[A woman with a painful elbow].

A 24-year-old woman with Ehlers-Danlos syndrome visited the emergency department because of a post-traumatic painful and swollen left elbow with impaired function. A plain radiograph showed chronic luxation of the radial head with impression in the humeral capitellum. The clinical presentation of chronic luxation of the radial head is easily confused with that of an acute dislocation of the radial head, which may wrongly result in subsequent reduction.

Imaging-based assessment of fracture stability does not reliably predict outcomes in patients with two-part proximal humeral fractures and may lead to unnecessary surgeries.

Two-part surgical neck fractures are the most common displaced proximal humeral fractures in the elderly. Most fractures can be categorized into varus-impacted or medially translated fracture patterns. The natural healing process often involves secondary displacement and partial resorption of the humeral head, but these changes are poorly correlated with shoulder function and patient satisfaction. Randomized trials have been unable to identify any benefits from surgery but report a high proportion of implant-related complications. In large prospective cohort studies, patients aged 60 or older …

The results of ORIF of displaced unstable proximal humeral fractures using a locking plate.

BACKGROUND/HYPOTHESIS: Surgical management of displaced unstable proximal humerus fracture remains a challenge due to poor proximal bone qua … es healed. Anatomic alignment was obtained in 72%. Two patients developed avascular necrosis of the humeral head. There were no cases of hardware failure, infection, or loss of reduction. Three separate reduction maneuvers were employed in this series depending on fracture type. CONCLUSION: The locking plate is an excellent device in the management of displac …

Suture Augmentation Neutralizes Deforming Muscular Forces in a Simulated 2-Part Osteoporotic Proximal Humeral Fracture Model.

OBJECTIVES: To evaluate the contribution that tension-relieving sutures, placed between a proximal humeral locking plate and the rotator cuf … us collapse. RESULTS: Suture augmentation to the SS, SB, and infraspinatus significantly decreased humeral head varus collapse when compared with the plate alone at nearly all loads ( P < 0.05). There were no significant differences in humeral head varus collapse between the 3 suture constructs. CONCLUSIONS: In our biomechanical evaluation of a simulated ost …

Intramedullary Fixation for Proximal Humeral Fractures.

Proximal humeral fractures are a commonly encountered injury; however, no consensus has been reached for the ideal treatment. Current surgic … ility to insert the implant from a superior starting point may help preserve vascular supply to the humeral head and tuberosities. With reported outcomes comparable with the aforementioned techniques and an evolving understanding of fracture characteristics and failures of fixation, intramedullary fixation represents an alternative treatment option for proxima …

Proximal periarticular locking plates in proximal humeral fractures: functional outcomes.

BACKGROUND: Some recent studies have asserted that locking plates do not provide adequate fixation of proximal humeral fractures. The purpose of this study is to review our experience with proximal humeral locking plates, including complications, functional outcomes, and predictors of successful treatment. MATERIALS AND METHODS: At our institution, 45 patients (46 shoulders) with displaced proximal humeral fractures were treated with a proximal humeral locking plate over a 6-year period. Patients underwent standard surgical and rehabilitation protocols and were evaluated clinically with Disab

Is there a correlation between humeral osteoarthritis and glenoid morphology according to Walch?

AIMS: The aim of this study was to determine whether there is a correlation between the grade of humeral osteoarthritis (OA) and the severity of glenoid morphology according to Walch. We hypothesized that there would be a correlation. METHODS: Overal, 143 shoulders in 135 patients (73 females, 62 males) undergoing shoulder arthroplasty surgery for primary glenohumeral OA were included consecutively. Mean age was 69.3 years (47 to 85). Humeral head (HH), osteophyte length (OL), and morphology (transverse decentering of the apex, transverse, or coronal asphericity) on radiographs were correlate…

Shoulder joint stability after arthroscopic subacromial decompression.

In 55 patients with type I or type II impingement lesions we performed arthroscopic subacromial decompression. Fifty-two patients are followed up 1 year postoperatively. In all patients the condition of the affected shoulder before and after decompression was documented using a 100-point shoulder score (pain on activity, 15 points; pain without activity, 15 points; function, 20 points; weight lifting, 10 points; muscle strength, 15 points; range of motion, 25 points). At follow-up we also documented the extent of passive inferior shift of the humeral head by ultrasound. The mean score preopera…

Chronic unreduced dislocations of the shoulder.

We evaluated the results of treatment in twenty-three patients with twenty-four shoulder dislocations that had gone unreduced for at least three weeks. Fourteen dislocations were posterior, eight were anterior, and one each was superior and inferior. Seventy-nine per cent of the posterior dislocations had not been recognized by the initial treating physician. Fourteen shoulders (58 per cent) were operated on. Of seven that were treated by open reduction with preservation of the humeral head, the results in two were graded as excellent; in three, as good; and in two, as fair. A Neer total shoul…

Morphometric Study of Bicipital Groove from Dry Adult Humerus.

BACKGROUND: Bicipital groove, the narrow sulcus present between the greater and lesser tubercles of proximal humerus, is an important anatomic landmark during prosthetic replacement of humeral head. Bicipital groove variations are associated with diseases of biceps tendon causing painful shoulder. The study aims to outline the morphometry of bicipital groove and create some baseline data which may be useful in arthroplasties and interpretation of radiologic anatomy of the shoulder. METHODS: A quantitative cross-sectional observational study was conducted on 100 dry humerus bones from convenie…

The Plate-Joystick technique to reduce proximal humeral fractures and nonunions with a varus deformity through the extended deltoid-splitting approach.

Fractures and nonunions in which there is a varus deformity of the humeral head producing posterinferior subluxation of the articular surface are increasingly recognized as an important subgroup of proximal humeral fractures. Operative open reduction and internal fixation of these injuries is often recommended when the varus deformity is severe. We describe a simple technique to assist in the open reduction and locking plate stabilization of this challenging and complex fracture subtype using tools and implants that are readily available in most modern orthopaedic trauma operating rooms.

Posterior fracture-dislocation of the shoulder. A superior subacromial approach for open reduction.

We describe 11 cases of posterior fracture-dislocation of the shoulder that required open reduction and fixation. Difficulties with access through anterior approaches led us to use the superior subacromial approach. This is an extension of the approach often used to expose the rotator cuff; the joint is opened by splitting the supraspinatus tendon 5 mm behind the cuff interval. The glenoid, proximal humerus and any fracture fragments can be seen from above, allowing reduction of the dislocation and osteosynthesis to be performed with minimal risk of damage to the humeral head and its blood sup…

[Osteosynthesis using the "palm-tree" nail technic in fractures of the surgical neck of the humerus].

Presented in 1974, this technique consists of internal fixation of the humeral neck by means of three divergent K-wires. These are introduce … uced into the medullary canal. After reduction of the fracture, they are driven into the cancellous humeral head bone. By turning the handle, the pins are made to diverge. Exceptionally, operative reduction is necessary by a separate axillary approach. X-ray control or the use of an image intensifier permits one to see if a K-wire is too long and to pull it ou …
