Surgeons' early experience with the Acessa™ procedure: gaining proficiency with new technology.

PURPOSE: Successful adoption of a new surgical procedure varies among practicing surgeons, and skill acquisition depends on the surgeon's innate ability, the complexity of the technique, and training. We report intraoperative and near-term postoperative outcomes from the Acessa procedure conducted by minimally invasive gynecologic surgeons new to Acessa, and report the surgeons' experiences during the training period. PATIENTS AND METHODS: The study was designed as a postmarket, prospective, single-arm, multicenter analysis of operative and early postoperative outcomes after proctored surgica

Cervical incompetence. A review.

Cervical incompetence is one of the main contributors to repeated pregnancy loss, accounting for approximately 25% of the cases. Typically it results in progressive cervical dilatation, leading to a painless second- or early-third-trimester abortion. In most instances cervical incompetence is the result of previous obstetric or gynecologic trauma; a congenital etiology is unusual. Despite the various reported diagnostic procedures, the obstetric history and frequent visual and digital examinations during pregnancy remain the most important factors in arriving at the correct diagnosis. In spite

Complications of female sterilization: immediate and delayed.

Surgical sterilization in women has changed dramatically over the past 20 years. The development of laparoscopy and minilaparotomy have made the procedure readily available even in developing countries. In the United States, changing social values and changes in hospital regulations have done as much as technology to account for the tremendous increases in the number of women undergoing sterilization. Improved sterilization procedures have resulted in lower costs for sterilization and lowered morbidity and mortality rates. Hysterectomy for sterilization alone carries unacceptable morbidity and

Self-retaining ureteral stent catheters in the management of urologic complications of gynecologic malignancies.

Fifteen patients with ureteral obstruction secondary to advanced and/or recurrent gynecologic malignancies underwent endoscopic (3) or surgical transvesical (12) insertion of Gibbons (12) or Double "J" (3) stent catheters. Renal function improvement was noted in all but one patient who had renal parenchymal disease. No complication attributable to the insertion procedure was recorded. This approach to the treatment of ureteral occlusion in patients with gynecologic cancer is discussed and recommended.

Perioperative sexual interest in women with suspected gynecologic malignancies.

OBJECTIVES: For women with gynecologic cancer, the impact of surgery on sexual interest and desire in the immediate and later postoperative period is not well characterized. The objective of this study was to report the perioperative trends of changing sexual interest and desire in a cohort of women undergoing surgery for suspected gynecologic malignancies. METHODS: This is an ancillary analysis of a cohort study analyzing health-related outcomes in women who underwent primary surgical management of a suspected gynecologic malignancy between 10/2013 and 10/2014. Subjects completed the Patient

Endoscopic management of uterine myoma.

This study was undertaken to evaluate the various gynecologic endoscopic surgical techniques including resectoscopic myomectomy, laparoscopic myomectomy, and laparoscopy assisted vaginal hysterectomy (LAVH) used in the treatment of uterine myomas. The medical records of 136 cases of uterine myomas treated using one or more of the gynecologic endoscopic surgical techniques in the Department of Obstetrics and Gynecology at Yonsei University were retrospectively reviewed from March 1997 to September 1998. Of the 136 cases reviewed, there were 40 submucosal myomas and 96 intramural and subserosal

Surgery for stress urinary incontinence.

The literature on the surgery of stress urinary incontinence from November 1989 to December 1990 is critically reviewed. This period was one of consolidation and assessment with surprisingly few reports of the management of complications and few reports of long-term follow-up. Retropubic procedures continue to be the mainstay of treatment in patients in whom a gynecologic abdominal procedure is indicated. For patients who do not require laparotomy, needle suspension procedures are increasingly used. The Raz procedure is frequently used for patients with type III incontinence. Possible complica

Immediate vaginal reconstruction following pelvic exenteration using the pedicled vertical Deep Inferior Epigastric Perforator (DIEP) flap: A technical note.

Immediate vaginal reconstruction is usually offered following pelvic exenteration for recurrent cervical cancer in women previously treated with radiotherapy or with severe radiation-induced fistulae. Introduction of muscle-sparing flaps, such as the pedicled vertical deep inferior epigastric perforator (DIEP) flap, provides viable tissues for vaginal reconstruction and is associated with reduced donor-site morbidity. This report describes the surgical technique, which is one of the procedures of choice for vaginal reconstruction. It is safe and beneficial, especially for women with gynecologi

Safety in Minimally Invasive Surgery.

Conventional and robot-assisted laparoscopic gynecologic surgery offers many advantages over a traditional laparotomy. However, these minimally invasive approaches can present their own particular risks. To ensure patient safety, procedures must be properly planned and performed by a skilled surgeon. Pre-operative patient optimization can help ensure safety and efficiency. Additional risks before starting the actual procedure arise from unique requirements for patient positioning and the need for peritoneal access. The authors discuss these risks and the importance of a thorough working knowle

An operative technique for laparoscopic hysterectomy using a retroperitoneal approach.

The technique used for abdominal hysterectomy does not lend itself well to a laparoscopic approach because vital structures will be difficult to visualize and retroperitoneal spaces difficult to open. An entirely different approach is required. An operative technique for laparoscopic hysterectomy is based on a systematic dissection of the retroperitoneum in a very precise sequence of operative steps. It provides complete control of the operative field and allows visualization of all important retroperitoneal structures. The technique is simple to learn, and requires no special surgical skills

Enhanced Recovery After Surgery (ERAS) in gynecologic surgery: hot topic debates at the 2025 ERAS World Congress.

INTRODUCTION: The Enhanced Recovery After Surgery pathway has transformed peri-operative care in gynecologic surgery through multi-disciplinary, evidence-based protocols. However, real-world adherence to and interpretation of specific Enhanced Recovery After Surgery elements remain heterogeneous, with ongoing discussion about their feasibility and clinical relevance. During the 2025 Enhanced Recovery After Surgery World Congress in Turin, Italy, a rapid-fire debate session addressed 4 "hot topics" in gynecologic Enhanced Recovery After Surgery implementation. GLYCEMIC CONTROL: Peri-operative

The Central Role of Nursing in Enhanced Recovery After Surgery for Gynecologic Laparoscopy.

Enhanced Recovery After Surgery (ERAS) has become increasingly prominent in gynecologic laparoscopic practice, yet the specific contributions of nursing to its implementation and outcomes remain poorly characterized in the existing literature. This integrative review aimed to synthesize and critically appraise current evidence on the role of nursing across the full perioperative continuum of ERAS in gynecologic laparoscopic surgery, covering both benign and oncologic indications. Guided by the Whittemore and Knafl framework, an integrative review design was employed to synthesize heterogeneous

Cases of pleural effusion possibly due to clinical pleuroperitoneal communication in the perioperative period of emergency gynecologic surgery: Case series and literature review.

Pleuroperitoneal communication poses a respiratory failure risk due to pleural fluid accumulation with thoracic migration of ascites. Here, we discuss the following cases: Case 1: A woman was diagnosed with a ruptured ovarian tumor with right pleural fluid and ascites, without respiratory failure. Ovarian cystectomy was performed with inadequate removal of ascites. Postoperatively, respiratory failure occurred, and thoracentesis detected pleural fluid resembling ascites. Case 2: A woman was diagnosed with a ruptured ectopic pregnancy with right pleural fluid and ascites without respiratory fai

A bibliometric analysis on laparoscopic pelvic floor surgery from 1996-2022.

INTRODUCTION: Pelvic organ prolapse is a common condition in middle-aged and older women. Laparoscopic pelvic floor surgery is frequently researched as a treatment for this issue. However, the effectiveness of this procedure has only been the subject of a few studies. METHODS: To quantitatively evaluate and display the relevant literature from the Web of Science database, we set the publication period of the literature from 1996 to 2022 and used VOSviewer and CiteSpace for visual analysis. RESULTS: Laparoscopic pelvic floor surgery research has increased year-on-year, with 1003 publications
