Microbial Agents as Putative Inducers of B Cell Lymphoma in Sjögren's Syndrome through an Impaired Epigenetic Control: The State-of-The-Art.

INTRODUCTION: Understanding the mechanisms underlying the pathogenesis of Sjögren's syndrome (SS) is crucially important in order to be able to discriminate the steps that lead to B cell transformation and promptly identify the patients at risk of lymphomagenesis. The aim of this narrative review is to describe the evidence concerning the role that infections or dysbiosis plays in the epigenetic control of gene expression in SS patients and their possible involvement in B cell lymphomagenesis. MATERIALS AND METHODS: We searched the PubMed and Google Scholar databases and selected a total of 9

Modification of chest radiography exposure parameters using a neonatal chest phantom.

BACKGROUND: The acquisition of chest radiographs in neonates is of critical importance in diagnostics because of the risk of respiratory distress syndrome and pneumothorax in preterm infants. OBJECTIVE: To achieve a dose reduction while preserving a diagnostic image quality for chest radiographs of neonates. MATERIALS AND METHODS: All radiographs, generated on a fully digital X-ray unit by using a neonatal chest phantom, were evaluated under variation of the tube voltage (40-70 kV) and mAs levels (1-10.2 mAs) with and without an additional 0.1-mm copper (Cu) filtration. Noise, contrast and c

Albumin Use in Patients With Cirrhosis in France: Results of the "ALBU-LIVE" Survey: A Case for Better EASL Guidelines Diffusion and/or Revision.

INTRODUCTION: The use of human albumin for the management of cirrhosis has increased. Recommendations have been published for therapeutic paracentesis (TP), spontaneous bacterial peritonitis (SBP), and type 1 hepatorenal syndrome (HRS). The goal of this survey was to assess the prescription practices of French hepatogastroenterologists. METHODS: All hepatogastroenterologists were contacted. The questionnaire evaluated (1) the use of albumin in validated indications and (2) the prescription of albumin for nonvalidated clinical situations. RESULTS: Responses were analyzed from 451 (50.1%) prac

Patient-specific pediatric silicone heart valve models based on 3D ultrasound.

PURPOSE: Patient-specific heart and valve models have shown promise as training and planning tools for heart surgery, but physically realistic valve models remain elusive. Available proprietary, simulation-focused heart valve models are generic adult mitral valves and do not allow for patient-specific modeling as may be needed for rare diseases such as congenitally abnormal valves. We propose creating silicone valve models from a 3D-printed plastic mold as a solution that can be adapted to any individual patient and heart valve at a fraction of the cost of direct 3D-printing using soft materia

[Screening for congenital infection by Trypanosoma cruzi in France].

The importance of congenital transmission of Chagas' disease increases with its emergence in communities infected with Trypanosoma cruzi, but where vector transmission has never existed or is fully controlled through vector control campaigns. In both endemic and non-endemic areas, the rates of mother-to-child transmission (MTCT) could be the same, by 5%, generating a constant source of new cases of the disease. Risk factors for vertical transmission are not fully elucidated, but the effectiveness of the adaptive immune response and the genetic susceptibility of both the mother and the child ar

Upper airway muscles awake and asleep.

Upper airway (UA) structures are involved in different respiratory and non-respiratory tasks. The coordination of agonist and antagonist UA dilators is responsible for their mechanical function and their ability to maintain UA patency throughout the respiratory cycle. The activity of these muscles is linked with central respiratory activity but also depends on UA pressure changes and is greatly influenced by sleep. UA muscles are involved in determining UA resistance and stability (i.e. closing pressure), and the effect of sleep on these variables may be accounted for by its effect on tonic an

Immunopathogenesis of Immune Checkpoint Inhibitor-Induced Myositis, Myocarditis and Myasthenia Gravis Overlap Syndrome: A Mechanistic Synthesis of a Mitochondrial Autoantigen Hypothesis.

Immune checkpoint inhibitors (ICIs) have improved outcomes for cancer patients. However, they can cause toxicities called immune-related adverse events (irAEs). One serious toxicity is the myocarditis, myositis and myasthenia gravis (MMM) overlap syndrome, an inflammatory syndrome involving the heart, skeletal muscles and neuromuscular junction. Most studies on the pathogenesis of myocarditis and myositis emphasise effector CD8+ T-cell driven inflammation while myasthenia gravis (MG) is believed to be a purely antibody-driven process. This approach has several challenges: (i) lack of a unified

Gradenigo syndrome in a child: case report and literature review.

INTRODUCTION AND IMPORTANCE: Gradenigo syndrome is a rare but potentially serious complication of suppurative otitis media, characterized by the classic triad of suppurative otorrhea, abducens nerve (VI) paralysis, and pain in the trigeminal nerve (V) territory. Although the advent of antibiotic therapy has significantly reduced its incidence, this condition remains relevant, particularly in children, requiring early recognition and appropriate multidisciplinary management. CASE PRESENTATION: We report the case of a 13-year-old girl who presented with a painful swelling behind her right ear t

Implementation of a workplace healthy lifestyle education intervention for metabolic syndrome risk reduction in Ethiopia: a mixed-methods process evaluation.

BACKGROUND: The prevalence of metabolic syndrome among office workers is markedly high, highlighting an urgent need for effective workplace interventions. In response to this need, a theory-informed healthy lifestyle education intervention was developed and implemented among office workers in Ethiopia, resulting in significant reductions in key metabolic risk factors and improvements in health-related behaviours. However, understanding the implementation process, how the intervention worked in practice, under what conditions, and why is critical to inform scale-up, adaptation, and sustainabili

Zorevunersen in Children and Adolescents with Dravet Syndrome.

BACKGROUND: Dravet syndrome is a severe developmental and epileptic encephalopathy caused primarily by SCN1A haploinsufficiency. Risks of sudden unexpected death in epilepsy and cognitive deficits are higher among patients with this syndrome than in the general population with epilepsy. The effects of zorevunersen, an antisense oligonucleotide designed to up-regulate NaV1.1 sodium channels, in patients with Dravet syndrome are not known. METHODS: We enrolled patients 2 to 18 years of age with Dravet syndrome who were receiving standard antiseizure medications in two phase 1-2a, open-label, mu

Electro-Acupuncture Effects Measured by Functional Magnetic Resonance Imaging-A Systematic Review of Randomized Clinical Trials.

INTRODUCTION: Electro-acupuncture, an innovative adaptation of traditional acupuncture, combines electrical stimulation with acupuncture needles to enhance therapeutic effects. While acupuncture is widely used, its biological mechanisms remain incompletely understood. Recent research has explored the neurophysiological aspects of acupuncture, particularly through functional magnetic resonance imaging (fMRI) to investigate its effects on brain activity. METHODS: In this systematic review, we conducted an extensive search for randomized clinical trials examining electro-acupuncture effects meas

Artificial intelligence-assisted reinterpretation of preclinical progeria research suggests a hierarchical nuclear-vascular resilience framework with translational implications.

Preclinical research traditionally advances through hypothesis-driven experimentation that establishes mechanistic pathways to support translational development. While this approach has generated major biological insights, it may underemphasize alternative organizational patterns embedded within complex datasets, particularly in rare diseases where opportunities for experimental reiteration are limited. Recent advances in conversational artificial intelligence (AI) provide an opportunity to support structured analytical dialogue as a complementary approach for re-examining validated experiment

Unraveling the complexity of PRRSV-2 recombination: recombination frequency, replication efficiency, and backbone fitness define highly recombinogenic backbone and evolutionary bottlenecks.

Recombination is a primary driver of porcine reproductive and respiratory syndrome virus (PRRSV) evolution. Currently, lineage 1 NADC30-like virus (L1C) and lineage 8 HP-PRRSV (L8E) recombinants dominate in China, whereas recombinants harboring the lineage five backbone are infrequently detected. To elucidate the mechanisms underlying this disparity, we investigated the interplay between recombination frequency, viral fitness, and strain-specific phenotypes using co-infection models. Next-generation sequencing (NGS) revealed that the lineage 5 replicase is prone to template switching, generati

[Wegener's granulomatosis: immunologic aspects of diagnosis, etiology and therapy].

Wegener's granulomatosis (WG) is a rare disease, which has, however, been diagnosed more frequently in the past few years. The etiology is unclear; the pathogenesis is only vaguely understood. The association between HLA-DR2 and/or B8 and WG indicates that there is a genetic predisposition. Without treatment WG is said to run a malignant course with a median survival time of approx. 6 months. With immunosuppressive treatment, however, it is said to be possible to induce long-term remission in about 90% of the cases. Our experience only partially confirms these figures from other countries. Fro

Modification of locomotion-respiration coupling in patients with obstructive sleep apnea syndrome.

INTRODUCTION: Locomotion is a complex motor task integrating sensory-motor and cognitive inputs through central pattern generator control. Locomotion-respiration coupling (LRC), defined as the synchronization between gait and breathing events, has been described during running and walking. Patients with obstructive sleep apnea syndrome (OSAS) exhibit altered gait patterns during wakefulness, which appear to be partially independent of nocturnal symptomatic treatment. While intermittent hypoxia may induce long-term ("fixed") central nervous system alterations, OSAS patients also display "revers

Stabilizing Genetically Unstable Simple Sequence Repeats in the Campylobacter jejuni Genome by Multiplex Genome Editing: a Reliable Approach for Delineating Multiple Phase-Variable Genes.

Hypermutable simple sequence repeats (SSRs) are major drivers of phase variation in Campylobacter jejuni. The presence of multiple SSR-mediated phase-variable genes encoding enzymes that modify surface structures, including capsular polysaccharide (CPS) and lipooligosaccharide (LOS), generates extreme cell surface diversity within bacterial populations, thereby promoting adaptation to selective pressures in host environments. Therefore, genetically controlling SSR-mediated phase variation can be important for achieving stable and reproducible research on C. jejuni. Here, we show that natural "

Dominant optic atrophy: Culprit mitochondria in the optic nerve.

Dominant optic atrophy (DOA) is an inherited mitochondrial disease leading to specific degeneration of retinal ganglion cells (RGCs), thus compromising transmission of visual information from the retina to the brain. Usually, DOA starts during childhood and evolves to poor vision or legal blindness, affecting the central vision, whilst sparing the peripheral visual field. In 20% of cases, DOA presents as syndromic disorder, with secondary symptoms affecting neuronal and muscular functions. Twenty years ago, we demonstrated that heterozygous mutations in OPA1 are the most frequent molecular cau

Neurocognitive Profiles of Pediatric Patients with ESES, Generalized Epilepsy, or Focal Epilepsy.

To date, the neurocognitive profile and comorbid psychiatric risks associated with ESES, a syndrome that commonly coincides with a seizure disorder, in contrast to generalized or partial/focal epilepsy without ESES remains unclear. Accordingly, this preliminary study aimed to characterize the neurocognitive differences across pediatric patients with ESES, generalized or focal epilepsy, and risk for comorbid diagnoses (mood disorder, autism, intellectual disability, learning disability, ADHD). We included data from a total of 79 pediatric patients, including those with recently diagnosed ESES (
