Charcot neuroarthropathy of the foot and ankle.

Charcot neuroarthropathy is a common cause of morbidity in persons with diabetes mellitus and sensory neuropathy. Although Charcot neuroarthropathy is rare, it likely will become more prevalent in conjunction with increased incidence of diabetes mellitus. Prevention of disease progression remains the mainstay of treatment, with surgical intervention usually reserved for refractory cases. Late deformities are often complicated by chronic ulceration, infection, and osteomyelitis. The clinical presentation is best summarized with the Eichenholtz classification, and progression often follows a pre

[Management of a diabetic foot ulcer].

A chronic diabetic foot ulcer requires a search for the etiology. The three main causes to search for are poor off-loading compliance, osteomyelitis, and peripheral vascular disease. The level of severity is measured with the U.T. classification and the level of infection with the classification of the International Consensus on the Diabetic Foot. Peripheral vascular disease must be precisely evaluated by Doppler ultrasound, which describes all the arteries of the lower limb. Angiography is required only in case of revascularization. Treatment of the ulcer includes strict off-loading, topical …

Foot and ankle imaging in the athlete.

Foot and ankle injuries are among the most common of all sports-related injuries. Many injuries are simplistic from a diagnostic standpoint, whereas others require a high level of suspicion and the use of appropriate diagnostic imaging modalities. Whether a professional athlete or a weekend warrior, all athletes want minimal disruption to their sporting activity and a quick return to competition. An accurate diagnosis aids in the development of an appropriate rehabilitation program and in return to activity. This article reviews the more common foot and ankle injuries seen in the podiatrist's …

Physical management of the Charcot foot.

Charcot arthropathy places individuals at risk of developing diabetic foot ulcers and potentially subsequent limb amputation by means of altering the anatomy of the foot and ankle. Physical trauma is an important component to the etiology of the condition. The physical management of the Charcot foot is concerned with minimizing the stress applied to the affected foot and ankle skeletal structure. The most appropriate device is temporally dependent on the progression of the disease. At the initiation of Charcot arthropathy, care by total contact cast is recommended. As the affected bones begin …

MR imaging of the diabetic foot.

MR imaging can be quite useful in the evaluation of pathologic processes related to the diabetic foot. The diagnosis of soft tissue processes such as callus formation, ulceration, foreign body granuloma, cellulitis, abscess, and gangrene can be made with the assistance of MR imaging. Osseous deformities such as hammertoe, rocker-bottom foot, neuropathic osteoarthropathy, and osteomyelitis can be detected and defined with MR imaging. Finally, although there are some overlapping features of neuropathic osteoarthropathy and osteomyelitis, the differentiation between the two entities is possible i…

Leiomyosarcoma of the foot: a case study.

Leiomyosarcoma is a very rare malignant tumor, with only 28 new cases diagnosed in the United States each year. The prognosis varies, with average 5-year survival of 65%. Although most leiomyosarcomas occur in the lower extremity, there is a paucity in the literature on these sarcomas in the foot. Only 15 cases of leiomyosarcoma in the foot have been reported in the literature since the mid-1930s. We describe a 31-year-old man with a history of an ingrown toenail and nonhealing pyogenic granuloma. His clinical presentation suggested atypical tissue. Biopsy findings confirmed the diagnosis of s…

[Involvement of the foot in metabolic diseases].

In the acute stage of gout, the hallux is most commonly involved followed by the mediotarsal joints and the Achilles tendons. Diagnosis of gout is established when typical monosodium urate crystals can be identified. Apart from NSAIDs, colchicine can be used when there is no renal impairment. Hypouricemic agents (allopurinol or uricosuric drugs) must be initiated one or two weeks after the acute attack of gout because there are risks of exacerbation. Losartan as well as fenofibrate have uricosuric properties. Chondrocalcinosis of the foot can be observed in hemochromatosis. Diffuse idiopathic …

[Common fungal infections of the foot].

Among the dermatological diseases affecting the foot, fungal infections are frequent in a daily practice. Treatments now available allow us to control most of them. However, multiple different clinical manifestations and the variety of fungal agents may lead to inappropriate diagnosis and treatments. The paper will consider cutaneous and nail manifestations of common fungal foot infections, their differential diagnosis and their local and systemic treatments.

[Posttraumatic arthritis in the hind foot].

Posttraumatic arthritis of the joint components of the hind foot is typically linked with hind foot deformity and involvement of the neighbouring joint. The principal goals of any hind foot reconstruction are to achieve a quasi-anatomical reconstruction of the geometry and a stable and plantigrade foot position as a prerequisite for acceptable gait function, and an overall satisfactory result for the patient. Profound knowledge of the functions of the ankle-hind foot complex is the basis for the development of a valid therapeutic strategy. Any surgical reconstruction has to consider previous i…

Postoperative imaging of the ankle and foot.

This article describes the relevant surgical detail and MR imaging appearance of common operations performed in the foot and ankle. To evaluate postsurgical patients critically, it is important to understand the primary clinical diagnosis, surgical treatment undergone, the interval since surgery, and patients' current clinical symptoms. Radiography is the most common imaging modality for evaluation of the postoperative ankle and foot. MR imaging may be useful for evaluating the soft tissues and osseous structures in the postsurgical foot and ankle.

MRI of trauma to the foot and ankle.

Traumatic injuries involving the foot and ankle are very common. With the advent of magnetic resonance imaging (MRI) and its unsurpassed ability for soft tissue characterization, its utility in the investigation of these patients with foot and ankle trauma has rapidly expanded over the last decade. MRI has the unique capability to evaluate osseous, ligamentous, tendinous, and muscular injuries about the foot and ankle, with a single imaging study. MRI also allows for characterization of injury based on known biomechanical patterns. Injuries to specific soft tissue structures such as the Lisfra…

Clinical examination of the foot and ankle.

The foot and ankle are critical components in our ability to ambulate. Injuries to either can significantly interfere with a patient's ability to carry out normal activities. In severe cases, they can be devastating to a patient's independence. Careful examination of the foot and ankle using established mechanical tests, along with understanding of the anatomy of the complex,is needed to confirm the history and to assist in the diagnosis and treatment of foot and ankle injuries. The following points are key to clinical examination of the foot and ankle: . The examination of the foot and ankle …

Midfoot osteotomies for the cavus foot.

Midfoot osteotomies have long been used for a wide variety of congenital and acquired deformities. Severe pes cavus often necessitates some form of surgical correction, and no single procedure can be used exclusively. Midfoot osteotomies may be combined with adjunctive procedures to form an appropriate strategy for the treatment of these deformities. The authors present an overview of pes cavus, including evaluation and classification of the deformity, as well as associated conditions. Several soft tissue and osseous procedures for correction of the cavus foot are discussed. Special attention …

Microsurgical management of the diabetic foot.

Allthough there has been dramatic progress in limb salvage in recent decades, management of nonhealing wounds in diabetic patients continues to present a dilemma for the reconstructive surgeon. However, the acceptance of free-flap resurfacing of diabetic foot ulcers has increased in recent years. This study reviews 10 microvascular free muscle flaps in nine patients over a mean follow-up period of 44 months. Five patients had evidence of peripheral vascular disease. There was one flap loss, and nine flaps were transferred successfully. No perioperative mortality was encountered. The operations…
