The rare presentation of a frontal mucocele complicated by a Pott's puffy tumor and an epidural-cutaneous fistula: illustrative case.

BACKGROUND: Frontal mucoceles develop due to accumulation of mucoid secretions within the frontal sinuses. They can lead to serious consequences with further expansion and destruction of the surrounding bones or infection that might spread intracranially. OBSERVATIONS: The authors present a case of a 37-year-old male with a frontal mucocele and the rare presentation of Pott's puffy tumor and an epidural-cutaneous fistula, as well as a literature review of previously reported cases of epidural cutaneous fistula and sinocutaneous fistula, their predisposing factors, and their management. A Clinical Case of Delayed Posttraumatic Frontal Sinus Cerebrospinal Fluid Leakage Management Via External Surgical Approach.

Cerebrospinal fluid rhinorrhea is frequently encountered after a fracture of the skull base. Individual fractures of the posterior wall of the frontal sinus after brain injury are uncommon. The authors present a case of a 33-year-old man with a distant history of skull base injury after a traffic accident, 12 years ago. He presented with intermittent rhinorrhea and reported 2 episodes of bacterial meningitis the last 7 years since the injury. Cerebrospinal fluid leakage was confirmed with β2-transferrin testing. Computed tomography and magnetic resonance imaging revealed a small defect of the

Study of Varied Anatomy of Supraorbital Foramina Present Along the Supraorbital Margin Along With Clinical Implications.

Supraorbital margin encompasses supraorbital notch, which is situated at the junction of medial 1/3 rd and lateral 2/3 rd of supraorbital margin. But sometimes supraorbital notch is converted into foramen known as supraorbital foramen by ossification of periosteal ligament. Supraorbital foramen/notch transmits supraorbital neurovascular bundle, which may be injured during maxilofacial surgeries creating complications. The aim of study is to highlight the incidence and location of supraorbital foramen along with clinical implications. Study was conducted in the department of Anatomy using 174 s

Case Report - Pyknodysostosis.

Pyknodysostosis is an extremely rare lysosomal storage disease of the bone. Osteosclerosis of the skeleton is due to a decrease of the bone turnover. A plethora of clinical and radiological findings are observed. Patients are short statured with acroosteolysis and dense bones and develop multiple spontaneous bone fractures. A female patient presented with frontal bossing, small and thick fingers and toes. The classical clinical and radiological findings confirmed it as pyknodysostosis.

The relationship between frontal sinus morphology and skeletal maturation.

BACKGROUND: The aim of this study is to evaluate the relationship between frontal sinus morphology and hand-wrist bone maturation by using postero-anterior (PA) cephalometric radiographs. MATERIALS AND METHODS: The study sample consisted of 220 patients divided into 11 groups based on the hand-wrist radiographs. The right and left maximum height, width and area of the frontal sinus parameters were measured in PA cephalometric radiographs of 220 subjects aged 8-18 years. The hand-wrist skeletal maturation stages were evaluated on the hand-wrist radiographs using the method of Fishman. The Kend

Diploic Bone Channel Drilling Facilitates Dissection of the Midline Dura and Protects the Superior Sagittal Sinus in Hyperostosis Frontalis Interna.

Patients with space-occupying lesions adjacent to the superior sagittal sinus (SSS) present several technical considerations. For craniotomi … frontal two-part parasagittal craniotomy was recommended. The preoperative imaging showed that the frontal bone was thick, with irregularity of the inner table. Intraoperatively, a channel was drilled in the diploic space of the bone, leaving the outer table intact. This provided a thin lip of the inner table that could be dissected over a short distance and …

Dinosaur ossification centres in embryonic birds uncover developmental evolution of the skull.

Radical transformation of the skull characterizes bird evolution. An increase in the relative size of the brain and eyes was presumably related to the loss of two bones surrounding the eye, the prefrontal and postorbital. We report that ossification centres of the prefrontal and postorbital are still formed in bird embryos, which then fuse seamlessly to the developing nasal and frontal bones, respectively, becoming undetectable in the adult. The presence of a dinosaur-like ossification pattern in bird embryos is more than a trace of their evolutionary past: we show how persistent modularity of…

The Bmp pathway in skull vault development.

The Bmp pathway is of critical importance in the development of the skull vault. Analysis of gain and loss of function phenotypes of Bmp pathway effectors, particularly Msx genes, has shown that the Bmp pathway functions in the growth of both mesodermal and neural crest-derived calvarial bones. It is required for the development of the frontal and parietal bones during the interval between the initial osteogenic mesenchymal condensations at E12.5 to the apposition of the paired frontal and parietal bones at E18.5. During postnatal development, forced expression of the Bmp inhibitor, noggin, ma

Intraosseous Orbital Cavernous Hemangioma with Frontal Extension and Dural Involvement.

Primary intraosseous cavernous hemangiomas are rare skull lesions that are not typically known to involve the orbital bones or the dura. We describe a rare case of a fronto-orbital bone cavernous hemangioma with extension into the dura. A 68-year-old female presented with a one-year history of diplopia with discomfort around her left orbit. Magnetic resonance images demonstrated a mass in the left frontal skull extending into the orbital rim. The patient underwent a craniotomy for tumor resection. Dural invasion was found intraoperatively. Gross total resection and reconstruction were achieved

Osteomyelitis of Frontal Sinus: A Rare Sequelae of Acute Bacterial Sinusitis Associated With Anatomical Variations in the Frontal Sinus Drainage Pathway.

Frontal osteomyelitis is characterized by localized osteal inflammation of the frontal bone. This is a rare complication of acute frontal sinusitis. The present case is being reported to highlight the likely role of anatomical variations in frontal sinus drainage pathways in the causation of this complication apart from other known predisposing factors like young age and immunocompromised state. The patient initially presented with seizures, fever, and headache and was diagnosed with viral encephalitis. However, the symptoms gradually progressed to cause right eye swelling and an increase in t…

Frontal Sinus Morphometry in Relation to Surgically Relevant Landmarks in the Middle East Population: Can We Globalize?

BACKGROUND: The frontal bone is frequently approached during neurosurgical procedures. Feared complications of such surgeries include cerebrospinal fluid leak, among others, and frequently result from a breach of the frontal sinus. For this reason, the sinus should be avoided when possible. The supraorbital notch (SON) is a reliable and easily identifiable surgical landmark and its relation to the frontal sinus has been previously studied. However, the frontal sinus shows significant variability in size and shape between populations. METHODS: In the present study, we investigate the frontal s…

Vascular pericranial graft: a viable resource for frontal sinus obliteration.

Inappropriate treatments of frontal sinus fractures may lead to serious complications, such as mucopyocele, meningitis, and brain abscess. Assessment of nasofrontal duct injury is crucial, and nasofrontal duct injury requires sinus obliteration, which is often accomplished by autologous grafts such as fat, muscle, or bone. These avascular grafts have an increased risk of resorption and infection, as well as donor site morbidity. For these reasons, pericranial flap, which is vascular, should be used for frontal sinus obliteration. The pericranial flap presented with less morbidity procedure and

The Pott's puffy tumor: a dangerous sign for intracranial complications.

The Pott's puffy tumor is a subperiosteal abscess of the frontal bone associated with osteomyelitis. The purpose of this article is to alert the physician to the severe complications of this entity. The records of six patients were reviewed retrospectively. There were four adults and two adolescents. Nasal endoscopy showed edematous, polypoid mucosa in middle meatus in three and nasal polyps in the rest. At initial admission, two had orbital subperiosteal abscess, but normal cranial CT findings. During hospitalization, three experienced frontal lobe abscess and one frontal cerebritis. Endoscop…

Intracranial hemorrhage complicating an orbital decompression.

We report a case of intracerebral hemorrhage in the left frontal lobe following a left medial wall orbital decompression for thyroid-related optic neuropathy. There was no obvious fracture in the orbital roof bone but the hemorrhage was felt to occur from a disruption in the orbito-frontal branch of the anterior cerebral artery. The patient was asymptomatic and remained so during the follow-up period.

[Plastic surgery of the frontonasal anastomosis using a hydroxyapatite prosthesis].

The authors offer a method of plastic reconstruction of the frontonasal anastomosis after radical surgery on the frontal sinus. The prosthesis made of hydrooxyappatite ceramics is established in the anastomosis created during radical surgery without additional fixation. Due to special biological properties of the material, the prosthesis firmly consolidates with bone walls of the anastomosis warranting reliable drainage and aeration of the frontal sinus.
