The doubled burden of diabetic bone disease: hip fracture and post-hip fracture mortality.

OBJECTIVE: Patients with diabetes have an increased risk of osteoporosis and shorter life expectancy. Hip fracture (HF) is the most serious consequence of osteoporosis and is associated with increased mortality risk. We aimed to assess the association of antidiabetic medications with HF and the post-hip fracture mortality risk among diabetic patients ≥50 years. DESIGN: In this nationwide case-control study 53 992 HF cases and 112 144 age-, sex- and region-matched non-hip fracture controls were analyzed. A cohort of hip-fractured diabetic patients were followed-up for an all-cause mortality.

A timeseries analysis of the fracture callus extracellular matrix proteome during bone fracture healing.

While most bones fully self-heal, certain diseases require bone allograft to assist with fracture healing. Bone allografts offer promise as treatments for such fractures due to their osteogenic properties. However, current bone allografts made of decellularized bone extracellular matrix (ECM) have high failure rates, and thus grafts which improve fracture healing outcomes are needed. Understanding specific changes to the ECM proteome during normal fracture healing would enable the identification of key proteins that could be used enhance osteogenicity of bone allograft. Here, we performed a ti

Endoscopic Endonasal Orbital Floor Fracture Repair With Mucosal Preservation to Reinforce the Fractured Bone.

Treatment of orbital floor fracture is often needed to cure diplopia and enophthalmos. However, the available surgical methods have many limitations. An endoscopic endonasal reduction is a familiar approach for otolaryngologists; however, reconstruction of the orbital floor may pose certain problems. Here, the authors developed the endoscopic endonasal orbital floor fracture repair with mucosal preservation procedure (mucosal preservation procedure). This study aims to evaluate outcomes after the mucosal preservation procedure. The authors analyzed 18 patients who had their orbital floor repai

Efficacy of bone-end intervention on fracture healing in bisphosphonate-related atypical femoral fractures.

BACKGROUND: Delayed unions are quite common in the treatment of atypical femur fractures, which are thought to result from the long-term use of bisphosphonates. The effects of interventions for sclerotic tissue on the fracture line in atypical femoral fractures are not fully known. For this reason, we compared the results of patients with atypical femoral fractures treated by closed intramedullary nailing to patients treated by open surgery accompanied with interventions for their sclerotic bone ends, aiming to answer: (1) In the treatment of atypical femoral fractures, do bone-end interventio

Predictors of fragility fracture and low bone mineral density in patients with a history of parental fracture.

OBJECTIVES: Bone mineral density (BMD) and fragility fracture (FF) have high heritability, but few data exist on impact of other factors on families with fracture history. We aimed to evaluate predictors of FF and low BMD, in patients with family history of FF. METHODS: This was a retrospective study on patients undergoing dual energy X-ray absorptiometry at a district general hospital (DGH), 2004-2016. Parameters recorded (in addition to standard dual energy X-ray absorptiometry parameters): age, smoking, alcohol, corticosteroids, aromatase inhibitors, Depo-Provera, hormone replacement thera

Antagonizing midkine accelerates fracture healing in mice by enhanced bone formation in the fracture callus.

BACKGROUND AND PURPOSE: Previous findings suggest that the growth and differentiation factor midkine (Mdk) is a negative regulator of osteoblast activity and bone formation, thereby raising the possibility that a specific Mdk antagonist might improve bone formation during fracture healing. EXPERIMENTAL APPROACH: In the present study, we investigated the effects of a monoclonal anti-Mdk antibody (Mdk-Ab) on bone healing using a standardized femur osteotomy model in mice. Additional in vitro experiments using chondroprogenitor and preosteoblastic cells were conducted to analyse the effects of r

[Secondary osteoporosis or secondary contributors to bone loss in fracture. Vitamin D deficiency and fracture].

Vitamin D plays an important role in maintaining bone and calcium metabolism. Vitamin D deficiency causes not only low bone mineral density via secondary hyperparathyroidism, but also impairment of bone mineralization. Vitamin D deficiency also increases the risk of fall. Thus, vitamin D deficiency results in increased fracture risk. Serum 25 (OH) D level reflects Vitamin D store and more than 30 ng/mL is recommended for bone health, but the measurement of serum 25 (OH) D is not reimbursed by Japanese health insurance system. Vitamin D supplementation to vitamin D deficient osteoporotics reduc

[Bone fracture and the healing mechanisms. A new method for evaluation of fracture healing by echo tracking].

Assessment of bone healing on radiographs is not necessarily objective. A clinical system was developed to quantify the stiffness of healing fractures of the tibia in patients by the echo tracking (ET) method in a manner similar to a three-point bending test. The lower leg was supported. An ultrasound probe was placed on the proximal and distal fragments and a load of 25 N was applied. With a multiple ET system, two probes measured the displacement of five tracking points on each of the proximal and distal fragments of the tibia, thereby detecting the bending of the two fragments generated by

Critical review of bone health, fracture risk and management of bone fragility in diabetes mellitus.

The risk of fracture is increased in both type 1 diabetes mellitus (T1DM) and type 2 diabetes mellitus (T2DM). However, in contrast to the former, patients with T2DM usually possess higher bone mineral density. Thus, there is a considerable difference in the pathophysiological basis of poor bone health between the two types of diabetes. Impaired bone strength due to poor bone microarchitecture and low bone turnover along with increased risk of fall are among the major factors behind elevated fracture risk. Moreover, some antidiabetic medications further enhance the fragility of the bone. On th

[Osteosynthesis with bone screws in closed oblique and screw-like fractures of the long tubular bones].

295 patients subjected to screw osteosynthesis for closed oblique and screw-shape fractures of crural, brachial and antebrachial bones were under observation. The use of screws of a cylindrical form with a somewhat conus-like head and deep slit seems to be most rational for osteosynthesis. To apply screws properly two drills should be available. One should be equal to the diameter of a screw, the other-0.5 mm less. Following screw ostenosynthesis an adequate plaster dressing is applied for 2-3 months. The results of the treatment were good in 280 patients (95%) and poor-in 15 patients (5%). Th

Treatment of an Ulnar Fracture Bone Defect Using Heterotopic Bone Autograft: A Case Report.

CASE: Autologous bone grafting has wide applications for the treatment of bony defects. Generally, cancellous or corticocancellous bone grafts are used depending on the characteristics and size of the bony defect and wound bed. The use of heterotopic bone as a potential source of bone graft has not been widely reported. We present a 56-year-old right-hand-dominant male victim of dog mauling who sustained a right ulnar fracture with a 5-cm bony defect, treated with the use of heterotopic bone autograft. CONCLUSION: Heterotopic bone can be successfully used as an autograft in the treatment of b

The potential protective effects of temporal bone pneumatization: A shock absorber in temporal bone fracture.

OBJECTIVES: We hypothesize that when temporal bone fractures occur, the pneumatic cells in the temporal bone are able to absorb most of the impact force during a traumatic event. This study aims to correlate the degree of pneumatization of the temporal bone with the severity of temporal bone fracture (TBF). METHODS: Charts and computed tomography scans representing 54 TBFs, diagnosed from 2012 to 2017 at a single tertiary hospital, were retrospectively reviewed. Temporal bone pneumatization (TBP) in the petrous apex and mastoid region was evaluated using previously published classification sy

[Secondary osteoporosis or secondary contributors to bone loss in fracture. Bone metabolism in hematological disorders].

Bone marrow provides a microenvironment responsible for hematopoiesis, in which osteoclasts and osteoblasts interact with each other to maintain bone remodeling. Multiple myeloma and a portion of lymphoid tumors have a unique propensity to expand in bone. Such tumors have been demonstrated to perturb bone metabolism and skew a microenvironment in bone suited for their growth and survival. However, a bone metabolic state remains largely unknown in most of hematological disorders without apparent bone disease, although hematopoiesis and bone metabolism appear to have a link. Also, the skeletal e

[Secondary osteoporosis or secondary contributors to bone loss in fracture. Endocrinological aspects of bone metabolism].

Bone works to play essential roles in mineral metabolism and hematopoiesis as well as to support our body and protect internal organs as a hard tissue. In order to accomplish these multiple functions, bone needs to communicate with other organs. Endocrine system functions as one of the communication pathways between bone and other organs. It has been known that bone is a target organ of many hormones. In addition, it has been established that bone itself produces hormones and works as an endocrine organ.
