Helicobacter pylori: from the benign to the malignant.

Half of the world's population is infected with Helicobacter pylori, making it a pathogen of potentially great significance. Although in the majority of cases, infection is harmless, many infected individuals develop peptic ulcer disease or gastric cancer. In developed countries, the prevalence of infection has decreased along with the incidences of duodenal and gastric ulcer and distal gastric cancer. In developing countries, H. pylori infection rates remain high, and, as life expectancy increases, the incidence of gastric cancer will increase. Recently, a working party of the World Health Or…

Barrett's esophagus and the presence of Helicobacter pylori.

OBJECTIVE: Although the role of Helicobacter pylori in the pathogenesis of peptic ulcer disease and antral gastritis has been well documented, the role of H. pylori in esophageal disease has not been clearly defined. To clarify this issue, we analyzed 141 patients with histologically confirmed esophageal disease. METHODS: The study group consisted of 82 patients with Barrett's esophagus, 19 with adenocarcinoma of the esophagus arising in columnar epithelium and 40 patients with reflux esophagitis without columnar metaplasia of the esophagus. In each of these cases the presence or absence of H…

Helicobacter infection in man: problems to be solved.

Helicobacter pylori (Hp) infection in man has several disease outcomes, varying from asymptomatic chronic gastric inflammation to Hp-associated dyspepsia, pepic ulcer disease, gastric adenocarcinoma, and Malt lymphoma. Particularly controversial is the role of Hp infection in the genesis of chronic dyspeptic symptoms. Only a small percentage of chronic dispeptics have long-lasting remission of the complaints after cure of the infection. It is now well established that healing of the inflammation through microbial eradication cures peptic ulcer disease. The high efficacy of bismuth or PPI tripl…

[The significance of Helicobacter pylori in medical science].

The discovery of Helicobacter pylori as a dominant cause of gastritis is at least revolutionary both with respect to its clinical consequences as to its contribution to fundamental basic science. The velocity with which basic and clinical experience was accumulated in the H. pylori field has been unsurpassed in medicine. The organism is highly complex and can be divided in two subclasses, a more virulent strain containing the socalled 'pathogenicity island' in addition to vacA and IceA1 antigen and an almost commensal-like subtype. The virulent type in particular is associated with severe infl…

Helicobacter pylori and the liver: any relationship?

Helicobacter pylori infection is being correlated to a number of human diseases, among which also those of the liver. From a clinical point of view, 4 "areas of interest" for the suggested correlation can be identified: 1. Helicobacter pylori and portal hypertension-related congestive gastropathy in cirrhotics. There are, in the literature, at least 7 studies confirming that the microorganism has no role in causing or worsening the disease. 2. Helicobacter pylori and duodenal ulcer in cirrhotic patients. Apparently, in the cirrhotic patient, the microorganism has no role in causing duodenal ul…

Halitosis and Helicobacter pylori: a possible relationship.

With the aim of investigating a possible relationship between "objective" halitosis (established by sulfide levels in the breath) and Helicobacter pylori, we performed a study in 58 dyspeptic patients reported to suffer from "bad breath." Furthermore, we evaluated the effects on halitosis of eradication therapy (only for H. pylori-positive patients) and chlorhexidine antiseptic mouth rinses (in all patients). Sulfide compound assay indicated objective halitosis in 52/58 patients, 30 of whom were positive and 22 negative for H. pylori. In 19/30 eradication by double therapy provoked a decrease …

Endoscopic methods for the diagnosis of Helicobacter pylori.

This article reviews endoscopic methods for the diagnosis of Helicobacter pylori. At the time of endoscopy certain findings (gastroduodenal ulceration and/or antral nodularity) may be highly suggestive of H. pylori infection. Endoscopic acquisition of gastric biopsies, however, leads to a definitive diagnosis of infection on the basis of both direct and indirect tests. Direct tests include culture and histological detection (considered the gold standard). There are a variety of stains available for the detection of H. pylori; their choice is influenced by local expertise and the clinical situa…

[Eradication of Helicobacter pylori in developing countries].

A NIH Consensus Conference recommended Helicobacter pylori eradication to all ulcer patients, based mainly on information coming from countries with a low prevalence of infection in general population. The epidemiological situation is different in developing countries, where a pandemic of H. pylori goes unchecked, and most people become infected at young age. It is possible that response to eradication therapies and reinfection rate were to be included among the differences between developed and developing countries, raising doubts about the worldwide applicability of NIH recommendations. Limi…

Symptomatology of Helicobacter pylori infection in children.

In a retrospective evaluation we reviewed the symptomatology of 143 children (age 2-15 years, mean 8.9 years) who were referred to us for upper gastrointestinal endoscopy because of recurrent abdominal pain with a duration of 6 weeks or longer. Helicobacter pylori infection was diagnosed in 36 out of 143 patients (25.2%). No statistically significant differences could be detected between the symptoms experienced by the 36 H. pylori-infected children and those experienced by the remaining 107 H. pylori-negative pediatric patients (p = 0.18-0.60). We conclude that no specific symptoms are associ…

Gastrin and acid in relation to Helicobacter pylori.

H. pylori infection, both in normal healthy subjects and patients with duodenal ulcer, results in modest elevations of serum gastrin concentrations in the fasting state and quite substantial elevations after a meal or gastrin releasing peptide (GRP) stimulation. Cure of the infection leads to normalization of gastrin homeostasis. Acid secretion in response to a submaximal infusion of GRP is three-fold higher in H. pylori-infected normal subjects and six-fold higher in DU patients than in non-infected controls. These changes also normalize after cure of the infection. H. pylori infection appear

Efficacy of ciprofloxacin in the eradication of Helicobacter pylori.

In small preliminary trials, ciprofloxacin has failed to eradicate Helicobacter pylori. Since fluoroquinolones have a marked reduction in bactericidal activity at acidic pH, we altered the gastric pH using omeprazole and investigated the efficacy of ciprofloxacin in eradicating H pylori. Forty-four consecutive patients infected with H pylori were prospectively studied in a randomized, double-blind, controlled trial comparing ciprofloxacin with a placebo for 2 weeks. Both treatment groups received bismuth and omeprazole. In 36 patients, follow-up endoscopy was done 4 weeks after the cessation o…

[Virulence and pathogenesis of Helicobacter pylori infection].

Recent studies on the relationship between Helicobacter pylori and gastritis or gastroduodenal ulcers have indicated that this organism can induce chronic active gastritis with neutrophil infiltration. However the mechanisms by which bacteria colonized and damage the host are often complex and multiple. The ammonia produced by H. pylori, which has strong urease activity, has been reported to have a cytotoxic effect on the gastric mucosa. With H. pylori the major areas of interest regarding pathogenesis are also related to other factors such as oxygen radicals, cytotoxins and cytokines. There m…

Helicobacter pylori as a risk factor for cancer.

In 1985, gastric cancer was the second most common cause of cancer death in the world. The rapid decline in gastric cancer rates over the last few decades has been attributed to a decline in the prevalence of environmental risk factors for gastric cancer and/or an increase in the prevalence of protective factors. One such risk factor could be the bacterium Helicobacter pylori. Epidemiological studies have shown that areas with high gastric cancer rates often have a correspondingly high prevalence of H. pylori and prospective studies have shown that subjects with serological evidence of H. pylo…

Efficacy of lansoprazole in eradication of Helicobacter pylori.

Fifty-eight Helicobacter pylori-positive ulcer patients received omeprazole 20 mg (n = 15), or lansoprazole 30 mg (n = 23), lansoprazole 60 mg (n = 13), or E3810 20 mg (n = 7) q.d. Another 63 H. pylori-positive ulcer patients received lansoprazole and clarithromycin for 2 weeks. Patients received lansoprazole 30 mg and clarithromycin 400 mg (group 1, n = 22), lansoprazole 30 mg and clarithromycin 800 mg (group 2, n = 12), or lansoprazole 60 mg and clarithromycin 800 mg (group 3, n = 29). Duodenal ulcer and infection with Helicobacter pylori.

The incidence of gastritis with Helicobacter pylori (HP) was studied in 225 patients with active duodenal ulcer diagnosed endoscopically. The infection was diagnosed by the urease test and in some cases by histopathological examination. Infection with Helicobacter pylori was detected in 55% of the patients, a proportion much smaller than the one generally reported in the literature, a rather surprising fact for our sanitary conditions. This might mean that in our country unlike the west-European countries other pathogenetic factors could be more important than HP.

[Status of ranitidine in eradication of Helicobacter pylori].

Permanent removal of Helicobacter pylori leads to a dramatic drop in the recurrence rate of peptic ulcers. The NIH Consensus Conference recommends that Hp-positive patients with duodenal or gastric ulcer should undergo eradication treatment. To date, however, there is no consensus on what constitutes optimal eradication treatment. Presently available therapy may be triple (bismuth plus two antibiotics), dual (acid inhibition and one antibiotic) or modified triple therapy (acid inhibition and two antibiotics). Among numerous treatment proposals, results gained with the H2 blocker ranitidine sta…
