Endolymphatic shunt: an evaluation by test results.

Failure in the endolymphatic shunt operation for Meniere's disease is reportedly 30% or more. The fact that many patients have relief from their symptoms following this surgery offers validity to the procedure. Pure tone audiograms and sinusoidal harmonic acceleration (rotary chair or rotary test, RT) were used concurrently to evaluate end-results of this operation in 31 patients with Meniere's disease. RT shows parameters of phase and labyrinthine preponderance (LP) calculated by a computer. LP was determined by separately integrating the total slow-phase output of the vestibulo-ocular reflex

[Development of the mastoid air cells and its relationship to Ménière's disease].

OBJECTIVE: To investigate the fundamental pathological anatomy and possible pathogenetic factors of Ménière's disease(MD), we compared the types of mastoid air cells between the MD group and the control group. METHODS: The MD group had 113 ears and the control group had 100 ears. Temoral bone CT scanning was performed in all the subjects. The types of mastoid air cells were determined by surgical findings and imaging data. All the mastoid air cells were divided into diploetic type, gasified type and sclerosis type. Analysis of the proportion of different types and the statistical analysis wer

Sequential bilateral cochlear implantation in a patient with bilateral Ménière's disease.

This case study describes a 45-yr-old female with bilateral, profound sensorineural hearing loss due to Ménière's disease. She received her first cochlear implant in the right ear in 2008 and the second cochlear implant in the left ear in 2010. The case study examines the enhancement to speech recognition, particularly in noise, provided by bilateral cochlear implants. Speech recognition tests were administered prior to obtaining the second implant and at a number of test intervals following activation of the second device. Speech recognition in quiet and noise as well as localization abilitie

Multicentre evaluation of the temporal bones obtained from a patient with suspected Menière's disease.

A multicentre study of the inner ears of an 88-year-old patient with vertiginous spells and severe hearing loss in the left ear was performe … was smaller than that in the cochlear nerve in the internal auditory canal. Changes occurred in the endolymphatic sacs but were considered non-specific. In this case severe, apparently progressive hydrops and sensorineural degeneration, characteristic of Menière's disease, were associated with atypical onset of clinical symptoms at a late age.

Endolymphatic hydrops after fenestration: a temporal bone study with implications on the function of the utriculo-endolymphatic valve.

A temporal bone specimen demonstrating endolymphatic hydrops 13 years after fenestration of the lateral semicircular canal is presented. Fibro-osseous tissue extending from the lateral semicircular canal and reaching the vestibule produced fixation of the membranous wall of the utricle to the bony wall. Fixation and retraction of the utricule appears to have resulted in a permanently open utriculo-endolymphatic valve leaflet. Similar findings of fibro-osseous changes arising from the area of the crus commune and an open valve were found in a Meniere's disease specimen. The implications of thes

Surgical treatment of vertigo.

Endolymphatic saccotomy, vestibular neurectomy via middle fossa, labrinthectomy and translabyrinthine vestibular neurectomy have been used by the authors in 54 patients with episodic vertigo, chiefly from Ménière's disease. Over a mean follow-up period of 15 months hearing retention and vertigo control were of the same general order as in the reports of other surgeons. Our current ideas on indications for the different surgical procedures are discussed.

Scanning electron microscopy of hair cells, stereocilia and cross-linkage systems in experimentally induced endolymphatic hydrops.

In this study scanning electron microscopy was used to examine the stereocilia and their cross-linkages in guinea pig cochleas 1, 2, 4 and 8 months after endolymphatic sac obliteration. Initial changes were restricted to the stereocilia of the outer hair cells and consisted of a disarrangement and bulging of the stereocilia with an interruption of their cross-linkage systems. Subsequently, the stereocilia became fused and atrophied. Cross-linkages of inner hair cells remained intact. Loss of both outer and inner hair cells started in the apex and progressed towards the base of the cochlea. The…

Case Report: Preservation of Otolithic Function After Triple Semicircular Canal Occlusion in a Patient With Intractable Ménière Disease.

Operative measures are considered when medical treatment fails to control vertigo in patients with intractable Ménière disease. The present report discusses a case in which triple semicircular canal occlusion was performed in a 30-year-old female patient who responded poorly to previously performed endolymphatic sac surgery. Her vestibular and auditory functions were evaluated both before and after surgery. Class A control of vertigo was achieved during the 76-month postoperative follow-up period. Ocular and cervical vestibular evoked myogenic potentials could be elicited before and after surg…

Effects of middle ear pressure changes on clinical symptoms in patients with Ménière's disease--a clinical multicentre placebo-controlled study.

Different medical and surgical methods have been tried in attempting to reduce endolymphatic pressure in Ménière's disease. Pressure treatment has a role on the treatment staircase, after pharmacological treatment and before destructive methods. Pressure chamber treatment has shown that some patients respond well to the treatment with diminishing inner ear symptoms and also some hearing improvement. Earlier studies have shown that electrocochlear measurements improve after local pressure treatment in the ear. The present study was a prospective randomized placebo controlled, multicentre clinic

Vascular loops in internal auditory canal as possible cause of Menière's disease.

Vascular loops were found in 2 patients diagnosed clinically as having Ménière's disease. A vascular loop was suspected of being present within the internal auditory canal in another patient who complained of tinnitus and vertigo. Diagnosis was performed by air CT cisternography in the former patients and by CT with contrast enhancement in the latter patient. In Case No. 1, the attacks of vertigo disappeared after endolymphatic sac decompression. In Case No. 2, hearing loss progressed despite remission of vertigo. In Case No. 3, the patient had only tinnitus and dizziness. Although it was diff…

Evidence of serum antibodies against inner ear tissues in the blood of patients with certain sensorineural hearing disorders.

In healthy human temporal bones, immunoglobulins (IgG, IgA) and the secretory component are demonstrated by two immunohistochemical methods in epithelial cells and in the lumen of the endolymphatic sac. The normal human inner ear does not otherwise show any localization of immunoglobulins. By means of indirect immunofluorescence, antibodies against healthy inner ear tissue were found in the serum of 15 out of 21 patients with double-sided progressive or fluctuating sensorineural hearing loss of undefined etiology. In 2 cases of Cogan's syndrome, it was also possible to demonstrate serum antibo…

Ear anomalies associated with renal dysplasia and immunodeficiency disease. A histopathological study.

The histopathologic study of the temporal bones of a case with low-set rudimentary auricles, without preauricular pits or cervical fistula is presented. Abnormalities of the middle and inner ear, fusion of the kidneys, hydrocephalus, short-limbed dwarfism and immunodeficiency are described. An abnormally low position of the middle cranial fossa in relation to the petrous pyramid was observed. The cochlea was of normal length. The modiolus was poorly developed with apparently normal population of ganglion cells and moderate diffuse hydrops of the cochlear duct and saccule. The stria vascularis

Meniere's disease: Pathogenesis, treatments, and emerging approaches for an idiopathic bioenvironmental disorder.

Meniere's disease (MD) is a severe inner ear condition known by debilitating symptoms, including spontaneous vertigo, fluctuating and progressive hearing loss, tinnitus, and aural fullness or pressure within the affected ear. Prosper Meniere first described the origins of MD in the 1860s, but its underlying mechanisms remain largely elusive today. Nevertheless, researchers have identified a key histopathological feature called Endolymphatic Hydrops (ELH), which refers to the excessive buildup of endolymph fluid in the membranous labyrinth of the inner ear. The exact root of ELH is not fully un

Computerized tomography of the petrous bone in Menière's disease.

High resolution transverse axial computerized tomograph (CT) encompassing the lateral semicircular canal (LSC) was used to measure the size of the petrous bone in ears with bilateral (n = 13) and unilateral (n = 38) Menière's disease, and in ears with unilateral chronic otitis media (n = 25) as a control group. Measurements were made of the following 4 distances in the petrous bone; A: the distance between the posterior semicircular canal (PSC) and the posterior petrous surface (PPS) (P-P distance), B: the distance between the PSC and the LSC (P-L distance), C: the distance between the vestibu
