Emerging therapeutic agents for onychomycosis.

Onychomycosis is a frequent disorder that represents the most prevalent fungal infection, particularly among older individuals. Diverse fungi of the dermatophyte, non-dermatophyte mold and yeast families have been reported to be responsible for onychomycosis. The output from the pharmaceutical industry of new antifungals to treat onychomycosis has been limited over the last decade. Present treatment options include both oral and topical drugs, with oral therapies giving better outcomes. However, neither of these treatment options provides high cure rates that are durable. At present, azoles an

The hand-foot syndrome--a frequent secondary manifestation in antineoplastic chemotherapy.

The hand-foot syndrome (HFS) (palmoplantar erythrodysesthesia) designates acute, painful erythemas of the palms and soles of the feet caused by antineoplastic chemotherapies. The most frequent trigger substances are 5-fluoruracil and its derivates. At maximum severity, the HFS is bullous to erosive or ulcerous in character. The pathogenesis has not yet been clarified. Histologically, the HFS is characterized by a toxic keratinocyte reaction. Furthermore, there is sub-basal edema with a tendency to bullae, dilated blood and lymph capillaries and usually only mild perivascular lymphocytic infilt

Calciphylaxis involving both the upper and lower extremities.

Calciphylaxis is an uncommon complication of end stage renal disease (ESRD) and secondary hyperparathyroidism. It characterized by cutaneous necrosis with mural calcifications and thrombosis in the small vessels of dermis. It is important to diagnose and treat, because of mortality rate from calciphylaxis is very high. We present the case of a patient with ESRD and type II diabetes mellitus developing calciphylaxis of the both upper and lower extremities had normal corrected calcium-phosphate product level. After amputation, necrosis was showed rapid progression resulting in death in one month

Fluconazole (Fungolon) in the treatment of onychomycoses.

UNLABELLED: Onychomycoses are fungal infections of the hand and foot nails, caused by dermatophytes, yeasts and molds. These conditions are difficult to treat, spontaneous remissions are rare and recurrences are not infrequent. AIM: The present study aimed at establishing the efficacy of pulse fluconazole therapy (200 mg once weekly for 4 months) in cases of hand nails infections and 200 mg once weekly for 6 months in cases of feet nail infections. MATERIAL AND METHODS: The study comprised 36 patients--23 with hand onychomycosis and 13 with feet onychomycosis (patients' age ranging from 22 t

Dermatomycoses caused by Nattrassia mangiferae in São Paulo, Brazil.

Nattrassia mangiferae formerly known as Hendersonula toruloidea, is a phaeoid coelomycete described by Nattrass in 1933. We report five cases of N. mangiferae infections in São Paulo, Brazil. This fungus was isolated from interdigital lesions on the feet in one patient, toenails in three cases and fingernails in the other one. The infections were initially considered to be caused by a dermatophyte. Although there are only a few cases described in the medical literature, the five cases reported suggest that N. mangiferae should be taken into consideration in the differential diagnosis of tinea

The frequency of Candida species in onychomycosis.

Mycological investigation of 108 nail specimens taken from a total of 41 patients examined over three years included direct microscopy and repeated cultures. A higher incidence of onychomycosis of the fingernails (75%) was observed in women while afflictions of the toenails (71%) prevailed in men. The highest prevalence of onychomycosis was found in patients between 50 and 70 years of age. Candida albicans was the dominant organism causing onychomycosis (prevalence rate 60.9%), followed by C. parapsilosis (19.6%), C. tropicalis (9.8), C. krusei (4.9), C. guilliermondii and C. zeylanoides (2.4%

Milialike idiopathic calcinosis cutis and syringoma in Down's syndrome.

A 6-year-old girl with Down's syndrome presented milialike whitish small papules on her hands and feet and periorbital syringoma. Histopathological examination of the hand lesion revealed small localized calcium deposits and syringoma in the adjacent upper dermis. This is a very rare but typical case of calcinosis cutis with syringoma in a patient with Down's syndrome.

Patch testing with the "sesquiterpene lactone mix": a marker for contact allergy to Compositae and other sesquiterpene-lactone-containing plants. A multicentre study of the EECDRG.

6278 patients were patch tested with a sesquiterpene lactone mix (SL-mix) in 10 European clinics. 4011 patients were tested only with 0.1% SL-mix, 63 (approximately 1.5%) of whom were positive, with 26 (41%) of these cases being considered clinically relevant. There were no cases of active sensitization, though 5 cases of irritation were reported. 22 irritant reactions and 22 cases of active sensitization occurred when testing also with 1% and 0.33% concentrations of SL-mix. SL-mix 0.1% pet. is shown to be an important patch test and many relevant sensitizations will be missed without routine

Are hyperlinear palms and dry skin signs of a concomitant autosomal ichthyosis vulgaris in atopic dermatitis?

In 30% to 40% of cases atopic dermatitis (AD) is believed to be associated with autosomal dominant ichthyosis vulgaris (ADI). The diagnosis of ADI can be proved by the ultrastructural demonstration of a defective keratohyalin (KH) synthesis, resulting in minute granules of crumbly appearance in only one layer of granular cells. To investigate the suggested frequent association of ADI with AD, ultrastructural examination of dry skin of 49 AD patients was performed. Only in 2 patients abnormal KH was demonstrated by electron microscopy. 17 patients, including the 2 patients with abnormal KH, sho

Pustulotic arthroosteitis.

We describe the case of a white male with chronic inflammation of one knee, the sternoclavicular area and the dorsal spine, who also had pustulosis palmaris et plantaris. Radiological changes were suggestive of pustulotic arthroosteitis. Our patient had prominent peripheral joint changes not previously emphasized.

Clinical features of superficial fungal infections caused by Hendersonula toruloidea and Scytalidium hyalinum.

The clinical appearances of infections in 128 patients caused by the mould fungi Hendersonula toruloidea (102) and Scytalidium hyalinum (37) are described. All the patients originated from the tropics or subtropics but those infected with S. hyalinum came from either the Caribbean or West Africa. Forty-one per cent of those studied had a concurrent dermatophyte infection. While the clinical features of Hendersonula and Scytalidium infections resembled those seen in 'dry type' Trichophyton rubrum infections, there were a number of distinctive features. These included the absence of dorsal infec

Pustulosis palmaris et plantaris. A review of clinical features and aggravating factors.

A review of the clinical and histological features of pustulosis palmaris et plantaris (PPP) was undertaken. The morphologic development of PPP is biphasic: the first phase consists of an eczematous reaction, and the second phase is the pustulation of eczematous vesicles. PPP may be aggravated by infections or non-infectious inflammatory processes elsewhere in the body. An abnormal glucose tolerance test was found in 55 (66.3%) of 83 patients with PPP. The deranged carbohydrate metabolism may be partly responsible for the increased susceptibility to focal infections in patients with PPP.
