Non-invasive evaluation of pulmonary capillary wedge pressure using the left atrial expansion index in mitral valve stenosis, prosthesis and repair.

Pulmonary capillary wedge pressure (PCWP) non-invasive evaluation is limited in patients with mitral valve (MV) stenosis, prosthesis, and surgical repair. This study aimed to assess the left atrial expansion index (LAEI) measured through transthoracic echocardiography (TTE) as a novel parameter for estimating PCWP in these challenging cardiac conditions. We performed a retrospective, cross-sectional study, including chronic cardiac patients receiving within 24 h a clinically indicated right heart catheterization (RHC) and transthoracic echocardiographic (TTE) exam. PCWP measured during RHC was

Concomitant use of Sapien 3 transcatheter valve for severe MAC with Intuity Elite rapid deployment valve for aortic stenosis.

BACKGROUND: Concomitant double valve pathology in the presence of severe MAC poses significant technical challenges when planning surgical intervention. With continued evolution of valve prosthesis, innovative techniques can be considered with the potential for additional therapeutic benefit. CASE PRESENTATION: We present a novel technique of using a rapid deployment surgical aortic valve in combination with open surgical transcatheter mitral valve implantation (TMVI) for severe Mitral Annular Calcification (MAC). The Intuity Elite rapid deployment prosthesis (Edwards Lifesciences, Irvine, CA

Predictors of moderate-to-severe paravalvular aortic regurgitation immediately after CoreValve implantation and the impact of postdilatation.

OBJECTIVE: To investigate the predictors of moderate-to-severe aortic regurgitation (AR≥2+) after CoreValve implantation and evaluate the feasibility and safety of postdilatation in reducing the degree of AR. BACKGROUND: Although transcatheter aortic valve implantation is an alternative treatment for high surgical risk patients with severe aortic stenosis, post-implantation paravalvular AR remains a complication. METHODS: From July 2008 to July 2010, we enrolled 79 consecutive patients with severe aortic stenosis who underwent CoreValve implantation. RESULTS: On univariable analysis, the pr

Invasive and Echocardiographic Mean Transvalvular Pressure Gradients of Different Transcatheter Aortic Valve Prostheses.

Background/Objectives: This study aimed to assess the effectiveness and clinical relevance of intraprocedural invasive measurements-specifically intraprocedural mean pressure gradients (IC MPGs) and diastolic delta (DD)-in comparison with echocardiography for evaluating transcatheter heart valve (THV) performance across different prosthesis types. Particular attention was paid to comparing outcomes between balloon-expandable (BE) and self-expandable (SE) valves, with further stratification by aortic annulus size. Methods: A retrospective analysis was performed on 926 patients who underwent tra

[Impact of preoperative 64-row multislice computed tomography for congenital aortic stenosis; report of a case].

A 63-year-old woman was diagnosed as having severe aortic stenosis (AS) with 98 mmHg peak pressure gradient detected by echocardiography. Since, preoperative enhanced 64-row multislice computed tomography (MSCT) showed bicuspid aortic valve with only 2 sinuses of Valsalva, congenital aortic stenosis was suspected. The left and right coronary arteries originated from respective sinus of Valsalva, and severely thickened cusps of aortic valve were detected clearly by preoperative 64-row MSCT. Aortic valve replacement with a 21 mm ATS mechanical bileaflet prosthesis was performed without aortic an

Alpha-gal xenoantigens in bioprosthetic valve recipients: clinical implications for bioprosthesis longevity.

BACKGROUND: Structural valve degeneration (SVD) is a key limitation of bioprosthetic heart valves (BHVs). The underlying mechanisms for this degeneration and pathophysiology remains only partially defined. Emerging evidence implicates a xenogeneic carbohydrate epitope, galactose-α-1,3-galactose (Alpha-gal), as a potential driver of immune-mediated valve deterioration. This review explores the current knowledge on alpha-gal (AG) sensitization and evidence linking it to SVD and the potential clinical implications. METHODS: A literature search was conducted using Embase, PubMed and Scopus, using

A Valved Expanded Polytetrafluoroethylene Conduit can Acquire Endothelium After Implantation.

We demonstrate histopathology, neointimal proliferation, and neo-endothelialization in an explanted valved expanded polytetrafluoroethylene (ePTFE) conduit 40 months postimplantation that was void of calcification and inflammation, confirmed by CD-31 positivity on immunohistochemistry. Grossly, there was no distortion with preserved leaflets and lack of calcification. Good biocompatibility, nonreactivity, and low antigenicity, combined with neointimal and endothelial layer generation within the conduit might explain the low infection rates and minimal thrombogenicity. These findings support th

Is a bioprosthesis preferable in tricuspid valve replacement?

We reviewed clinical data on 29 patients (17 acquired disease and 12 congenital disease) who underwent tricuspid valve replacement using a Carpentier-Edwards porcine valve or a St. Jude Medical valve at Niigata University Hospital between January 1978 and October 1995, and analyzed risks related to the outcome. Age ranged from 8 to 62 years, and 17 females were included. New York Heart Association functional class was IV in 9 patients (31%). Previous valve surgery had been performed in 10 patients (34%). An urgent surgery was required in 4 patients and simultaneous mitral (with aortic) valve r

Successful transcatheter aortic valve implantation for pure aortic regurgitation using a new second generation self-expanding J-Valve(TM) system - the first in-man implantation.

Transcatheter aortic valve implantation (TAVI) has been recognised as an effective treatment option for high-risk or inoperable patients with aortic stenosis. However, experience with TAVI for non-calcified aortic regurgitation is still limited. The new J-Valve(TM) prosthesis is designed for antegrade transapical implantation which is characterised by three U-shape graspers that could facilitate intuitive 'self-positioning' valve implantation and provide axial as well as radial fixation by embracing the native valve leaflets. Here we report the first in-man TAVI procedure of J-Valve(TM) prosth

Early inspiris resilia valve failure in a patient with idiopathic pulmonary valve regurgitation.

BACKGROUND: Pulmonary valve failure requiring replacement (PVR) is more commonly seen in children and young adults with congenital heart disease (CHD). Adults with CHD and pulmonary regurgitation have traditionally undergone PVR with bioprosthetic valves. The inspiris resilia bovine pericardial valve is an FDA-approved bioprosthesis for the aortic position with encouraging data on 7-year outcomes. Previous reports on PVR using the Inspiris valve in young patients with CHD have demonstrated early failure of the valve. We report the early failure of this device in an elder patient with idiopathi

Percutaneous transseptal transcatheter mitral valve-in-valve implantation under endovascular cerebral protection.

Various interventional and minimally invasive surgical approaches are currently available for the treatment of mitral valve pathologies. However, only a few of these options are applicable in patients with previously operated on mitral valves. In this case report, we provide detailed insight into the step-by-step guidance of a percutaneous transseptal transcatheter mitral valve-in-valve implant under cerebral protection in a patient with a deteriorated surgically implanted mitral bioprosthesis.

Transcatheter Mitral Valve Replacement for Degenerated Bioprosthetic Valves and Failed Annuloplasty Rings.

The risk of recurrent mitral regurgitation after surgical mitral valve repair for ischemic functional mitral regurgitation is 28 % at 10 years. Also, an increasing number of patients with degenerated mitral bioprostheses are seen in daily clinical practice due to a dramatic shift from mechanical to biological bioprostheses over the past few decades. Therefore, it can be anticipated that there will be growing need for therapy options to treat high-risk patients in case of recurrent mitral regurgitation subsequent to surgical mitral valve repair or replacement. Interventional therapy for failing

Simplified Surgical Conversion of Mechanical to Bioprosthetic Bentall With Leaflet Fracture Technique.

We report a case of simplified surgical conversion from a prior mechanical composite valved conduit to a bioprosthetic aortic valve using a leaflet fracture technique. A 69-year-old man presented with progressive heart failure 7 years after aortic root replacement for bicuspid aortic stenosis and root aneurysm. Imaging revealed severe prosthetic valve stenosis and suspected pannus formation. Given significant comorbidities, a simplified approach was chosen to avoid full root re-replacement. Following redo sternotomy and graft incision, the mechanical valve leaflets were fractured and removed.

[Heart failure in patients with valve prostheses].

Congestive heart failure in patients with prosthetic valves is a complex syndrome which poses difficult clinical and therapeutical problems. In order to identify etiologic factors, pathophysiologic substrates, clinical pictures and natural history we retrospectively evaluated 124 consecutive patients (mean age 61 +/- 11 years) with prosthetic valves, hospitalized during the 1984-1990 period because of congestive heart failure. The following main etiologies were identified: acute prosthetic valve failure (19%), chronic prosthetic failure (15%), preexisting left ventricular dysfunction (9%), new

3-Dimensional personalized planning for transcatheter pulmonary valve implantation in a dysfunctional right ventricular outflow tract.

BACKGROUND: Identification of adequate landing zone for transcatheter pulmonary valve implantation (TPVI) is crucial to successfully treat an aneurysmatic native right ventricle outflow tract (RVOT); three-dimensional (3D) patient-tailored digital and physical printed models are available but their actual strengths and weaknesses still not well documented. The aim of the study was to tackle TPVI planning in the dysfunctional and borderline RVOT exploiting both digital and physical printed 3D patient-specific models. METHODS: Electrocardiographically gated computed tomography (CT) angiography

Impact of prosthesis-patient mismatch on long-term survival in patients with small St Jude Medical mechanical prostheses in the aortic position.

BACKGROUND: The impact of aortic prosthesis-patient mismatch (P-PtM) on long-term survival is unclear. METHODS AND RESULTS: Between 1985 and 2000, 388 patients at Mayo Clinic in Rochester, Minn, underwent aortic valve replacement (AVR) with 19- or 21-mm St Jude Medical prostheses and had transthoracic echocardiography within 1 year after AVR. Mean age of patients was 62+/-13 years; 69% were female. Prosthesis effective orifice area (EOA) was derived from the continuity equation. P-PtM was classified as severe (indexed EOA < or =0.60 cm2/m2), moderate (0.60 cm2/m2<indexed EOA< or =0.85 cm2/m2)
