Displaced four-part fracture dislocation represents the highest risk of osteonecrosis of the humeral head due to a disruption of the blood supply. In a few cases, that dislocation can be intrathoracic which worsens prognosis with life-threatening injuries such as hemothorax or pneumothorax. This paper presents the case of a 77-year-old woman after having fallen down the stairs with an intrathoracic dislocation four-part fracture leaving the head in the thorax. We analyze mechanisms and review the literature in order to highlight this lesion and provide a take home message for the treatment.

Major wounds of the chest usually cause immediate or early death. Of 35 patients with major thoracic battle injuries who were treated in one naval hospital, six had non-penetrating wounds of the chest and other areas. Of the 29 with penetrating wounds of the chest, 23 were treated by aspiration of hemothorax, decortication in three cases, and thoracotomy for control of hemorrhage in one case and for removal of a foreign body in another. Four cases are reported, two to illustrate the usual course of treatment and two in which the patient died.

Acute subclavian artery dissection (SAD) is a rare entity which is usually associated with several vascular abnormalities and traumatic events. Spontaneous SAD remains exceptional and often affects the left artery. We report the autopsy case of a 29-year-old female who died suddenly following a spontaneous dissection of the right subclavian artery.

Vasculopathy is a relatively frequent but poorly recognised manifestation of von Recklinghausen's neurofibromatosis. One of its more dramatic presentations is as spontaneous haemothorax.
