[Vitrectomy in secondary complications caused by intraocular foreign body].

Endophthalmitis and reduced vision as late complications of penetrating ocular injuries cause specific diagnostic problems. A persisting endopthalmitis may be caused by unnoticed organic foreign bodies (f.b.) like lashes or a splinter of glas. Endophthalmitis recurring months or years after a perforating injury indicates the possibility of a spontaneous mobilization of a f.b. Vitrectomy and removal of the f.b. is the therapy of choice in either situation. Five typical cases are presented: lashes encapsulated in the ciliary body following a limbal perforation; an intraretinal incarceration of a

The use of mucous membrane from the hard palate in the treatment of trichiasis and cicatricial entropion.

I have discussed a technique for grafting hard palate mucous membrane to the lid margin as a treatment for trichiasis. Because of the resistance of this material to shrinkage and recurrence of trichiasis, it forms an excellent barrier to recurrence of trichiasis.

Diagnosing and treating Phthirus pubis palpebrarum.

Phthiriasis palpebrarum is an uncommon cause of blepharitis and conjunctivitis and may easily be overlooked. A high index of suspicion and careful examination of the patient's lid margins and eyelashes will lead to the proper diagnosis. Treatment is best accomplished by careful removal of the lice and nits (louse eggs) from the patient's lashes. Local application of a pediculocide such as yellow mercuric oxide N.F. 1% ophthalmic ointment applied twice daily for one week or 0.25% physostigmine (Eserine) ointment applied twice daily for a minimum of ten days, to the lid margins should be conside…

Significance of Demodex folliculorum and Demodex brevis in Pathogenesis of Dermatological Diseases-Current State of Knowledge.

Demodex folliculorum and Demodex brevis are external parasites that reside in human hair follicles and sebaceous glands, most commonly on the cheeks, chin, nose, and eyelids, inhabiting the eyelash follicles. The prevalence of Demodex spp. varies with age. The highest concentration of mites is observed in older people, being almost 100%, and the lowest is found in children. Although the presence of parasites does not directly lead to the development of pathological symptoms, their high density is associated with diseases such as rosacea or blepharitis. This manuscript delves into the biologica

Treatment of moderate-to-severe alopecia areata in patients over the age of 65 years with baricitinib.

Baricitinib is a Janus kinase inhibitor that has been approved by the US Food and Drugs Administration for the treatment of severe alopecia areata (AA) in adults. However, the clinical trials that demonstrated the efficacy of baricitinib in the treatment of severe AA did not include men aged > 60 years or women aged > 70 years. We retrospectively assessed the efficacy and safety of baricitinib in 14 patients aged ≥ 65 years with moderate-to-severe AA. After a mean (SD) duration of 18.5 (11.9) months, a 72% reduction in mean Severity of Alopecia Tool score from baseline was observed. Partial or

Uncommon presentation of mycosis fungoides: eyelid margin involvement.

Mycosis fungoides is a cutaneous T-cell lymphoma that has been rarely reported to involve ocular structures. A 33-year-old woman who had received therapy for mycosis fungoides on the trunk for 11 years, presented to our clinic with new plaques and tumors on her eyebrows and eyelid margin, and alopecia of her eyelashes and eyebrow. The histopathological examinations supported the diagnosis of mycosis fungoides. There was no intraocular involvement with tumor. The mycosis fungoides was of stage II B, and the patient was referred to medical oncology and radiation oncology clinics for treatment. S…

Ocular leishmaniasis: a case report.

Cutaneous leishmaniasis (CL) is a protozoal disease which is endemic in Iran usually caused by Leishmaniasis major and Leishmaniasis tropica and transmitted by the bite of a sandfly. In Isfahan province CL is highly prevalent and we observe some unusual clinical features of disease. The eyelid is rarely involved possibly because the movement of the lids prevents the fly vector from biting the skin in this region. We report a case of ocular leishmaniasis with eyelid and conjunctival involvement that had simulated chalazion and was complicated with trichiasis. The patient was diagnosed by direct

Trichotillomania.

PURPOSE: Trichotillomania is characterized by an irresistible urge to pull one's hair, and may involve the eyelashes or eyebrows. The authors present four cases of trichotillomania, and review the management of this unusual disorder. METHODS: The cases of four patients with trichotillomania were reviewed retrospectively. RESULTS: All four patients had characteristic areas of broken lashes along the lid in the absence of other signs of disease. Three of the four knew they were plucking the hair, yet could not control it. In the fourth, it was only after a lengthy observation period that she w…

Reducing ocular Demodex using petroleum jelly may alleviate dry eye syndrome, blepharitis, facial dermatoses, ocular and respiratory allergies, and decrease associated prescribing: a hypothesis.

Demodex eyelash mites are increasingly associated with eye and skin inflammation in humans, and cause demodectic mange in mammals. Informal accounts of symptom improvement and reduced need for anti-allergy medicines, when Demodex reproduction is prevented, indicate a further role linking Demodex to rhinitis, asthma and dermatitis. Their mobility, allergenic debris and consequential immunological impact may also explain progression of allergies in the "allergic march". Being photophobic and nocturnal, Demodex folliculorum shelter, feed, and sleep in eyelash follicles during daylight. Coston (19

Melatonin premedication and the induction dose of propofol.

BACKGROUND AND OBJECTIVES: Melatonin (N-acetyl-5-methoxytryptamine) is the main indolamine secreted by the pineal gland. Many studies showed that premedication with melatonin is associated with preoperative anxiolysis and sedation without impairment of cognitive and psychomotor skills and without prolonging recovery. We hypothesized that melatonin decreases the amount of propofol required to produce an adequate depth of hypnosis at induction time. METHODS: After approval from the research committee of the anaesthesia department, informed written consent was taken from 45 adult patients underg

Scalp infestation by Pthirus pubis in a 91-year-old woman: A rare cause of chronic scalp pruritus.

Pthirus pubis is an ectoparasite that typically infests pubic hair and is primarily transmitted through close sexual contact. Although extragenital involvement may occur, scalp infestation remains exceptionally uncommon. We report the case of a 91-year-old woman who presented with a two-month history of persistent scalp pruritus that failed to respond to oral terbinafine and ketoconazole shampoo. Dermatological examination revealed numerous adult lice and nits attached to scalp hairs, with additional involvement of the eyelashes, eyebrows, and pubic region. Microscopic examination confirmed Pt…

Ablepharon macrostomia syndrome: Absent prepuce in the first case report in West Africa.

Ablepharon macrostomia syndrome (AMS) is an extremely rare congenital ectodermal dysplastic disease characterised by craniofacial, skin, skeletal and genital abnormalities. Very few cases have been reported since the first case report in 1977. We report the case of a 6-day-old male delivered to unrelated parents. He was dysmorphic with absent eyelids, eyelashes and eyebrows, large fish-shaped mouth, hyperpigmented thick anterior abdominal wall, absent prepuce amongst other features. Skull X-ray showed poorly developed zygomatic bones. The patient is being managed as a case of AMS in a multidis…

Ethnic differences in trichotillomania: phenomenology, interference, impairment, and treatment efficacy.

In this Internet study, we explore differences among minority and Caucasian participants in the phenomenology of, interference and impairment related to, and perceived efficacy of treatments for trichotillomania (TTM) symptoms. A demographic difference was found for number of children only. Results indicate that the minority sample was less likely to report pulling from their eyebrows and eyelashes than the Caucasian sample. Minorities were less likely to report increased tension before a pulling episode. Minorities reported high levels of TTM interference with home management but Caucasians r…
