Tinea corporis gladiatorum (TCG) is a variety of tinea corporis transmitted by repeated and close skin contacts among athletes, in particular wrestlers and judokas. Trichophyton tonsurans is the most frequently isolated dermatophyte. Cases of TCG were reported in USA, Iran, Japan, Turkey and France, where wrestling or judo are popular. No cases of TCG were reported in Italy. The typical clinical presentation of tinea corporis is not always present in TCG: a bacterial folliculitis-like appearance is not rare. Fluconazole is the therapy of choice.

A 24-year-old male presented with orogenital ulcers, folliculitis, and progressive painful skin ulcers with a raised inflammatory border. Colonoscopy revealed volcano-shaped intestinal ulcers in the ascending colon, and hence, he was diagnosed as intestinal Behçets disease (BD) with pyoderma gangrenosum (PG). Treatment with systemic glucocorticoids and adalimumab dramatically improved the patient's symptoms. Our case demonstrates that early induction of adalimumab may contribute to the successful treatment of such difficult-to-treat conditions as intestinal BD with PG.

A 65 year old HIV-infected patient (CDC A2, diagnosed in 07/2008) presented with facial, erythematous infiltrated papular lesions. Consistent with progressive immunodeficiency a low CD4+ T-cell count and an increase of the viral load was seen simultaneously and an eosinophilic pustular folliculitis (EPF) was assumed. Though, the lesional biopsy revealed a follicular eosinophilic infiltration and endotrichial hyphae, proving for an infiltrating dermatophytosis. This shows how an infiltrating Tinea faciei is mimicking clinically and histologically an HIV-associated EPF of the face.

In brief: Wrestlers face a significant risk of injury and of contracting skin infections and respiratory and systemic illnesses. Because annual injury rates as high as 75% have been reported in wrestling, medical personnel should be present at all competitions. The article briefly describes mechanisms of injury and provides treatment guidelines for lacerations, cauliflower ear, and injuries to the spine, extremities, and ribs. Also provided is advice for the treatment and prevention of skin problems, including herpes gladiatorum, folliculitis, impetigo, fungal infections, and others.

Coal tar has been used in dermatology for over a hundred years. It was the main therapeutic agent for the treatment of various skin orders before the introduction of topical steroid. Since the advent of topical steroid the use of coal tar has reduced considerably. It is still being used a lot in the treatment of psoriasis. In this paper I will describe how I use coal tar not only in psoriasis, which is well known, but also in acne, folliculitis, eczema and seborrheic dermatitis and vitiligo, which are less well known.

Lithium is known to cause a variety of dermatological problems, common ones being acneiform eruptions, folliculitis and psoriasis including its pustular form. Hidradenitis suppurativa and acne conglobata are lesser known side effects, with only three reports so far. We report a patient, who had bipolar affective disorder, was on lithium for a long duration and developed hidradenitis suppurativa and acne conglobata during therapy, which subsequently decreased once lithium was stopped. We describe this case for its rarity and analyze its pathogenesis.

Patients infected with human immunodeficiency virus (HIV) have a high prevalence of UV radiation-responsive skin diseases including psoriasis, pruritus, eosinophilic folliculitis and eczemas. On the other hand, UV has been shown to suppress T cell-mediated immune responses and to induce activation and replication of HIV. These developments have prompted clinicians and investigators to question whether phototherapy is safe for HIV-infected individuals. We have reviewed these issues and hereby provide a summary and critique of relevant laboratory and clinical evidence.
