Treatment of gingival recession using platelet concentrate with a bioabsorbable membrane and coronally advanced flap: a report of two cases.

The aim of this report is to present a new treatment strategy for gingival recession. The use of platelet concentrate gel in a collagen sponge carrier combined with a bioabsorbable membrane and coronally advanced flap procedure is demonstrated in two patients. In both patients, the recession defects treated were equal to or greater than 2 mm. Complete root coverage was achieved in both cases. The patients were satisfied with the esthetic result and appreciated the excellent soft tissue contour and texture. This technique requires further investigation.

Acellular dermal matrix allograft used alone and in combination with enamel matrix protein in gingival recession: histologic study in dogs.

Gingival recession was created in six mongrel dogs. The dogs were divided into two groups based on treatment: group 1--AlloDerm only, group 2--AlloDerm + Emdogain. The histologic results were compared. At the end of the study, the mean values were, for groups 1 and 2, respectively: 0.06 and 0.32 mm for cementum regeneration; -0.75 and -0.86 mm for bone regeneration; -2.15 and -3.11 mm for attachment level; and 4.90 and 5.51 mm for defect extent. The epithelial formation parameter was 2.88 mm in group 1 and 2.15 mm in group 2, which was a statistically significant difference. It could be conclu…

Analysis of the Community Periodontal Index of Treatment Needs--study on adults in France. IV. The significance of gingival recession.

A total of 1005 persons were examined using the CPITN criteria which were recorded for every tooth. All the teeth were also measured on both their buccal and lingual aspects to assess the amount of gingival recession. The combination of pocket depth and gingival recession was computed using a specially written program: 93.8 per cent of the teeth had 1 mm or less gingival recession; 82.5 per cent of the teeth with gingival recession did not present pockets; 26.6 per cent of all subjects had at least one tooth with gingival recession of 2 mm or more but only 9.9 per cent had at least one tooth w…

Difficulty score for the treatment of isolated gingival recessions with the coronally advanced flap: a preliminary reliability study.

OBJECTIVES: The treatment of gingival recessions (GRs) is operator-sensitive and dependent upon several local anatomical factors. The aim of this study was to introduce a difficulty score for the treatment of localized GRs with the coronally advanced flap (CAF) and to test its consistency among different operators. MATERIALS AND METHODS: A rubric (difficulty score) consisting of the assessment and grading of 8 anatomical parameters (anatomical papilla, apical and lateral keratinized tissue width, apical and lateral frenum, vestibulum depth, scar tissue, and mucosal invagination) is described …

Bilaminar Technique with Coronally Advanced Flap and Cryopreserved Human Amniotic Membrane in the Treatment of Gingival Recessions.

Gingival recessions are usually treated with surgical therapies which involve the use of connective tissue autograft in order to thicken the gingival tissue. This has an important biological repercussion on patients since they will have surgical wounds in two different oral cavity sites. In this case report, we offer an alternative technique for the treatment of a Miller class I gingival recession. A 40-year-old female patient has been treated with a combination of bilaminar and coronally advance flap techniques to cover a Miller class I gingival recession with addition of cryopreserved human …

Root surface modifiers and subepithelial connective tissue graft for treatment of gingival recessions: a systematic review.

BACKGROUND AND OBJECTIVE: Many techniques and flap designs have been used to treat gingival recession by root coverage, but subepithelial connective tissue graft (SCTG) seems to be the gold standard procedure. In an attempt to improve the healing process and increase the success rate of root coverage, some authors have used root modifiers, including different root conditioners, lasers, EMD, recombinant human growth factors and platelet-rich plasma (PRP). The aim of this systematic review was to evaluate the effects of root biomodification in clinical outcomes of gingival recessions treated wit…

Treatment of multiple gingival recessions adopting modified tunnel subepithelial connective tissue graft technique.

Gingival recession related to periodontal disease or developmental problems can result in root sensitivity, root caries, and esthetically unacceptable root exposures. In the past, multiple surgical procedures have been proposed to obtain root coverage on exposed buccal root surfaces. There has been great interest in the treatment of gingival recession defects, especially with subepithelial connective-tissue grafting (SCTG). Recent advances have focused on SCTG by the tunnel technique. This article highlights the esthetic results obtained by adopting a modification of the tunnel technique using…

Treatment of multiple adjacent gingival recessions with the tunnel subepithelial connective tissue graft: a clinical report.

This article describes a surgical periodontal plastic procedure for the coverage of multiple adjacent gingival recessions. This surgical technique is based on the construction of a tunnel under the gingival tissue by means of a sulcular incision beyond the mucogingival line without raising the papillae. A large connective tissue graft obtained from the palatal mucosa is introduced through this tunnel, covering the adjacent gingival recessions. A suturing technique to allow this graft to slip through the tunnel under the gingival tissues and to secure and stabilize the graft covering the recess…

Treatment of localized gingival recessions with coronally displaced flaps and citric acid. An experimental study in the dog.

The present study was designed to examine the effect of citric acid conditioning on the result of healing following treatment of localized gingival recessions with coronally displaced flaps. In 3 beagle dogs, localized gingival recessions were surgically created at the buccal surfaces of canines, premolars and molars. All experimental sites were allowed to accumulate plaque for a period of 6 months. Following preparation of notches in the buccal root surfaces at the level of the gingival margin, buccal and lingual mucoperiosteal flaps were raised. The exposed portion of the root surfaces was s…
