Locked plate fixation of the comminuted distal fibula: a biomechanical study.

BACKGROUND: The purpose of this study was to compare the biomechanical properties of locked versus nonlocked lateral fibular bridge plating of comminuted, unstable ankle fractures in a mode of catastrophic failure. METHODS: We created comminuted Weber C fractures in 8 paired limbs from fresh cadavers. Fractures were plated with either standard or locked one-third tubular bridge plating techniques. Specimens were biomechanically evaluated by external rotation to failure while subjected to a compressive load approximating body weight. We measured the angle to failure, torque to failure, energy

A biomechanical comparison of multidirectional nail and locking plate fixation in unstable olecranon fractures.

BACKGROUND: The main theoretic advantage of proximal olecranon fracture intramedullary fixation is decreased soft-tissue irritation and, potentially, less subsequent hardware removal. Despite this possible benefit, questions remain as to whether intramedullary devices are capable of controlling olecranon fractures to the same extent as locking plates. This study evaluates the ability of a novel multidirectional locking nail to stabilize comminuted fractures and directly compares its biomechanical performance with that of locking olecranon plates. MATERIALS AND METHODS: We implanted 8 stainles

Treatment of the femoral and tibial fractures with Grosse and Kempf locking nails.

In a series of 104 femoral and 50 tibioendomedullary nailings using Grosse-Kempf (G-F) locking nails, 24% of femoral fractures and 12% of tibial fractures were comminuted; 48% of the patients had multiple injuries. The nail was locked at proximal and distal levels in 54% of the cases for the femur. Dynamization of the nail was carried out at an average of 12 weeks. Nonweight-bearing was imposed for both series from three weeks to three months. Bone healing was achieved before six months in 100% of tibial fractures. Following Thoressen criteria, the results of femoral cases were all excellent a

Gunshot Wound Resulting in Femoral Neck Fracture Treated With Staged Total Hip Arthroplasty.

A 39-year-old male presented with multiple gunshot wounds and resultant left comminuted femoral head and neck fractures with retained intraarticular bullet fragments. Successful staged reconstruction was performed with initial placement of an antibiotic spacer and subsequent conversion to total hip arthroplasty. Staged reconstruction with an antibiotic spacer and conversion to total hip arthroplasty is a viable treatment approach for a gunshot wound resulting in intraarticular bullet fragments and comminuted femoral head and neck fractures not amenable to open reduction and internal fixation t

Reconstruction of the zygomatic arch using a mandibular adaption plate.

Zygomatic complex fractures are one of the most common facial fractures treated by maxillofacial surgeons. Because of the importance of the zygomatic complex in midfacial skeletal orientation, comminuted zygomatic arch fractures can lead to significant morbidity and deformity. Common repair modalities include closed reduction via a Gilles approach, open reduction and internal fixation with screws and/or plating systems, primary bone grafting, and/or biosynthetics. In patients with significant comminution and arch defects, who are poor candidates for bone grafting, reconstruction can be difficu

A case report of Schatzker type VI tibial plateau fracture treated with double reverse traction closed reduction combined with minimally invasive percutaneous plate osteosynthesis technique: A case report.

RATIONALE: The knee joint is an important weight-bearing joint, tibial plateau fractures affect knee function and stability. High-energy intra-articular fractures involving the tibial plateau can cause management-related problems such as wound dehiscence; severe comminution leading to malalignment; and delayed complications such as varus collapse, implant failure, and arthritis of the knee joint. The treatment of severe or complex tibial plateau fractures can be quite difficult. Traditional methods of open reduction and plating require extensive exposures, which may further compromise soft tis

Tibial pilon fracture repair using Ilizarov external fixation, capsuloligamentotaxis, and early rehabilitation of the ankle.

Open surgical treatment of comminuted tibial pilon fractures is associated with substantial complications in many patients. Indirect reduction and stabilization of fractures by means of distraction using a circular external fixator can be a useful method of achieving satisfactory joint restoration, and employs the principles of closed reduction to realign disrupted bones and joint structures. In this report, we describe the case of a patient with a comminuted tibial pilon fracture treated by means of closed reduction and stabilization using a circular external fixator.

Clinical outcomes of treatment with locking compression plates for distal femoral fractures in a retrospective cohort.

BACKGROUND: Plate fixation is one of the standard surgical treatments for distal femoral fractures. There are few reports on the relationship between the screw position and bone union when fixing by the bridging plate (relative stability) method. METHODS: This retrospective study included 71 distal femoral fractures of 70 patients who were treated with the locking compression plate for distal femur (DePuy Synthes Co., Ltd, New Brunswick, CA, USA). The following measurements were evaluated and analyzed: (1) bone union rate, (2) bridge span length (distance between screws across the fracture),

Fracture location-type synergy impacts stability in surgical stabilization of rib fractures.

Early chest wall stability following surgical stabilization of rib fractures (SSRF) is directly associated with respiratory complications. However, the influence of fracture location and type on rib fracture fixation stability and the underlying biomechanical mechanisms remain unclear. This study employs finite element analysis to quantify the impact of different fracture locations and types on rib fracture fixation stability, identify high-risk fracture patterns, and provide biomechanical rationale. Three-dimensional models of the fourth rib were constructed from computed tomography data of e

Radial head fractures.

The radial head is the most commonly fractured bone of the elbow, with most fractures occurring in women older than 50 years. The radial head is an important stabilizer for valgus, axial, and posterolateral rotational forces. Loss of articular segments of the radial head negatively affects elbow kinematics and stability. Most fractures are treated nonsurgically. Indications for nonsurgical treatment include nondisplaced and isolated displaced fractures without a block to motion. Fragment excision is indicated when a mechanical block from a small, displaced fragment impedes elbow motion. If tec

Internal fixation of dorsally displaced fractures of the distal part of the radius. A biomechanical analysis of volar plate fracture stability.

BACKGROUND: Volar plate fixation with use of either a locking plate or a neutralization plate has become increasingly popular among surgeons for the treatment of dorsally comminuted extra-articular distal radial fractures. The purpose of the present study was to compare the relative stability of five distal radial plates (four volar and one dorsal), all of which are commonly used for the treatment of dorsally comminuted extra-articular distal radial fractures, under loading conditions simulating the physiologic forces that are experienced during early active rehabilitation. METHODS: With use

Lateral talar process fracture and peroneal tendon dislocation: a previously unrecognized injury complex.

BACKGROUND: Lateral talar process fractures and peroneal tendon dislocations are frequently unrecognized at the time of injury. Lateral process fractures were initially classified by Hawkins as three types. Type II injuries are comminuted fractures involving both the talofibular and talocalcaneal articular surfaces. The purpose of this retrospective chart review was to describe an injury complex of Type II lateral talar process fracture with peroneal tendon dislocation. MATERIALS AND METHODS: Between January of 1995 and December 2006, 13 patients were seen for a lateral talar process fracture

Inferior alveolar nerve injuries associated with mandibular fractures.

The study evaluates the incidence of inferior alveolar nerve injuries in mandibular fractures, the duration of their recovery, and the factors associated with them. Fifty-two patients with mandibular fractures involving the ramus, angle, and body regions were included in this study; the inferior alveolar nerve was examined for neurological deficit posttraumatically using sharp/blunt differentiation method, and during the follow-up period the progression of neural recovery was assessed. The incidence of neural injury of the inferior alveolar nerve was 42.3%, comminuted and displaced linear frac

Extensor Tendons Rupture after Volar Plating of Distal Radius Fracture Related to a Dorsal Radial Metaphyseal Bone Spur.

Extensor tendon ruptures due to volar plating in distal radius fractures have mostly been described in relation with technique failures such as screw prominence and drill penetration. We report the case of a 71-year-old female with a C2 distal radius fracture with severe dorsal metaphyseal comminution. The patient underwent surgical treatment with reduction of the large fragments and fixation with a volar locking plate; the small dorsal metaphyseal nonarticular fragments were not reduced. Six months later, the patient developed extensor digitorum communis (EDC) rupture and extensor indicis pro

Lag screw with DHS (LSD) for vertical angle femoral neck fractures in young adults.

Vertical Pauwels Type III fractures of the neck of femur in young patients are difficult fractures to treat. These are usually as a result of high energy trauma which are associated with a significant degree of fracture comminution in the neck of femur. This makes the fracture reduction and fixation difficult. Many fixation methods and implants have been described for use in these fractures but are not without reported complications such as non-union and varus collapse. In this article, we highlight the incidence of fracture comminution in both the anterior and posterior cortices of the femora

Displaced intra-articular calcaneal fracture treated surgically with limited posterior incision.

The incidence of calcaneal fracture has been slowly increasing; however, the ideal treatment for displaced intra-articular fracture is not available yet, even though the fracture brings frequent complication and disability. Between April 1991 and March 1998, we treated 103 displaced intra-articular calcaneal fractures of 92 patients surgically with limited posterior incision, modified Gallie approach. There were thirty-seven tongue-type fractures, fifteen tongue-type fractures with moderate comminution, nineteen joint-depression fractures, twenty-nine joint-depression fractures with moderate c

Scaphoid Plate Osteosynthesis in Complex Fractures and Wrist Trauma: A Case Series.

Introduction Treatment of scaphoid fracture is challenging due to its unique blood supply and geometry. Traditionally, a headless compression screw is the standard treatment for unstable scaphoid fracture. Some fractures are complex, for example, comminution with bone loss. A scaphoid plate is an option in these difficult fractures providing adequate rotational stability. Aim To share our experience in using scaphoid plates in complex wrist trauma and comminuted fractures. Method and material Complex wrist trauma involving scaphoid fractures that were comminuted and multi-fragmentary fractures

Endoscopy-Assisted Frontal Sinus Obliteration.

Comminuted fractures of the anterior table of the frontal sinus involving frontonasal ducts are traditionally treated with sinus obliteration. This technique entails precise mucosa removal which requires unimpeded visualization of the bony recesses. To achieve adequate access to the mucosa, temporary removal of a large part of the frontal bone may often be necessary. This in turn increases fracture comminution requiring placement of additional hardware associated with the risk for bony malalignment, infection, plate palpability, and visibility, especially in thin-skin individuals. Enhanced SJ Tripod Suspension Technique: Achieving Comfort and Predictable Outcomes in Severe Nasal Fracture Treatment.

Managing comminuted and complicated nasal fractures is challenging because it requires obtaining both functional and cosmetic satisfaction. This research examines patient satisfaction with the closed reduction strategy paired with the use of SJ tripod suspension and splintage technique for severe nasal fractures. Our hypothesis is that proper duration of internal and external splintage is the key to satisfying results.Case series.A total of 186 consenting individuals presenting over 10 years with severe nasal fractures underwent this technique. The average duration of splintage was 10 days. Pat

Alternatives in the treatment of ipsilateral fractures of the hip and femur.

Eight cases of ipsilateral femoral neck and shaft fractures were reviewed. The diagnosis of the femoral neck fracture was delayed in two cases. Seven of the femoral shaft fractures were comminuted. In six cases, both fractures were treated with rigid internal fixation. A combination of traction and internal fixation was used in two cases. Two cases of avascular necrosis of the femoral head were observed. The fracture reduction was insufficient in both cases and the diagnosis of femoral neck fracture was delayed in one case. A radiograph of the pelvis is mandatory in high-velocity trauma victim
