Outcomes of antenatal hydronephrosis and other renal abnormalities in a population-based cohort.

INTRODUCTION: Hydronephrosis, commonly detected during second trimester ultrasound screenings, can signal underlying renal abnormalities. In 2018, Sweden introduced a national guideline recommending follow-up for fetal renal pelves with an anteroposterior diameter (APD) ≥6 mm at the second trimester ultrasound. This study evaluates the impact of the guideline by assessing the incidence and resolution of hydronephrosis and other renal abnormalities. Secondary outcomes included postnatal renal function, frequency of febrile urinary tract infections (UTI), and the need for surgical intervention b…

Fetal Hydronephrosis: A Narrative Review of Diagnostic Pathways, Prognostic Markers, and Management.

Fetal hydronephrosis is the most frequently detected prenatal urinary tract abnormality and encompasses a broad clinical spectrum, ranging from transient physiological dilation to clinically significant obstructive uropathy with potential long-term renal consequences. The broader term urinary tract dilation integrates renal pelvic and calyceal dilation with abnormalities of the renal parenchyma, ureters, bladder, and amniotic fluid volume, thereby supporting more comprehensive risk stratification. This narrative review synthesizes evidence on fetal urinary tract development, prenatal diagnosis…

Retroperitoneal fibrosis and bilateral hydronephrosis secondary to gastric cancer: A case report.

RATIONALE: Retroperitoneal fibrosis (RPF) is a rare fibroinflammatory condition that can lead to ureteral obstruction and renal dysfunction. While most cases are idiopathic, secondary causes including malignancies should be considered. RPF secondary to gastric cancer is extremely rare, with only a few cases reported. PATIENT CONCERNS: A 56-year-old woman presented with generalized edema, decreased urine output, and nausea that had begun 7 days prior. Laboratory tests revealed acute kidney injury, and imaging showed bilateral hydronephrosis. DIAGNOSES: Further evaluation through abdominal-pel…

An unusual case of a peritoneal dialysis catheter causing hydronephrosis: A case report.

Peritoneal dialysis (PD) catheter migration is a common complication of PD which usually results in obstruction of dialysate outflow. We report the first known case in the literature of a 62-year-old male with end-stage renal disease on PD who presented with acute renal colic secondary to the PD catheter overlying right mid-ureter causing hydronephrosis with spontaneous resolution of pain and hydronephrosis two days later. The patient was discharged home with a functioning dialysis catheter and complete resolution of both symptoms and radiographic findings of hydronephrosis. While management o…

Utility of F-15 diuretic MAG3 renography in assessment of paediatric hydronephrosis.

INTRODUCTION: Diuretic renography is crucial in evaluation of paediatric hydronephrosis. Furosemide is conventionally given 15-20 min after radiolabelled tracer (F+15/F+20 protocol), however this is equivocal in around 15% of patients. Giving furosemide 15 min prior to tracer (F-15 MAG3 protocol) has been suggested as an additional tool in the investigation of patients with suspected upper urinary tract obstruction. However, the role of this method in assessment and management of paediatric hydronephrosis is not widely reported. OBJECTIVE: To investigate utility of F-15 renograms in children …

Retrocaval ureter presenting with hydronephrosis: A case report of rare entity in Nepal.

Hydronephrosis, characterized by the dilation of the renal pelvis and calyces, often results from obstructive uropathy, commonly due to ureterolithiasis. However, rare congenital anomalies, such as a retrocaval ureter (RCU)-where the ureter passes behind the inferior vena cava- can also cause hydronephrosis. This case report describes an 11-year-old male presenting with right-sided flank pain and hematuria. Initial ultrasonography and noncontrast CT revealed right hydronephrosis without ureteral stones. Subsequently, contrast-enhanced CT urography found an abnormality in the course of the righ…

Diagnostic Value of DCE-MRI and Tofts Model in Children with Unilateral Hydronephrosis.

BACKGROUND: Hydronephrosis is a common condition, and the correct diagnosis of hydronephrosis is necessary to improve the early diagnosis rates of pediatric hydronephrosis. OBJECTIVE: The objective of this study is to explore and analyze the diagnostic value of dynamic contrast- enhanced magnetic resonance imaging (DCE-MRI) analyzed using the Tofts model in children with unilateral hydronephrosis. METHODS: We retrospectively selected data from 88 children with unilateral hydronephrosis treated in our hospital from September 2018 to October 2020. Routine and DCE-MR renal image indexes were co…

Percutaneous Nephrostomy in the Treatment of Hydronephrosis in Renal Transplant Patients - Case Report.

Percutaneous nephrostomy is a first-line minimal invasive treatment option for ureteral obstruction following kidney transplantation, with high effectiveness and a low complication rate. Percutaneous nephrostomy might be used as a temporary salvage therapy, providing acute decompression of the kidney collecting system and preventing graft loss. It can also function as a permanent and sometimes only possible option in transplant patients with frequent recurrences of ureteral stenosis who either fail an open surgical reconstruction or who are not good candidates for these procedures. We present

Acute life-threatening hyperkalaemia in a patient with giant hydronephrosis: a case report.

A 52-year-old man with a history of urolithiasis presents to the emergency department with a sudden, sharp, continuous right flank colicky pain. Laboratory workup demonstrates acute kidney injury with a mild hyperkalaemia. During the observation period, the patient develops an atypical broad complex sinus bradycardia and eventually short asystolic periods. This was caused by a severe therapy-resistant hyperkalaemia, wherefore emergency haemodialysis was necessary. Radiographic results showed a giant hydronephrosis with a blowout of the right kidney and an obstructing calculi of 21 mm in the di…

Asymptomatic obstructive hydronephrosis associated with diabetes insipidus: a case report and review.

The treatment of asymptomatic hydronephrosis due to ureteropelvic junction obstruction (UPJO), as well as the timing and indications for surgical intervention, remain controversial. Diabetes insipidus (DI) is a rare disease in infants that is known to cause non-obstructive hydronephrosis, while its association with obstructive hydronephrosis has not been reported. Some studies have found that increased water intake is a predisposing risk factor of developing hydronephrosis. However, there are no studies or guidelines that indicate the specific follow-up intervals and surgical indications for m…

Giant neonatal hydronephrosis of the upper pole of a complete duplicated renal system.

Ureteropelvic junction obstruction (UPJO) in the upper pole of a complete duplicated renal system is extremely rare. We report a case that was diagnosed prenatally and we review 10 cases published in the literature. Diagnoses of adrenal hematoma and cystic neuroblastoma were suspected on prenatal ultrasound, based on the severity of dilatation and the difficulty of identifying the exact origin of this anomaly. Neonatal magnetic resonance imagery (MRI) was subsequently able to show a normal adrenal gland. Surgical management is similar to that of a single-system pyeloureteral junction obstructi

Recurrent hydronephrosis and spontaneous renal rupture caused by lymphoplasmacytic inflammation in a cat.

A seven-year-old male cat that was previously diagnosed with - and treated for - left hydronephrosis due to suspected idiopathic proteinaceous materials in the renal pelvis, presented with a short history of anorexia and vomiting. The abdominal ultrasound revealed bilateral hydronephrosis, and the intravenous pyelography showed a moderate amount of free fluid in the retroperitoneal space at 48 hours. After the nephrectomy, the gross examination of the right kidney revealed a very thin capsule with urine leakage, and the right renal pelvis showed small, black deposits. The histopathology of the…

Late diagnosis of imperforate hymen with hematometrocolpos and bilateral hydronephrosis of a horseshoe kidney.

Imperforate hymen is a fairly uncommon genital disorder where the hymen completely obstructs the vaginal opening. Several authors have ruled out its co-occurrence with other congenital anomalies. In this report, we discuss the exceptional case of a late diagnosis of imperforate hymen associated with bilateral hydronephrosis of a horseshoe kidney in a 19-year-old female patient. To our knowledge, an association of imperforate hymen and horseshoe kidney has never been reported.

Giant hydronephrosis secondary to ureterocele with duplex system in adults: Report of a case.

Giant hydronephrosis in adults is a rare entity. It is defined as an extensive dilatation of the pyelocaliceal cavities occupying a large part of the abdominal cavity. Giant hydronephrosis is usually due to pelvi-ureteric junction obstruction and is usually diagnosed in children and infants. Ureterocele, which is a cystic dilatation of the terminal ureter, often drains the upper part of the kidney in patients with a duplex system. Massive hydronephrosis in a patient with duplex system and obstructive ureterocele was described by Aeron et al. in 2017. A thorough search of the major medical data…

Hydronephrosis in a dairy calf: A diagnosis delayed by a clinician's Bayesian brain reasoning.

A 3-week-old female dairy calf was presented because of severe weakness and diarrhea. Physical examination and ultrasound findings lead to a tentative diagnosis of intestinal obstruction, although serum biochemistry suggested a primary renal disorder. Reassessment of ultrasound images allowed diagnosis of hydronephrosis that had been misdiagnosed probably because of the clinician's biased Bayesian reasoning. Pyelonephritis without renal failure was diagnosed 2.5 months later. The calf was euthanized and the diagnoses were confirmed.

Neonate with urinary ascites but no hydronephrosis: unusual presentation of posterior urethral valves.

Posterior urethral valves (PUV) are an important cause of paediatric obstructive uropathy. PUV are usually diagnosed by prenatal ultrasonography (US) revealing hydronephrosis and bladder distention. We describe a 17-day-old male infant with abdominal distention who had no hydronephrosis on prenatal US. Laboratory investigations showed serum creatinine of 12 mg/dL, hyperkalaemia and metabolic acidosis. Abdominal US showed large amount of ascites, normal-sized kidneys without hydronephrosis and incompletely distended bladder. Paracentesis revealed clear, yellow ascitic fluid with creatinine leve…
