Most of the complications of surgical treatment for slipped capital femoral epiphysis result from unrecognized joint penetration. Standard radiographic projections are unable to locate implants precisely since the tridimensional joint is viewed in one plane at a time. The use of a single screw placed in the center of the femoral head prevents unrecognized joint penetration.

With the aim of detecting patients at risk of developing coxarthritis, 34 patients were investigated prospectively by MRI 6-14 years after epiphysiolysis for slipped capital femoral head. In particular, cartilage changes were analysed by gradient-echo sequences. In 40% of the hip joints investigated cartilaginous lesions were present, detectable as irregularity and flattening of contour and more rarely as changes in signal intensity. The results of treatment of 29 hips with slipped capital femoral epiphysis stabilized "in situ" with one or two AO cancellous screws have been presented. Excellent and good results have been achieved in 26 hips, the remaining 3 cases were rated as fair and poor. At the follow-up examination no early osteoarthritic changes of avascular necrosis of the femoral head were found in any case. The complications seem to decrease in number with the use of one screw only.

A percutaneous computed tomography (CT) guided pinning of experimentally reproduced slipped capital femoral epiphysis in five cadavers and one patient--who had bilateral pinning for moderate and severe grade chronic slip--was conducted. Prior investigations of cadavers revealed that this procedure can be performed in selected cases. The details of the technique are illustrated in this article.

We looked for structural differences between normal patients and those with slipped capital femoral epiphysis to explain shear failure of the growth plate in terms of mechanics. Slipped capital femoral epiphysis remains the unsolved adolescent hip disorder. The epidemiology, clinical signs and symptoms, pathology, and radiographic findings have been well described and are reviewed. However, since we do not know the cause of the disorder, prevention is not possible and an ideal method of treatment has not yet been found. Goals of treatment, methods of treatment, as well as results and complications of treatment are discussed.

A retrospective study was performed of 30 consecutive patients operated on with ASIF epiphysiodesis for slipped capital femoral epiphysis. During extraction, complications were encountered in 13/23 patients. A total of 38 screws were removed. Thirteen screws broke and another three screws were embedded in bone, making extensive chiseling of cortical bone necessary before extraction was possible. Due to the frequent complications at extraction, the ASIF epiphysiodesis screw is not suitable for treatment of the slipped capital femoral epiphysis.

Ten subcapital resection osteotomies were performed in nine patients with severe slipped capital femoral epiphysis. The risks of this operation are discussed and the operative technique is described in detail. In one patient postoperative chondrolysis of the hip joint developed, which resulted in a stiff but painless hip. With subcapital osteotomy, normal anatomy and mobility of the hip joint can be restored, except for shortening of the femoral neck by one or two cm. At follow-up, ranging from two to 10 years, correction of the deformity and good function of the joint have been maintained.

External compressive fixators were used for fixation after a femoral trochanteric osteotomy for severe slipped capital femoral epiphysis in two large teen-aged boys. The method was found to simplify both the operative fixation and the postoperative management. However, the healing time seemed to be prolonged.
