The natural history of hooked forefoot.

This paper reports on a prospective study undertaken to determine the natural history of untreated idiopathic hooked forefoot. The progress of 130 affected feet in eighty-three children was followed for an average of seven years. At review 86% of the feet were normal or only mildly deformed and all were fully mobile; 10% were still moderately deformed but were asymptomatic; 4% remained deformed and stiff. It was not possible to detect these resistant cases before the age of three years, but the low failure rate would seem to justify a policy of expectant treatment.

Natural history of the leg amputee.

For 173 patients undergoing major leg amputations, the operative mortality was 13 per cent. The ratio of below-knee (BK) to above-knee (AK) amputations was approximately unity. Of the 150 patients who survived amputation, 93 were given prostheses. Amoung the latter group, 76 per cent of the unilateral AK amputees and 90 per cent of the unilateral BK amputees had a successful rehabilitation. For those patients who had to be converted from BK to AK unilateral amputations, 40 per cent experienced successful rehabilitation, and for those who had either bilateral BK or bilateral mixed amputations,

The Goldenhar syndrome: a natural history.

Twenty-four patients with the 1st branchial arch (Goldenhar) syndrome have been ascertained ranging in age from newborn to 58 years. In infancy, the ocular, auricular, and palatal problems appear most prominent. In childhood, the correction of, or compensation for, hearing deficit assumes greater importance. Incoordination of deglutition, achalasia of the esophagus, hiatus hernia, and mobile cecum (hitherto unreported features) are characteristically troublesome in infancy and early childhood. Cosmetic problems though significant throughout, engender particular concern during adolescence and e

The natural history of truncus arteriosus.

The cases of 23 patients whose condition was diagnosed as truncus arteriosus, type I or II, and who were seen at the Mayo Clinic during the decade preceding 1967, that is, before corrective operation became feasible, were reviewed. Ten were infants (through one year of age), and all ten have died. Eight ranged in age from more than one year through seven years of age, and all are living, except one, who diet 11 years after diagnosis. Five were older than seven years, and all had severe pulmonary vascular obstructive disease; three have died. Thus, 14 of the 23 have died, and all but one surviv

The natural history of prostatic cancer.

There are inner and outer gland groups in the human prostate. Benign nodular hyperplasia develops from the inner group of glands. Cancer develops from the outer gland group and consequently spreads beyond the prostate at an early stage in the disease. The biological malignancy of prostatic cancer varies from patient to patient and from part to part of the same tumor. Some tumors remain biologically inactive or latent so that there must be some naturally-occurring factor which controls tumor growth in these cases. Prostatic cancer patients may be divided into groups which differ in their respon

Observations on the natural history of atherogenesis.

Longstanding scoliosis of the lumbar spine with convexity to the left leads to severe atherosclerosis in the adjacent segment of the abdominal aorta. By contrast, the lateral wall of the ascending aorta, which is free to pulsate, usually is not affected even in very old persons. This relative immunity, however, is not maintained in cases of syphilitic mesaortitis; damage to the elastic tissues of the media leads to atherosclerosis. Man is the only mammal known to be highly susceptible to severe atherosclerosis. The concept is advanced that the erect posture of man, which leads to the physiolog

On the natural history of hemophilic pseudotumor.

Seven cases of hemophilic pseudotumor were followed for at least ten years. Apparently adequate and early substitution therapy for bleeding in muscles and skeleton will prevent the occurrence of such pseudotumors. Conservative treatment of established pseudotumors with substitution therapy at bleeding symptoms is advocated, but in the event of continued growth of the pseudotumor and threatened perforation, radical extirpation may have to be done.
