INTRODUCTION: Langerhans cell histiocytosis is a rare disease within the adult population, with very few cases reported as solitary hypophyseal lesions in adults. Of the reported cases, most have been treated successfully with surgery, radiotherapy, and/or chemotherapy. Radiotherapy has been thought to be curative at the relatively low dose of 20Gy. Here we report a case of recurrent hypophyseal Langerhans cell histiocytosis 9 months after radiotherapy with an interval period of symptomatic and radiographic response to therapy.

A pituitary mass compressing the optic nerve was revealed by magnetic resonance imaging (MRI) in a 35-year-old woman complaining of visual disturbance in the post-partum period. Responses of plasma gonadotropin and corticotropin-cortisol levels to respective hypothalamic hormones were delayed or blunted, but the response of plasma prolactin to thyrotropin-releasing hormone was exaggerated. We observed a 53 year old man, who presented with a progressive impotence, a decrease of libido and a left sided defective vision. A MRI-study detected a left-sided meningioma para- and suprasellar and an intrasellar adenoma of the anterior lobe of pituitary. The pituitary function tests revealed a prolactinoma. In a program of in vitro fertilization, laparoscopies for oocyte aspiration were performed on 35 patients receiving human pituitary gonadotropin (hPG) and human chorionic gonadotropin (hCG). In the observations over the patients subjected to operations on the limbs under thiopental narcosis and in the experiments on white rats exposed to a shockogenic trauma (by Kennon's method) it has been found that the prophylactic injection of 1--2 mg/kg dose of dibazol rises the non-specific resistance of the body.
