidacy for CI remains relatively low, this subgroup has garnered increasing clinical interest. The main objective of this work is to evaluate the influence of preoperative vestibular function and radiological markers of endolymphatic hydrops (EH) on long-term auditory outcomes after cochlear implantation (CI) in patients with definite Ménière’s disease (MD). METHODS: All patients underwent CI and were assessed using a dedicated 3T MRI hydrops protocol. Audiological outcomes were measured using pure-tone audiomet…

Menière's symptom-complex in conjunction with chronic otitis media (COM) has occasionally been encountered in otologic practice. This study … endolymphatic system drainage procedures (80% versus 20%). COM is regarded as one of the causes of endolymphatic hydrops (EH). The results of this study seem to indicate both 1. the likelihood of the cause-and-effect relationship between COM and EH, and 2. the effectiveness of endolymphatic shunting procedures on control of incapacitating vertigo, thus supporting the …

Certain peripheral vertigos undoubtedly do not raise from neuro-sensorial structures but rather of the secretory structures and can be called "endolymphatic". They deserve to be individualized because concerning different pathogenic mechanisms and certainly are relevant for a specific treatment. We hope that in the next years new investigation tools will allow to explore in a noninvasive way the endolymphatic contents and dysfunctions of these structures.

Temporal bone findings in two cases of head injury are reported. In one patient, longitudinal fractures occurred in both temporal bones, and extended to the middle ear bilaterally. Bleeding was seen in the tympanic cavity, mastoid air cells, internal auditory meatus and facial nerve canal. Bleeding was observed in the scala tympani, cochlear aqueduct and endolymphatic sac, but there were no fractures. In the other patient, bleeding was seen in the internal auditory meatus and facial nerve canal. In the right ear, endolymphatic hydrops was observed in all turns of the cochlea.

OBJECTIVE: To investigate the relation between the vestibular system and vasopressin (AVP).
MATERIAL AND METHODS: We examined the effects o … VP levels in rats. Therefore, vestibular disorder would seem to increase plasma AVP and thus worsen endolymphatic hydrops. EDSS rapidly decreased plasma AVP and would seem to reduce hydrops. Inhibition of vertigo spells by EDSS or intratympanic GM injection would reduce a possible stress response, resulting in a decrease in plasma AVP levels a long time after the treat …

Although endolymphatic hydrops is generally considered to be the most prominent factor in the etiology and pathology of Ménière's disease, we have concluded that this condition more probably represents a polyganglionitis caused by the herpes simplex virus with secondary hydrops changes. The wide range of symptoms occurring in the Ménière's disease complex is illustrated in seven selected cases which support this conclusion. Vestibular nerve section can stabilize hearing and relieve episodic vertigo by removing the locus of viral infection and precluding recurrent activation.

There are a variety of techniques available to investigate endolymph dynamics, primarily seeking to understand the cause of endolymphatic hydrops. Here we have taken the novel approach of injecting, via a glass micropipette, fluorescein isothiocyanate-dextran (FITC-dex) and artificial endolymph into scala media of anaesthetized guinea pigs, with subsequent imaging of the inner ear using Light Sheet Fluorescence Microscopy (LSFM) as a means to obtain highly resolved 3D visualization of fluid movements. Our results demonstrate endolymph movement into the utricle, semicircular canals and endolymp…

Ménière's disease is characterized by attacks of vertigo and by sensorineural hearing loss, tinnitus, and ear fullness; these cochlear symptoms are unilateral in the majority of cases. Medical treatment tends to control vertigo and to cure the possible causes of endolymphatic hydrops (autoimmune disease, syphilis, metabolic and endocrine derangements, etc). In case of failure of medical treatment, surgery is considered. Endolymphatic mastoid shunt is indicated when hearing is normal or fluctuating. In all other cases, vestibular neurectomy or chemical vestibular labyrinthectomy, by means of in…
