Ilizarov external fixation for severely comminuted supracondylar and intercondylar fractures of the distal femur.

Our aim was to determine the clinical effectiveness and safety of Ilizarov external fixation for the acute treatment of severely comminuted extra-articular and intercondylar fractures of the distal femur. A total of 14 consecutive patients with complex fractures was treated. There were three type-A3, two type-C2 and nine type-C3 fractures according to the AO/ASIF system. The mean follow-up was 14 months. Most fractures (13) united primarily at a mean of 16 weeks. One patient with a type-IIIA open fracture had infection and nonunion. The mean range of flexion of the knee at the final follow-up

Use of the "S" Quattro dynamic external fixator in the treatment of difficult hand fractures.

Thirty-seven patients were treated over a 7-year period using the "S" Quattro dynamic external fixator. There were 30 intra-articular and nine extra-articular fractures. Patients were reviewed at an average of 22.5 months. The average total range of motion for the affected digit at follow-up was 232 degrees for intra-articular and 241 degrees for extra-articular fractures. We believe that this device is a simple, reliable technique in the treatment of these difficult fractures.

Recurrent dorsal angulation of the distal radius fracture during dynamic external fixation.

Thirty-three fractures of the distal radius treated with a dynamic external fixator (that allowed for wrist motion between 2 to 4 weeks after surgery) were analyzed, focusing on loss of fracture reduction during external fixation. Fractures with preoperative dorsal angulation greater than 20 degrees and those involving the distal radioulnar joint had a significantly larger loss of reduction of dorsal angulation (8.9 degrees and 6.9 degrees, respectively) than fractures with less severe preoperative dorsal angulation or those with an intact distal radioulnar joint (3.0 degrees and 2.6 degrees,

Femoral lengthening: callotasis with Ilizarov external fixator alone and with intramedullary locking nail.

Callus distraction (callotasis) has improved the results of limb lengthening and has decreased complication rates. Unfortunately, this technique requires a long time in an external fixator device to allow bone regeneration and consolidation, especially in adults. We retrospectively compared the results and complications of two groups of patients who underwent femoral lengthening by means of callotasis with the Ilizarov external fixator. Group I underwent callotasis alone, and group II underwent callotasis with an intramedullary locking nail. Group I consisted of 14 patients, with a median age

Open tibial fractures treated with the Ex-fi-re external fixation system.

Fifty open tibial fractures were treated with the Ex-fi-re external fixation system from 1987 to 1994. According to the Gustilo and Anderson classification, there were 12 Grade I, 14 Grade II, 10 Grade IIIA, 13 Grade IIIB, and 1 Grade IIIC injuries. Eight fractures were segmental. The average patient patient age was 39 years (range, 16-85 years). With the reduction unit of the system, displaced tibial fractures could be reduced by the functions of the device in contrast to the manual reductions needed with other unilateral devices. Compression could be applied even to oblique fractures. The re

The effect of laparotomy and external fixator stabilization on pelvic volume in an unstable pelvic injury.

OBJECTIVE: Determine if laparotomy further destabilizes an unstable pelvic injury and increases pelvic volume, and if reduction and stabilization restores pelvic volume and prevents volume changes secondary to laparotomy. DESIGN: Cadaveric pelvic fracture model. MATERIALS AND METHODS: Unilateral open-book pelvic ring injuries were created in five fresh cadaveric specimens by directly disrupting the pubic symphysis, left sacroliac joint, and sacrospinous and sacrotuberous ligaments. Pelvic volume was determined using computerized axial tomography for the intact pelvis, disrupted pelvis with b

External fixation in war trauma management of the extremities--experience from the war in Croatia.

During 18 months, the authors treated 109 wounded patients whose complicated gunshot fractures were externally fixated. The total of 116 external fixators were applied. Besides bone fractures, blood vessels, nerve, and soft-tissue injuries required additional reconstructive procedures. The most frequent complications were infection of the tissue surrounding pins (35.34%), osteomalacia around the pins (18.10%) and osteomyelitis (7.70%). In 25 patients (21.55%) the external fixation was removed after the treatment of fractures. In eight wounded patients (6.89%) surgery had to be repeated to corr…

The results of external fixation of the radius in the treatment of comminuted intraarticular fractures of the distal end.

14 unstable comminuted intraarticular fractures of the distal radius were treated by the use of the A/O mini-external fixator. The distal pins were inserted in the distal fragment, thus leaving the wrist joint free to mobilize. Clinical results were assessed at 3 to 12 months using the Sarmiento demerit point system. Nine were male and five female, with a mean age of 37 years. Ten fractures were closed and four were open. 11 patients (78.5%) had an excellent functional score and three (21.5%) had a good score. All patients had normal wrist morphology with an average radial length of 11 mm, rad

The unilateral external fixator (Monofixator) in acute fracture treatment: experience in 50 fractures.

From 1988 to 1992, 50 fractures with associated soft tissue lesions were treated with an unilateral fixator (Monofixator). The location of the fracture was the tibia in 39 cases, the pelvis in 5 cases, the femur in 4 cases, the ankle and the humerus in one patient respectively. Twenty-two tibial fractures (56%) were open. Unproblematic healing with the monofixator was achieved in 71.8% of tibial fractures after a mean healing time of 18.2 weeks. Axial dynamic compression was performed in 18 tibial fractures. Planned reosteosynthesis was performed in 7 tibial fractures (18%). After secondary in

Treatment of a central fracture-dislocation of the hip using external fixation with iliofemoral distraction.

Three patients were treated with external fixation with iliofemoral distraction as an alternative treatment of comminuted central fracture-dislocations of the hip. The patients were mobilized on the first postoperative day and discharged from the hospital an average of 10 days postaccident. The frame was left in place for 6 weeks while the traction was maintained. Full weight bearing was started 3 months after the injury. A full range of motion in the injured hip was achieved at 4 months postoperation. No signs of avascular bone necrosis were seen during follow-up for up to 2 years. The clinic

[Change in the procedure from external fixator to intramedullary nailing osteosynthesis of the femur and tibia].

Shaft fractures of femur and tibia can be treated successfully by intramedullary nailing. In recent years the use of interlocking nails widened the indication for nailing fractures of the proximal and distal bone and more difficult fractures. There are still limits in polytrauma patients, chain fractures with or without participation of joints and in fractures with severe soft tissue injury (open or closed). In these cases primary treatment with external fixation has proved worthwhile. The main problems and risks of primary nailing osteosynthesis occur in the early phase of treatment, whereas

[Change in treatment procedure after primary external fixator osteosynthesis in polytrauma patients].

In polytrauma patients stabilisation of fractures is of minor importance compared with life-saving surgical interventions concerning the body cavities. Because of persisting instability especially of long bones with disadvantageous effects on the state of shock and the respiratory system (fat embolism, ARDS) early stabilisation of fractures should be achieved. In our experience the majority of cases allows external fixation of long bone fractures simultaneously with or following the life-saving operations. Postoperative positioning and nursing as well as additional diagnostic procedures are co

Hand and wrist function after external fixation of unstable distal radial fractures.

Twenty-eight patients with 31 fractures of the distal radius were treated by closed external fixation. The complication rate for this series was 50%. Anatomic results were acceptable but hand function at final review was disappointing, particularly in the measurement of grip strength and the patient's ability to perform the activities of daily living. There was a highly significant correlation between complications and poor functional outcome. Although external fixation can achieve restoration of normal anatomy, functional outcome may be poor because of the severity of the associated soft tiss

Corrective tibial osteotomy for genu recurvatum by callus distraction using an external fixator.

Genu recurvatum due to an osseous abnormality is a condition that rarely requires surgical intervention. When necessary, the operative solution usually considered is an opening wedge osteotomy using bone graft and plate fixation. The donor site (usually the iliac crest) has a well reported morbidity with a further procedure being required to remove the plate. We report two cases treated by a hemicorticotomy and callus distraction (hemicallotasis) of the tibia using an external fixator. Correction was performed under supervision as an out-patient. Hospital stay was short, the morbidity of the d

Open tibial fractures. Treatment by uniplanar external fixation and early bone grafting.

Seventy-nine open tibial fractures were treated with unilateral uniplanar tubular external fixators. Excellent stability allowed early weight-bearing. All comminuted fractures, with or without bone loss, and some transverse or short oblique fractures with intermediate fragments were treated by early bone grafting through a posterolateral approach. The external fixator was dynamised as soon as periosteal callus was seen on the radiograph. Bone healing times ranged from 11 to 40 weeks (mean 20). Significant ankle stiffness occurred in 10.9% and leg shortening in 2.8%. Pin track infection was see…

Early clinical experience with Orthofix external fixation of complex distal radius fractures.

Twelve patients with complex fractures of the distal radius were treated with the Orthofix external fixation system (EBI Medical Systems, Fairfield, NJ). On follow up physical examination, the average wrist dorsiflexion was 46 degrees, and the average volar flexion was 55 degrees. The average grip strength in the involved hand was 38 lb force. The average final radial angle was 18 degrees, and the average residual radial height was 12 mm. Average volar tilt was 3 degrees. The complication rate was 42%. Very rigid axial stability is afforded by the fixator, but the large 3.5 mm tapered bone scr

Simultaneous Bennett's and distal radius fractures treated with external fixation. A case report.

Colles' fracture is rarely associated with a carpal or metacarpal fracture. We report a case of ipsilateral, simultaneous Bennett's and distal radius fracture after moderate trauma. Closed reduction and external fixation led to uneventful healing and no residual symptoms.
