Infections are a common complication in immunocompromised patients with haematological and oncological diseases, including in outpatient settings. They lead to distressing morbidity and can delay or jeopardize the implementation of effective antineoplastic therapy. Identifying risk groups for complicated infections may reduce morbidity and mortality. The prevention of bacterial and viral infections as well as Pneumocystis jirovecii pneumonia through drug prophylaxis and consistent vaccination are essential here. In many cases, precise clinical evaluation of patients with febrile neutropenia al

INTRODUCTION: Pneumocystis jirovecii pneumonia (PJP) remains a life-threatening lower respiratory tract infection associated with respiratory failure in patients with impaired T-cell immunity. Its incidence is increasing in non-HIV patients, who typically experience a more severe disease course and poorer outcomes than HIV patients. Keeping a high level of suspicion for PJP when confronted to an immunocompromised patient presenting with an acute respiratory failure (ARF) is essential to start promptly the treatment and thereby reduce associated morbidity and mortality.

The most frequent invasive mycoses in France are candidemia, Pneumocystis jirovecii pneumonia, and invasive aspergillosis. They occur primarily in immunocompromised patients. The risk of Pneumocystis pneumonia and cryptococcosis has notably decreased for people living with HIV through the past two decades. Patients with diabetes or an autoimmune disease represent an increasing proportion of these cases. Mortality of invasive mycoses is still high, particularly in cases of candidemia and invasive aspergillosis. Azole-resistant Aspergillus spp. isolates remain rare in France and the proportion o

OBJECTIVES: This systematic review and meta-analysis summarized documented breakthrough hepatitis A (HepA) infections and assessed whether they occur more frequently in immunocompromised populations (ICPs).
METHODS: We searched Medline, Embase, and Global Index Medicus for records on HepA breakthrough infections, defined as symptomatic HepA among previously vaccinated individuals, published 1991-2024 (PROSPERO: CRD42023450205). Risk of bias was assessed with an adapted Newcastle-Ottawa Scale. Moderate and good-quality studies were analyzed using random-effects and Bayesian hierarchical meta-a

BACKGROUND: Invasive mould infection (IMI) is a major cause of morbidity and mortality in immunocompromised children. Outcomes for paediatric patients with IMI remain poor, due in part to the limitations of available diagnostic tools and therapeutic agents.
OBJECTIVES: To summarize current and emerging modalities for diagnosis and treatment of IMI in immunocompromised children, focusing on recent advances and remaining barriers.
SOURCES: A comprehensive literature search of tools for diagnosis and management of IMI in children was conducted using PubMed, including clinical and preclinical st

OBJECTIVES: Posttransplant infections may lead to dire consequences in immunocompromised organ recipients. Oral foci of infection are therefore often eliminated prior to solid organ transplantation to reduce posttransplant morbidity. However, despite increasing numbers of organ transplantations the necessity of pretransplant dental treatment and its effect on transplant outcome remains uncertain. The aim of the present systematic review was to evaluate the impact of oral foci of infection and pretransplant dental treatment on adverse events following solid organ transplantation.

Toxoplasmosis is an infectious disease caused by the intracellular protozoan parasite, Toxoplasma gondii. While the infection is typically asymptomatic in healthy individuals, it can progress to cerebral toxoplasmosis, especially in those with human immunodeficiency virus (HIV) or weakened immune systems. In this communication, we present a case of a newly diagnosed HIV infection patient who presented with neurological symptoms. The patient was later diagnosed with probable cerebral toxoplasmosis. The case depicts the severe consequences of toxoplasmosis in immunocompromised patients, highligh

Improving short- and long-term outcomes in immunosuppressed patients, including patients with cancer, solid-organ transplant recipients, and patients with short- and long-term steroid treatment, has led to a paradigm shift in intensive care unit admission policies. Approximately 20% of patients currently admitted to the ICU carry a diagnosis of immune deficiency. In this population, acute respiratory failure and acute respiratory distress syndrome are the most frequent causes of ICU admission. Intensivists should therefore be aware of etiologies, diagnostics, and management strategies unique t

Legionella is an underdiagnosed and underreported etiology of pneumonia. Legionella pneumophila serogroup 1 (LpSG1) is thought to be the most common pathogenic subgroup. This assumption is based on the frequent use of a urinary antigen test (UAT), only capable of diagnosing LpSG1. We aimed to explore the frequency of Legionella infections in individuals diagnosed with pneumonia and the performance of diagnostic methods for detecting Legionella infections. We conducted a scoping review to answer the following questions: (1) "Does nucleic acid testing (NAT) increase the detection of non-pneumoph

In recent years, treat-to-target strategy and early intervention strategies with immunosuppressive agents have attempted to improve the prognosis and outcome in patients with autoimmune inflammatory rheumatic diseases. However, infectious complications due to side effects of medication remain a major concern in routine practice. In this regard, vaccine immunity and vaccination programmes are of the utmost importance in patients with systemic lupus erythematosus (SLE) in terms of morbidity and mortality. Encouragingly, research investigations have increased exponentially, both in monitoring the

Cryptococcal infection is well-documented in immunocompromised individuals. Cutaneous manifestations are not as common and are often difficult to diagnose due to variable presentations. Furthermore, there have been reports of coexisting cutaneous Cryptococcus and malignancy. We describe a patient that presented with a fast-growing mass (suspected sarcoma) in the hand that was ultimately treated for a Cryptococcus skin infection. We believe familiarity with the possibility of coexistence of these two conditions in an immunocompromised host could have brought about earlier diagnosis and possibly…

In parallel with the introduction of modern therapeutic and pharmacological interventions that have successfully resolved many diseases and conditions, previously deemed incompatible with life, there has been a significant increase in the number of patients experiencing secondary immunodeficiency. As a result, these patients are highly susceptible to various opportunistic infections. Among these infections, pneumocystis pneumonia (PCP) stands out as one of the most frequent and potentially life-threatening ones, necessitating prompt diagnosis and treatment. Observational studies have clearly s

Toxoplasmosis is one of the most prevalent parasitosis in the world. It is associated with perinatal morbidity and in immunocompromised population. Currently, there are several diagnostic tests, both serological and molecular, that can help confirm the diagnosis and differentiate a recent infection of a pass. We propose some recommendations for the diagnosis and treatment of toxoplasmosis in different clinical scenarios based on the currently available evidence.

Fungal infections of the orbit are infrequent. They typically affect immunocompromised individuals but, in some instances, can also affect an immunocompetent person. They mimic much orbital pathology clinically. Ours is a series of three patients eventually diagnosed with orbital aspergillosis but who, initially, were thought to have some other pathology.

Sternal osteomyelitis due to aspergillosis is extremely rare. Among all cases of invasive aspergillosis reported in the literature, the incidence of osteomyelitis is less than 3%. Aspergillosis mainly affects immunosuppressed patients. Clinical and radiological manifestation is nonspecific. Contamination is primarily caused by inhaling spores, but it can also directly reach a vulnerable area after medical procedure. Diagnosis is often difficult and may take several weeks, in particular when aspergillosis is not suspected. Positive diagnosis is based on imaging tests but it is confirmed by anat

PURPOSE: To report a case of Aspergillus fumigatus endogenous endophthalmitis in an immunocompetent patient initially diagnosed as acute retinal necrosis.
METHODS: Case report.
PATIENT: A 67-year-old woman with a remote history of treated pulmonary tuberculosis and no ocular history presented to an outside retina specialist with a sudden onset of floaters and blurred vision in one eye. Examination and fluorescein angiography at the time revealed findings suspicious for acute retinal necrosis, and the patient was started on oral valganciclovir and an intravitreal injection of ganciclovir.
