Ichthyosis vulgaris palmaris et plantaris dominans: a peculiar form of ichthyosis vulgaris localized on the palms and soles.

4 cases with congenital hyperkeratotic lesions localized on the palms and soles, occurring in three generations of one family, were observed. The atrophic, hyperkeratotic lesions accompanied by anhidrosis were dirty yellowish-brown coloured, with an ill-defined border but without any erythematous or pigmented halo. Further clinical findings were conspicuous dryness, adherent scale formation, fissurings and parchment paper-like luster. Histological findings were identical to those of ichthyosis vulgaris, with an absence of eccrine sweat glands. Hence, this type of hyperkeratosis can be distingu

Methicillin-resistant Staphylococcus aureus foot infection after fish pedicure.

Fish pedicure has become a popular cosmetic procedure involving immersion of the feet in a tank with freshwater fish (Garra rufa) that nibble off dead skin. There are concerns about the potential transmission of pathogens, but no cases of infections after this procedure have been published so far. We present a patient who developed foot infection with methicillin-resistant Staphylococcus aureus (MRSA) after fish pedicure.

Fungal foot infection, cellulitis and diabetes: a review.

AIMS: To review the current evidence for the presence of fungal foot infection (tinea pedis and toenail onychomycosis) as a risk factor for the development of cellulitis within the lower limb, particularly for those individuals with diabetes. METHODS: A structured review of medline, embase and cinahl databases was undertaken to identify publications investigating fungal foot infection as a risk factor for the development of cellulitis. RESULTS: Sixteen studies were identified. Eight studies adopted a case-control methodology, with the remainder being cross-sectional surveys. The majority of

Prevalence of toe nail onychomycosis in diabetic patients.

Onychomycosis among diabetic patients has been reported in some studies to be of high prevalence. This study aimed to investigate the prevalence of onychomycosis among diabetic patients at a Danish University Hospital. Clinical and mycological examinations were performed on type 1 and 2 diabetic patients from in- and out-patient clinics. A total of 271 patients were enrolled, 72% males, mean age 61.3 years, 26% of the patients had diabetes type 1. The prevalence of toe nail onychomycosis (positive culture and/or microscopy) was 22% (n = 59) of which 55 cases were caused by dermatophytes (93%)

Investigations on the biology, epidemiology, pathology and control of Tunga penetrans in Brazil: I. Natural history of tungiasis in man.

Tungiasis is an important health problem in poor communities in Brazil and is associated with severe morbidity, particularly in children. The causative agent, the female flea Tunga penetrans, burrows into the skin of its host, where it develops, produces eggs and eventually dies. From the beginning of the penetration to the elimination of the carcass of the ectoparasite by skin repair mechanisms, the whole process takes 4-6 weeks. The present study is based on specimens from 86 patients, for some of whom the exact time of penetration was known. Lesions were photographed, described in detail an

Management of the arthritic forefoot.

Rheumatoid diseases frequently involve the forefoot producing severe hallux valgus and dislocated metatarsophalangeal (MTP) joints. One must stress the importance of continual evaluation of the foot in these patients. The relationship of midfoot and hindfoot deformities, especially pronation deformities, to the forefoot is essential. Nonoperative treatment can be highly effective and consists of skin and nail care and proper shoes which enclose and support early deformities. Operative care now spans the gamut from excising all or part of the MTP joints, to silicone implant arthroplasty of the

Hyperhidrosis - Sweating Sites Matter: Quality of Life in Primary Hyperhidrosis according to the Sweating Sites Measured by SF-36.

BACKGROUND: Primary hyperhidrosis has negative impacts on quality of life. The aim of this study was to investigate whether the impacts of primary hyperhidrosis on quality of life are different depending on the localisation of the sweating. METHOD: We compiled background data, Hyperhidrosis Disease Severity Scale (HDSS), and Short-Form Health Survey (SF-36) post hoc results from 2 previous studies. Cases who described only 1 site as their most problematic area of sweating were included (n = 160/188) while individuals with multifocal primary sites of hyperhidrosis were excluded (n = 28/188).

Incidence of dermatophytosis in a public hospital of São Bernardo do Campo, São Paulo State, Brazil.

The incidence of cases of dermatophytosis in dermatology outpatient departments has increased in recent years. Infection control is essential to prevent transmission, and accurate diagnosis of this type of infection is important to avoid confusion with other dermatological processes caused by non-fungal agents. The objective of this study was to determine the incidence of dermatophytosis and its etiological agents in a public hospital of São Bernardo do Campo, Brazil. From February 2005 to May 2006, 273 samples were collected from 191 patients in a public dermatology outpatient department. The

Purpuric generalized lichen nitidus: an unusual eruption simulating pigmented purpuric dermatosis.

BACKGROUND: Generalized haemorrhagic lichen nitidus is rare. To our knowledge, this form of presentation has only been reported once. OBJECTIVE: To describe a new case of generalized haemorrhagic lichen nitidus simulating a pigmented purpuric dermatosis. METHODS AND RESULTS: We document a 24-year-old man who presented with an 8-month history of a progressive non-pruritic, red-brown papular eruption on the dorsa of the feet, ankles and distal third of the legs. A diagnosis of Schamberg's progressive pigmentary dermatosis was made, and no treatment was prescribed. Two months later, the lesions

Update on current approaches to diagnosis and treatment of onychomycosis.

Onychomycosis is a chronic fungal infection of the nail bed, matrix or plate. It accounts for roughly 50% of all nail disease. As the prevalence of onychomycosis is increasing, a critical review of diagnostic techniques and treatment options is required. Areas covered: This review discusses the current diagnostic techniques associated with diagnosing onychomycosis, such as microscopy, culture, periodic acid Schiff stain (PAS) and polymerase chain reaction (PCR). Oral and topical therapies are also discussed, as well as, the utility of device-based treatments and combination therapy. Expert com

Chronic idiopathic acrocyanosis and methylenetetrahydrofolate reductase C677T (p.Ala222Val) and A1298C (p.Glu429Ala) polymorphisms.

BACKGROUND: Chronic idiopathic acrocyanosis is a common acrosyndrome. Methylenetetrahydrofolate reductase (MTHFR) is an enzyme involved in the metabolism of folate. Two functional polymorphisms of MTHFR have been identified, C677T and A1298C. OBJECTIVE: To compare the prevalence of these two MTHFR polymorphisms in patients with chronic idiopathic acrocyanosis to a control group. MATERIALS AND METHODS: The study was conducted on 43 consecutive patients with acrocyanosis and on 100 controls. RESULTS: The risk of acrocyanosis was significantly higher in patients homozygous for the mutation c.6
