Reversal of Phen-Fen associated valvular regurgitation documented by serial echocardiography.

We report a case of anorexigen-associated moderate to severe aortic and mitral regurgitation in which the regression of lesions was marked and well documented over more than 2 years. The stability of our patient, as well as the degree of regression of regurgitation documented in our patient, and others, suggests it is prudent both to observe such patients and to avoid operation until such observation has been carried out.

Appetite suppressants and valvular heart disease - a systematic review.

BACKGROUND: Although appetite suppressants have been implicated in the development of valvular heart disease, the exact level of risk is still uncertain. Initial studies suggested that as many as 1 in 3 exposed patients were affected, but subsequent research has yielded substantially different figures. Our objective was to systematically assess the risk of valvular heart disease with appetite suppressants. METHODS: We accepted studies involving obese patients treated with any of the following appetite suppressants: fenfluramine, dexfenfluramine, and phentermine. Three types of studies were re…

[Tolerance and acceptability of dexfenfluramine in the long-term treatment of obesity in private practice. ESIM Group].

OBJECTIVE: The value of a one year prescription of dexfenfluramine (dF) was demonstrated by a large multicentric study performed in centres specialised in the treatment of obesity (Guy-Grand 1989). We tried to evaluate the safety and acceptability of this treatment prescribed in routine practice. METHOD: 27 internal physicians or general practitioners recruited 58 overweight patients who received 15 mg of dF twice a day in association with a balanced hypocaloric diet and were followed for one year with two-monthly visits, plus a visit after the first month of treatment. At each visit, the pat…

Effects of benfluorex metabolites on membrane fluidity and insulin-related processes.

As little work has dealt with the antihyperglycemic property of benfluorex at the hepatocyte level, we studied the effects of its main metabolites, S422 and S1475, on membrane fluidity and on insulin binding, internalization and action in healthy rat hepatocytes. Both metabolites were effective fluidizing agents. Neither one affected insulin binding. Only S422 favored the bound insulin-receptor internalization process. The metabolites produced no change in basal alpha-aminoisobutyric acid uptake. Only S422 promoted the insulin-stimulated alpha-aminoisobutyric acid uptake in a dose-dependent wa

Microcrystalline identification of drugs of abuse: stimulant street drugs.

The microcrystallographic properties of the diliturate (5-nitrobarbituric acid) derivatives of certain stimulant drugs found in illicit street preparations have been determined. These microcrystallographic data can be used as a means of drug identification.

An Italian consensus on the management of Lennox-Gastaut syndrome.

PURPOSE: Although international guidelines exist, the clinical heterogeneity of Lennox-Gastaut syndrome (LGS) and the increasing availability of new and repurposed drugs (e.g., fenfluramine and cannabidiol) requires a practical guide to patient management in the clinical context. We report the results of a consensus survey among 42 Italian experts in the diagnosis and treatment of LGS. METHODS: The consensus procedure followed a modified Delphi approach. Statements were formulated, based on the most recent published evidence and the clinicians' personal experience, then discussed, and agreed …

The pharmacologic approach to the treatment of obesity.

Obesity is a major risk factor for morbidity and mortality, and a series of pharmacologic approaches are available for helping to manage the problem. Obesity is caused by an imbalance between caloric intake and energy expenditure, which is influenced by both environmental and genetic factors. Pharmacologic treatments include anorexigenic agents, which fall into two broad categories: those that act via brain catecholamine pathways and those that act via serotonin pathways. The most recent oral agents approved are dexfenfluramine, which is currently being marketed, and sibutramine. Both agents i…
