Congenital nephrogenic diabetes insipidus (CNDI), also known as arginine vasopressin resistance, is a rare inherited disorder of water homeo … oted. A diagnosis of CNDI was made based on vasopressin levels, plasma and urine osmolarity values, hypernatremia, and family history. The patient's treatment was challenging; he responded favorably and remains under outpatient follow-up. The presentation, results of additional tests, and short-term course are described.

BACKGROUND: A 39-year-old male with multiple myeloma was admitted for treatment with melphalan and autologous stem cell reinfusion. He presented with hypokalemia and hyperchloremic non-anion-gap metabolic acidosis with a high urinary pH. He also had hypomagnesemia, hypophosphatemia, hypouricemia, proteinuria and glucosuria. The patient subsequently developed polyuria with a low urine osmolality, hypernatremia and, finally, acute renal failure.
INVESTIGATIONS: Physical examination, blood and urine analyses, kidney biopsy and tonicity balance.
DIAGNOSIS: Fanconi syndrome with proximal (type II…

A 5-month-old puppy was evaluated for rapidly progressive neurologic signs and pyrexia. Magnetic resonance imaging showed multifocal meningoencephalitis with transtentorial and foramen magnum herniation. A cerebrospinal fluid tap revealed highly cellular fluid, and the puppy was euthanized. Histopathology showed lymphoplasmacytic and neutrophilic meningoencephalitis. Viral polymerase chain reaction testing for Eastern equine encephalitis was positive. Rapid progression of neurologic signs and respiratory arrest necessitated mechanical ventilation. Severe hypernatremia, most consistent with cen…

Central pontine myelinolysis is a rare but devastating cause of morbidity and mortality after orthotopic liver transplant. The exact cause of central pontine myelinolysis is uncertain. However, rapid correction of hyponatremia has been described as a major factor. We describe a patient with central pontine myelinolysis after orthotopic liver transplant in the absence of significant hyponatremia. Although rapid correction of hypernatremia has been reported in association with central pontine myelinolysis, to our knowledge, in this case, where the serum sodium went from normal to hypernatremic, …

A four-month-old infant was hospitalized because of RSV bronchiolitis. Two days after presentation he developed rhabdomyolysis, most probably as a result of multiple factors (hyperthermia, hypovolemia/dehydration, hypernatremia, metabolic acidosis), followed by severe complications including acute renal failure, hepatic dysfunction and disseminated intravascular coagulation. Under sufficient fluid supply he recovered completely. In rhabdomyolysis clinical symptoms vary. Seldom, the classical trial of muscle pain, weakness, and dark urine is observed. Severe complications are hypovolemia, elect…

Salt poisoning has been described under various circumstances in adult cattle. Presenting clinical signs in 6 Holstein beef cattle with such poisoning were primarily dysfunction of the central nervous system and included ataxia, opisthotonus, nystagmus, depression, muscle twitching and intermittent convulsions, as well as abdominal pain and polydipsia. Diarrhea occurred in 2, and blindness in 3/6 cattle. Hypernatremia (161.8 - 178.8 mmol/L) and hyperosmolality (331.81 - 366.18 mOsm/L) were present in all animals. To treat the affected cattle, access to fresh water was restricted, vascular volu…

To study whether sodium bicarbonate given in cardiopulmonary resuscitation may produce life-threatening hyperosmolality or hypernatremia, arterial blood was analysed for blood gas, alcohol, blood urea nitrogen, electrolyte and osmolality. The blood was drawn after resuscitation in successful cases, and while effective massage and ventilation were being applied in unsuccessful resuscitations. Seven of the 17 resuscitations were successful. Serum sodium concentrations ranged from 135 to 154 with one exception and did not correlate with the amount of sodium bicarbonate administered. Arterial pH r…

Endothelial glycocalyx is a layer composed by glycosaminoglycans, proteoglycans and glycoproteins attached to the vascular endothelial luminal surface. It has several physiological roles: shear stress mechanotransduction to the endothelial cells, regulation of fluids and macromolecules vascular permeability, of coagulation cascade activation and fibrinolysis, and protects the endothelium from platelets and leukocytes adhesion. In general, glycocalyx protects vascular wall against pathogenic insults. The glycocalyx may be damaged by abnormal shear stress, reactive oxygen species, hypernatremia,…

The aim of our work was to study the different blood parameters as well as the activity of the vasopressinergic axis in young and mature male rats under normal conditions and following a 3-day dehydration cycle by water deprivation. Under normal conditions, our study demonstrates higher levels of vasopressin in mature rats as compared to young rats. This could be due in part to the higher blood osmolality in the mature rats. After dehydration, hypovolemia, plasmatic hyperosmolality, hypernatremia and hyperproteinemia cause a stimulation in vasopressin synthesis and release, as seen in results …

A 26-year old woman presented with acute hepato-renal dysfunction, coagulation abnormalities and diabetes insipidus associated with hypernatremia in the latter term of pregnancy (39 weeks). Such transient diabetes insipidus during pregnancy as in this case has been reported to be resistant to AVP, but to respond to DDAVP. Because of fetal compromise, an urgent cesarean section was performed. Spinal anesthesia was chosen because of the possible deleterious effects of general anesthesia on liver function. After delivery of twin babies, her symptoms recovered gradually. In conclusion, diabetes in…

Chronic dietary K+ depletion was induced in rats for up to 90 days. The treatment induced hypokalemia, moderate hypernatremia, and moderate metabolic alkalosis which was partly compensated for by a hyperproteinemic acidosis at an unchanged arterial PCO2. Whereas the K+ content of skeletal muscle was reduced to almost half its regular content, the heart muscle was much less affected and the brain K+ content was minimally reduced or not affected depending on the duration of the K+ depletion. The reduction in muscle tissue K+ content was not fully compensated for by an increase in Na+ content. Ce…

Two cases of idiopathic hypothalamic dysfunction (one boy and one girl) are reported. Symptoms of hypothalamic dysfunction were noted by the age of 2 years: initial polyphagia and obesity with subsequent anorexia and emaciation were observed in one patient. Thermoregulation and thirst disorders, recurrent accesses of hypernatremia, acrocyanosis and profuse sweating were present. Impaired growth and delayed puberty in one case, and in the other hypogonadism, absence of growth hormone and gonadotrophins release in response to provocative stimuli were observed as well as abnormal thyroid stimulat…

Osmotic demyelination syndrome (ODS) is an acute demyelinating disorder characterized by the loss of myelin in the center of the basis pons, defined as central pontine myelinolysis (CPM), and demyelination in locations outside the pons, defined as extrapontine myelinolysis (EPM). ODS including CPM and EPM is mainly caused by rapid correction of hyponatremia. However, there are several reports of ODS in medical conditions such as malnutrition; alcoholism; liver transplantation; malignancy; sepsis; and electrolyte imbalance including hypernatremia, hypokalemia, hypophosphatemia, and chronic illn…

A 51-year-old man committed suicide by swallowing the contents of a fire extinguisher. A few hours after his suicide attempt, he was referred to the medical center for poisoning. At the time of admission, the patient was conscious with stable vital signs. The patient complained of burning lips and mouth, mentioning diarrhea. Initial treatments included gastric lavage with activated charcoal, while paraclinical measures were requested. The patient had undergone hypernatremia (Na: 152 mEq/l) and metabolic alkalosis. Treatment focused on the adjustment of sodium level and alkalosis. On the first …

Most sodium disturbances in patients with CNS lesions result from disturbed water regulation. Possible systemic and iatrogenic causes must be evaluated prior to treatment. Insufficient secretion of ADH leads to hypernatremia if fluid intake is inadequate and can be treated with either fluid or hormone replacement. Care must be exercised in patients with acute diabetes insipidus because of the potentially variable and transient nature of the disturbance. Hyponatremia usually results from inappropriate secretion of ADH and should be managed aggressively in symptomatic patients with loop diuretic…

Abnormalities in salt and water balance are common following traumatic brain injury (TBI), manifesting clinically as either as hypo- or hypernatraemia. Dysnatraemia is associated with a greater risk of secondary brain injury from resultant changes in brain fluid levels, greater morbidity, longer length of hospital stay and in the case of hypernatraemia, greater mortality following TBI. Dysnatraemia occurs in the acute phase following TBI, is often transient and resolves in the majority of patients with recovery from the initial insult. Hyponatraemia secondary to the syndrome of inappropriate a

Sodium perturbations are a common complication after pituitary surgery, with hyponatremia being the most frequent. Postoperative assessments should be tailored to the early and late periods, and monitoring sodium perturbations is recommended. Cerebral salt wasting is rare after pituitary surgery, and diagnosis and management can be challenging. Providing patient counseling and close postoperative follow-up is important to effectively manage diabetes insipidus and reduce hospital readmissions due to sodium perturbations.
