Experimental studies of pneumococcal meningitis.

This thesis summarizes experimental meningitis research conducted at Statens Serum Institut in collaboration with the Copenhagen HIV programme and the Danish Research Centre for Magnetic Resonance between 2001 and 2007. Previous experimental studies had shown that the host inflammatory response in invasive infections contributed significantly to an extremely poor outcome despite initiation of efficient antimicrobial chemotherapy. Consequently, we aimed to investigate and clarify how the course of disease in pneumococcal meningitis was modulated by local meningeal inflammation and concomitant s

Multidisciplinary management of severe odontogenic maxillofacial multi-space infection in a patient with HIV-syphilis coinfection and advanced immunosuppression: a case report.

BACKGROUND: HIV-syphilis coinfection poses a substantial challenge in the management of severe odontogenic infections, with few reports addressing maxillofacial multi-space infections in this setting. CASE PRESENTATION: A 50-year-old man developed severe left maxillofacial multi-space infection one week after molar extraction, presenting with respiratory distress, trismus, elevated C-reactive protein (235.20 mg/L), computed tomography evidence of gas-forming abscesses, reactive HIV antibody, positive syphilis serology, and a CD4+ T-cell count of 133.76 cells/μL. Emergency tracheostomy and inc

Deep space neck infection: principles of surgical management.

Knowledge of the management of infections of the deep spaces of the neck is essential to the daily practice of oral and maxillofacial surgery. Timely decisions must be made through the acute course of the disease. Interventions must be performed with the appropriate surgical skill. The surgeon must decide on medical and surgical management, including antibiotic selection, how to employ supportive resuscitative care, when to operate, what procedures to perform, and how to secure the airway. To make these decisions the surgeon must understand the anatomy of the region and the etiology of infecti

Periodontal infections and cardiovascular disease: the heart of the matter.

BACKGROUND: Oral infection models have emerged as useful tools to study the hypothesis that infection is a cardiovascular disease (CVD) risk factor. Periodontal infections are a leading culprit, with studies reporting associations between periodontal disease and CVD. The results, however, have varied, and it often is unclear what conclusions can be drawn from these data. SUMMARY: An association exists between periodontal disease and CVD. It is unknown, however, whether this relationship is causal or coincidental. Early studies predominantly used nonspecific clinical and radiographic definitio

[The impact of periodontal infection on systemic inflammatory process and atherosclerosis].

It is becoming increasingly clear that infections and chronic inflammatory conditions, such as periodontitis can be linked with atherosclerotic process. Periodontitis and atherosclerosis have many pathogenetic mechanisms in common. The objective of this based on current knowledge review is to present the putative mechanisms whereby periodontitis which is chronic and inflammatory in nature and initiated by microbial plaque can influence the atherosclerosis. Two main processes in particular are worthy of consideration and may provide the link between these two diseases. Induction of the chronic

[Orbital abscess of dental origin].

We describe an orbital abscess secondary to maxillary sinusitis of dental origin focusing on the seriousness of orbital infections and the dramatic consequences of delayed treatment. Classification of acute orbital conditions and appropriate management practices are presented.

Oral health and respiratory infection.

The oral cavity has long been considered a potential reservoir for respiratory pathogens. The mechanisms of infection could be aspiration into the lung of oral pathogens capable of causing pneumonia, colonization of dental plaque by respiratory pathogens followed by aspiration, or facilitation by periodontal pathogens of colonization of the upper airway by pulmonary pathogens. Several anaerobic bacteria from the periodontal pocket have been isolated from infected lungs. In elderly patients living in chronic care facilities, the colonization of dental plaque by pulmonary pathogens is frequent.

Suppurative thrombophlebitis in children: a ten-year experience.

OBJECTIVE: To determine the clinical and diagnostic features, complications, management and prevention of superficial suppurative thrombophlebitis (ST) in children < 18 years of age. STUDY DESIGN: A retrospective review of medical records was performed for patients in two urban hospitals from January 1, 1985, through June 30, 1995, with a discharge diagnosis of phlebitis. RESULTS: We identified 21 patients, including 12 neonates, with ST. The majority had underlying medical conditions or preceding invasive procedures and administration of broad spectrum antibiotics or total parenteral nutrit

Managing periorbital space abscess. Secondary to dentoalveolar abscess.

Although orbital space infections are a relatively rare sequela of dentoalveolar infections, such cases have been reported. This case report describes the management of an abscess involving the left eye that originated as a dental abscess.

[Clinical evaluation of ceftriaxone in severe infections in adults].

Thirty patients (17 male, 13 female; age 17 to 84 years; normal renal function in 23 cases) with severe bacterial infections were treated with ceftriaxone. The infections was septicemia in 20 cases, a septicemia-like condition in 2 and a focal infection in 8 (2 abscesses of the lung, 2 pyelonephritis, 1 abscess of the liver, 1 subphrenic abscess, 1 meningitis developed from an abscess of the brain and 1 acute intestinal infection). 25 infections were bacteriologically documented, with recovery of the following pathogens: 20 Gram negative rods (including 10 E. coli) that were all susceptible to…

Stroke-Like Presentation of Cerebral Toxoplasmosis: Two HIV-Infected Cases.

Toxoplasmosis is the most common opportunistic infection of the central nervous system in patients with acquired immunodeficiency syndrome (AIDS). Clinical presentation of cerebral toxoplasmosis in these patients includes headache, focal neurological deficits and seizures. Prompt diagnosis and appropriate therapy results in rapid clinical and radiological improvement as well as good outcome for patients. In this article, we report two cases with stroke-like presentation of cerebral toxoplasmosis in the setting of HIV infection.

[Hypoglossus and laryngeal nerves palsy after an intubation for Legionella infection].

Extracranial involvement of the hypoglossal nerve and recurrent laryngeal branch of the vagal nerve can be a complication of anaesthetic airway management (Tapia's syndrome) or focal involvement due to Legionella infection. We present a patient with bilateral hypoglossal and unilateral recurrent laryngeal nerves palsy after a complicated intubation and a Legionella infection. Clarithromycin therapy was started. Within months, tongue mobility and swallowing gradually improved. Two months after discharge persisting unilateral recurrent nerve palsy was observed.

Papular mucinosis associated with acquired immunodeficiency syndrome.

Papular mucinosis, also known as lichen myxedematosus, is a rare, idiopathic disorder in which focal deposits of mucin accumulate in the dermis. It has been previously reported in two patients with human immunodeficiency virus infection; we describe a third such case and note the presence of a granuloma adjacent to the mucin deposit. We also comment on other clinical manifestations of cutaneous mucin deposition and granuloma formation in human immunodeficiency virus infection.

Comprehensive dental management of odontogenic infection and ankyloglossia under general anaesthesia in a medically complex child with intellectual disability.

Children with intellectual disability often experience difficulties in accessing routine dental care because behavioural limitations and associated medical conditions make treatment challenging. Untreated dental infections in such patients may pose significant health risks, particularly when systemic conditions are present. A child in middle childhood with moderate intellectual disability presented with pain and progressive swelling in the right posterior mandibular region. The patient was non-verbal and had underlying cardiac dysfunction and encephalopathy, which further complicated clinical
