Development and validation of a visual nomogram for predicting clinically significant prostate cancer in negative mpMRI using 68Ga-PSMA PET/CT.

Multi-parametric magnetic resonance imaging (mpMRI) is a valuable medical technology for detecting clinically significant prostate cancer (csPCa). The diagnostic accuracy of mpMRI for csPCa in negative mpMRI (PI-RADS 1-2) remains suboptimal, underscoring the need for improvements for csPCa. This study aimed to build a visual predictive nomogram for early detection of csPCa in negative mpMRI. We retrospectively reviewed 303 men from our institution who simultaneously underwent 68Ga-PSMA-11 PET/CT and mpMRI before a biopsy between March 2020 and July 2022 and 130 men from the outside institution

Dual-tracer PET/CT identifies an aggressive PSMA-negative/FDG-positive phenotype predicting early failure of radical prostatectomy in treatment-naive prostate cancer.

BACKGROUND AND PURPOSE: The metabolic phenotype of negative prostate-specific membrane antigen (PSMA) and positive [18F]F-fluorodeoxyglucose ([18F]F-FDG) uptake is an established indicator of unfavorable prognosis in advanced-stage prostate cancer. It is also associated with resistance to PSMA-targeted therapies. However, its prognostic significance in the preoperative setting for treatment-naive patients scheduled for therapy with curative intent remains to be elucidated. This study aimed to investigate whether metabolic heterogeneity assessed by dual-tracer [68 Ga]Ga-PSMA-11 and [18F]F-FDG P

A dual-tracer approach for enhanced nodal staging in prostate cancer: the role of [18F]FDG PET/CT for detecting [68 Ga]Ga-PSMA-11-negative metastases.

BACKGROUND AND PURPOSE: A subset of newly diagnosed prostate cancer patients harbor occult lymph node metastases (LNM) that are not detected by standard [68 Ga]Ga-PSMA-11 PET/CT, leading to potential understaging and suboptimal treatment planning. This study investigates the performance of [18F]FDG PET/CT as a secondary imaging tool to identify these PSMA-negative LNM. METHODOLOGY: We retrospectively analyzed 133 patients with newly diagnosed prostate cancer who were considered node-negative (miN0) by [68 Ga]Ga-PSMA-11 PET. All patients underwent a subsequent [18F]FDG PET/CT scan followed by

68Ga-Prostate-Specific Membrane Antigen PET/CT Uptake in Intraventricular Meningioma in the Choroid Plexus.

A 68-year-old man in whom a prostate cancer biochemical recurrence was being investigated was referred for a full-body Ga-prostate-specific membrane antigen PET/CT. Focal uptake in a lesion on the choroid plexus projection was detected. Patient had no neurological complaints. A previous MRI performed 4 years ago showed the same lesion. According to the location and radiological and clinical characteristics, the diagnosis of meningioma was defined. The interpretation of prostate-specific membrane antigen PET/CT must take into account the low frequency of metastases in sites such as the central

68Ga-PSMA Uptake in Combined Hepatocellular Cholangiocarcinoma With Skeletal Metastases.

Ga-PSMA PET is a rapidly evolving imaging modality for whole-body staging of prostate cancer. We report a case of a 70-year-old man with mildly elevated prostate-specific antigen (8.1 μg/L) and clinical suspicion of prostate cancer (osteoblastic metastases on radionuclide bone scan) who was referred for Ga-PSMA PET/CT for primary staging. Multiple Ga-PSMA-avid skeletal foci, mostly associated with lytic changes on low-dose CT, as well as hepatic foci of avid Ga-PSMA uptake, were identified. No abnormal focus of avid Ga-PSMA uptake was identified in the prostate. On histopathological examinatio

Follicular Thyroid Adenoma Showing Avid Uptake on 68Ga PSMA-HBED-CC PET/CT.

68Ga-prostate-specific membrane antigen (PSMA) PET/CT imaging is a relatively new imaging technique used to evaluate the extent of disease in prostate carcinoma. Various other neoplasms may also express PSMA and show uptake on PSMA PET/CT scan. We report a case of a 62-year-old man who had a PSMA PET/CT scan for restaging of prostate carcinoma. A PSMA-avid thyroid lesion was identified, and subsequent tissue sampling confirmed the diagnosis of follicular thyroid adenoma. It is important to be aware of this possibility to avoid scan misinterpretation. Tissue biopsy of PSMA-avid thyroid lesions

Radiolabeling of lipid-based nanoparticles for diagnostics and therapeutic applications: a comparison using different radiometals.

Radiolabeling of nanoparticles (NPs) has been performed for a variety of reasons, such as for studying pharmacokinetics, for imaging, or for therapy. Here, we describe the in vitro and in vivo evaluation of DTPA-derivatized lipid-based NP (DTPA-NP) radiolabeled with different radiometals, including (111)In and (99m)Tc, for single-photon emission computed tomography (SPECT), (68)Ga for positron emission tomography (PET), and (177)Lu for therapeutic applications. PEGylated DTPA-NP with varying DTPA amounts, different composition, and size were radiolabeled with (111)In, (177)Lu, and (68)Ga, usin

PSMA-Directed PET/MRI Enables Noninvasive Diagnosis and Prognosis in Patients with Increased PSA Levels: Results from the Prospective Randomized RAPID Trial.

Systematic transrectal ultrasound-guided biopsy lacks accuracy in the primary diagnosis of prostate cancer (PCa) and causes side effects. We investigated prostate-specific membrane antigen (PSMA)-targeted PET/MRI as a less-invasive alternative for biopsy guidance and risk assessment. Methods: The RAPID study was a randomized, controlled, single-center, open-label phase 3 trial comparing the diagnostic efficacy of 68Ga-PSMA-11 PET/MRI with systematic transrectal ultrasound-guided prostate biopsy. In total, 220 men with suspected PCa were randomized to either a standard (random 12-core biopsy; R

Beyond FDG: Serial 68 Ga-FAPI PET/CT for Staging and Treatment Response Assessment in Metastatic Pleomorphic Lobular Breast Cancer.

68 Ga-FAPI PET/CT has demonstrated higher sensitivity than 18 F-FDG PET/CT in breast cancer, particularly in invasive lobular carcinoma. However, its role in treatment response assessment remains limited. We report a case of recurrent metastatic pleomorphic lobular breast cancer in which baseline 68 Ga-FAPI PET/CT revealed extensive regional and distant disease underestimated by 18 F-FDG PET/CT. Serial follow-up FAPI PET/CT after initiation of olaparib demonstrated marked response with near complete resolution of disease, highlighting the potential utility of FAPI PET/CT for response assessmen

Preoperative prediction of pathological pelvic lymph node metastasis in prostate cancer using an elastic net model integrating clinicopathologic and 68Ga-PSMA PET/CT features: development and temporal validation.

Preoperative decision-making regarding extended pelvic lymph node dissection (ePLND) in prostate cancer remains challenging. Existing nodal-risk prediction tools are mainly based on clinicopathologic variables and may show suboptimal performance across different clinical settings. This study aimed to develop and temporally validate a preoperative prediction model for pathological pelvic lymph node metastasis (pN1) by integrating clinicopathologic and 68Ga-PSMA PET/CT-derived features in a single-center Chinese surgical cohort. In this single-center retrospective study, consecutive patients wit
