Calcaneal compartment syndrome after tibial fractures.

Compartment syndrome of the newly discovered calcaneal compartment of the foot is a theoretical possibility following tibial fracture due to the communication with the deep posterior compartment of the calf. Forty-nine patients were reviewed at least 18 months after open or closed tibial shaft fractures treated with tibial nailing in order to determine the prevalence of foot deformities secondary to previously undetected calcaneal or leg compartment syndromes. Ankle movements, foot height, length of feet and degree of clawing of the toes were all measured and compared with the unaffected oppos…

Spectrum of operative treatments and clinical outcomes for atraumatic osteoarthritis of the tarsometatarsal joints.

BACKGROUND: The purpose of this study was to identify subtypes of atraumatic osteoarthritis of the tarsometatarsal joints based on accompanying foot deformities and to determine whether concurrent procedures done for each subtype were effective. METHODS: The study included 59 patients (67 feet) with atraumatic tarsometatarsal joint osteoarthritis treated with tarsometatarsal fusion. The average patient age was 60.2 years with 40.6 months followup. Patients were evaluated with radiographs, the American Orthopaedic Foot and Ankle Society (AOFAS) Midfoot Scale, the Foot Function Index, and the S…

Techniques for the management of burn contractures with the Ilizarov Fixator.

The Ilizarov fixator was used to correct chronic burn deformities of the foot and ankle. These contractures can be classified as "simple" or "complex" based on associated deformities and musculoskeletal function. The simple, undirectional deformities of equinus, cavus, rockerbottom, and toe dislocations are fairly easy to correct and maintain. Correction and maintenance of complex deformities with varus or valgus angulation, bone abnormality, or muscle loss are more difficult.

Tibialis anterior transfer "into talus" for control of the severe planus pediatric foot: a preliminary report.

Transfer of tibialis anterior into the talus has been utilized for correction of the vertical talus, as well as for paralytic valgus foot deformities with the suggestion for its use in severe flatfoot deformities. The authors present transfer of tibialis anterior through the talus for controlling the severe planus pediatric foot. The procedure is presented, and six feet having undergone this procedure are analyzed. This procedure, in combination with others, has proven successful in reconstructing this pediatric deformity clinically and radiographically.

Cadaveric Dissection of Adult Neglected Talipes Equinovarus.

INTRODUCTION: Congenital talipes equinovarus is a musculoskeletal deformity causing foot disability with plantar flexed, inverted and adducted foot. The deformities associated with club foot must be corrected at an early age to prevent long term residual disabilities. CASE REPORT: In this case, we describe a rare anatomical variation of the neglected adult club foot, which was noticed during routine dissection of lower limb for IMBBS in AIIMS, Mangalagiri. The adult neglected club foot was dissected to identify the exact soft tissue and bony abnormalities involved in the deformity. The planta

Management of the flexible flat foot in the child: a focus on the use of osteotomies for correction.

Pes planus, commonly referred as flat foot, is a combination of foot and ankle deformities. When faced with this deformity in children, the treating surgeon should use a systematic method for evaluation to distinguish normal variation from true pathology, as well as conditions that have a benign natural history versus those that may lead to significant disability if left untreated. Certain deformities will inevitably worsen and therefore require surgery. Common sense clearly supports the indication for a simple procedure, such as an arthroereisis or an osteotomy, performed in the young child a

Does the use of high-heeled shoes lead to fore-foot pathology? A controlled cohort study comprising 197 women.

BACKGROUND: High-heeled shoes have been suggested as a main explanation for the female dominance in foot pain and deformities. Aim of study was to test this hypothesis scientifically. METHODS: Women 40-66 years were included in two groups. 95 women who had worn high-heeled shoes at work for at least 5 years were compared to 102 women who had never worn high-heeled shoes at work. The investigations were weight bearing radiographs of foot and ankle, the SEFAS questionnaire and the AOFAS Clinical Rating System. Evaluators were blinded to the group-affiliation. RESULTS: Radiographs showed no sta

Podiatry alterations in Ehlers-Danlos syndrome.

BACKGROUND AND OBJECTIVE: This paper aims to describe the type of foot and most frequent podiatric alterations, as well as the level of pain, disability, fatigue and alteration of quality of life in patients with Ehlers-Danlos syndrome (EDS). MATERIALS AND METHODS: Thirty-eight individuals with hypermobile or classic EDS participated. The percentage of pain in the foot and deformities was recorded, and several questionnaires were administered. The type of foot was classified according to the footprint and the Foot Posture Index. RESULTS: There was a high degree of pain, disability, intensity

Ischemic contracture of the foot and ankle: principles of management and prevention.

A variety of clinical presentations can be encountered following compartment syndrome of the leg and foot. Deformity and functional impairment in the foot and ankle secondary to ischemia are determined by: 1) which leg compartments have been affected and to what degree extrinsic flexor or extensor "overpull" is exhibited, 2) degree of nerve injury sustained causing weakness or paralysis of extrinsic or intrinsic foot and ankle muscles, 3) which foot compartments have been affected and to what degree intrinsic "overpull" is exhibited, and 4) degree of sensory nerve injury leading to anesthesia,

One-stage reconstruction of complex lower extremity deformity combining Ilizarov external fixation and sural neurocutaneous flap.

BACKGROUND: Complex lower extremity deformities include multidirectional foot and ankle deformities, which are complicated therapeutic and surgical challenges. Correction often requires several stages, which is time consuming and costly. The need to restore the physical, mechanical, and cosmetic aspects of the lower extremity results in a difficult balancing act between these concerns and the deformity correction and soft tissue reconstruction. METHODS: Between January 2009 and September 2011, we treated 5 patients with multidirectional foot and ankle deformities. Significant mobility limitat

Double and Triple Tarsal Fusions in the Complex Cavovarus Foot.

The cavovarus (cavus) foot is one of the most perplexing and challenging of all foot deformities and may prove to be one of the most difficult conditions to treat. This deformity is characterized by increased plantar flexion of the forefoot and midfoot in relation to the hindfoot resulting in high foot arch. Because cavus foot rarely occurs in an isolated form, the term "cavus foot" rather describes a part of a complex multiplanar foot deformity. Because the underlying disease is mostly neurogenic characterized by muscle imbalance in almost every case a combined bony and soft tissue surgery is…

Peroneus longus tendon lengthening.

This article presents lengthening of the peroneus longus tendon as an adjunctive procedure in the surgical correction of a plantarflexed first ray. The technique has enhanced the effectiveness of primary surgical procedures utilized in the correction of a plantarflexed first ray. Technical difficulty associated with lengthening of the peroneus longus tendon is minimal, and the incision placement introduced in this manuscript provides excellent exposure.

Ankle and Foot Spasticity Patterns in Chronic Stroke Survivors with Abnormal Gait.

Chronic stroke survivors with spastic hemiplegia have various clinical presentations of ankle and foot muscle spasticity patterns. They are … of these muscles and their interactions. Careful and thorough clinical assessment of the ankle and foot deformities is needed to determine the primary cause of each deformity. An understanding of common ankle and foot spasticity patterns can help guide clinical assessment and selection of target spastic muscles for botulinum toxin injection or nerve block.

[Clubfoot in adult patients treated with tibiocalcaneal arthrodesis: about two cases (3 feet)].

Inveterate clubfoot (inveterate talipes equinovarus, ITEV) is a vicious attitude of the foot which doesn't rest on the floor through its normal base of support but weight is thrown to the outer side of the foot and to the foot point. When this vicious attitude persists a long time, it results in severe osteoarticular deformities in adults. Fixed ITEV is a good indication for tibiocalcaneal arthrodesis. In the majority of cases this provides painless and stable plantigrade foot. This study aims to assess the clinical and radiological outcomes of astragalectomy with tibiocalcaneal fusion. This c

The distal metatarsal articular angle in hallux valgus deformities. Comparisons of radiographic and weightbearing CT scan measurements with variations in hindfoot position.

BACKGROUND: When evaluating hallux valgus (HV) deformity with anteroposterior (AP) foot radiographs, the distal metatarsal articular angle (DMAA) has been the subject of frequent debate. Although a straightforward indicator of structural alignment of the distal first metatarsal articular surface, inter- and intraobserver measurements can vary widely. Alterations in the radiographic appearance of bony "structure" with positional changes of the foot/foot bones in HV deformities in no small part contributes to these inaccuracies. The aim of this study was to determine the effect of hindfoot posit
