Refractory adult-onset still disease successfully treated with abatacept.

Adult-onset Still disease (AOSD) is an inflammatory condition of unknown etiology that responds to glucocorticosteroids and disease-modifying antirheumatic drugs, particularly methotrexate. However, disease refractory to conventional treatment has led to the reported use of biologic therapy including tumor necrosis factor α inhibitors (infliximab, etanercept, and adalimumab), anakinra, rituximab, and tocilizumab. We report the successful use of abatacept in the treatment of a patient with AOSD manifested by polyarthritis, rash, fevers, elevated liver function tests, and ferritin levels refract

Use of a Desensitization Protocol in a Patient with Brentuximab Hypersensitivity: A Case Report from a Hospital in Rancagua, Chile.

Hypersensitivity reactions to antineoplastic agents may jeopardize treatment continuity. Brentuximab vedotin, an anti-CD30 antibody-drug conjugate, may elicit hypersensitivity reactions that require specific management strategies. Case: A 19-year-old male with refractory Hodgkin lymphoma developed significant hypersensitivity to brentuximab during prior cycles. A 12-step rapid desensitization protocol with standardized premedication and close monitoring was implemented. Two hypersensitivity-like events occurred during infusion and were controlled with supportive measures and temporary infusion

Artificial Intelligence-Enabled Continuous HER2 Assessment: A Patient Selection Framework for the Antibody-Drug Conjugate Era.

IntroductionAntibody-drug conjugates have expanded HER2-targeted therapy to HER2-low breast cancer, yet categorical immunohistochemistry scoring inadequately supports patient selection at expression boundaries where interobserver agreement is suboptimal.MethodWe propose that artificial intelligence-enabled continuous quantification addresses this gap by preserving expression gradients essential for treatment eligibility decisions while maintaining pathologist interpretability.ResultUsing a pilot dataset (n=66), we demonstrate that data-driven thresholds derived from continuous scores can suppo

Targeted Therapy in Hepatobiliary Pancreatic Cancer (HPC): Advantages and Advancements of Antibody Drug Conjugates, a Type of Chemo-Biologic Hybrid Drugs.

Currently, there are very few efficient treatment options for hepatobiliary pancreatic cancer (HPC), which comprises pancreatic ductal adenocarcinoma (PDAC), biliary tract cancer (BTC), and hepatocellular carcinoma (HCC). The HPC tumors are the most lethal malignant tumors in the world. Traditional chemotherapy offers little survival benefit and is associated with notable systemic toxicity, which has made antibody-drug conjugates (ADCs) a hopeful treatment option. Strong cytotoxic drugs combine with monoclonal antibodies to attack tumor-associated antigens. This review discusses the benefits a

Advances in antibody-drug conjugate-based combination therapy strategies for non-small cell lung cancer.

INTRODUCTION: Non-small cell lung cancer (NSCLC) continues to be one of the leading causes of cancer death worldwide. Although targeted therapies and immune checkpoint inhibitors have meaningfully improved outcomes for some patients, many still lack targetable mutations or quickly develop resistance. Antibody-drug conjugates (ADCs), which combine tumor-specific antibodies with potent cytotoxic payloads, have become an increasingly promising option. AREAS COVERED: This review looks at ADC-based combination strategies in NSCLC, grouped by key molecular targets such as Trop-2, HER2, HER3, c-Met,

Recent advances in bispecific antibody-drug conjugates for breast cancer therapy.

PURPOSE: Bispecific antibody-drug conjugates (BsADCs) represent a promising strategy to overcome limitations of conventional ADCs in breast cancer, such as tumor heterogeneity and inefficient internalization. This review summarizes recent advances and the therapeutic potential of BsADCs. METHODS: We conducted a literature review, focusing on BsADC candidates selected for clinical relevance (e.g., ZW49, BL-B01D1), mechanistic innovation (e.g., biparatopic targeting, engaging fast-internalizing receptors), and potential in challenging subtypes like triple-negative breast cancer (TNBC). RESULTS

Engineering antibody-drug conjugates targeting an adhesion GPCR, CD97.

Adhesion G protein-coupled receptors (aGPCRs) are key cell-adhesion molecules involved in many cellular functions and contribute to human diseases, including cancer. aGPCRs are characterized by large extracellular regions that could serve as readily accessible antigens. However, the potential of aGPCRs as targets for biologic therapeutics has not been extensively explored. CD97, also known as ADGRE5, is an aGPCR that is upregulated in various cancer types, including acute myeloid leukemia (AML) and glioblastoma (GBM), and their respective cancer stem cells. Here, we developed antibody-drug con

In silico decrypting of the bystander effect in antibody-drug conjugates for breast cancer therapy.

Antibody-Drug Conjugates (ADCs) are a promising cancer treatment that deliver toxic drugs directly to cancer cells, reducing harm to healthy tissue. A key feature of newer ADCs is the "bystander effect," in which nearby cancer cells are also affected by passive diffusion. However, the mechanisms underlying this effect remain unclear. Using computer simulations, this study investigates how the drug's ionization state and the linker connecting it to the antibody influence its ability to cross cell membranes. The results show that the ionization state of the drug impacts its membrane permeability

[Current Status and Issue of Emesis and Vomiting Caused by Trastuzumab Deruxtecan].

Emesis and vomiting are the most common adverse events related to trastuzumab deruxtecan(T-DXd). T-DXd is listed as a moderate emetogenic chemotherapy(MEC)agent in the latest guidelines for the appropriate use of antiemetic drugs in Japan; however, the National Comprehensive Cancer Network Guidelines regard it as a highly emetogenic chemotherapy (HEC)agent. We investigated the risk factors for chemotherapy-induced nausea and vomiting(CINV)associated with T-DXd by analyzing data from 40 patients with advanced breast cancer(median age, 56 years)were receiving T-DXd administration between June 20

A Novel Hybrid LC-MS/MS Methodology for the Quantitative Bioanalysis of Antibody-siRNA Conjugates.

Small interfering RNA (siRNA) is a new class of oligonucleotide therapeutics that is rapidly growing in drug research and development for the treatment of various diseases. One major challenge for siRNA therapeutics is their inefficient delivery to target tissues by systemic administration. Antibody-siRNA conjugates (ARC) are being developed as a promising approach to enhancing the selectivity and delivery of siRNA payloads to target tissues. A sensitive, selective, and reliable method to quantify ARCs in biological samples is critical to understand their pharmacokinetics, pharmacodynamics, to

Idiosyncratic Drug-Induced Liver Injury and Trastuzumab Deruxtecan in Breast Cancer: A Case Report.

Trastuzumab deruxtecan (T-DXd) has demonstrated efficacy in HER2-positive and HER2-low breast cancer. Its main safety concern is interstitial lung disease, while clinically relevant hepatotoxicity is rarely reported. In our case report we describe a 49-year-old woman with HER2-positive advanced breast cancer that developed persistent grade 3 transaminase elevations after 2 cycles of T-DXd, refractory to corticosteroid treatment and requiring treatment discontinuation. This case underlines the unpredictable and idiosyncratic nature of T-DXd associated hepatotoxicity and the importance of liver

Complete response following trastuzumab-deruxtecan in a man with HER2-overexpressing metastatic, poorly differentiated scrotal carcinoma.

Overexpression of human epidermal growth factor receptor 2 (HER2) has been implicated as a molecular driver of numerous solid tumor subtypes. An antibody drug conjugate (ADC) directed to HER2, trastuzumab-deruxtecan, recently gained tumor-agnostic FDA approval for the treatment of advanced HER2-positive (IHC 3+) solid tumors. Here, we present a patient with HER2-positive invasive poorly differentiated scrotal carcinoma treated with trastuzumab-deruxtecan. The treatment resulted in a complete response, with a tolerable side effect profile and ongoing treatment-free survival. Our case adds to th

Management of peripheral neuropathy associated with brentuximab vedotin in the frontline treatment of classical Hodgkin lymphoma.

The ECHELON-1 trial demonstrated the effectiveness of brentuximab vedotin (BV) in combination with doxorubicin, vinblastine, and dacarbazine as a frontline treatment regimen in classical Hodgkin lymphoma. However, peripheral neuropathy (PN) is common with this regimen, occurring in up to two-thirds of patients. While standard prescribing information recommends BV dose modification at the onset of grade 2 PN, management strategies for PN are not well-defined. Most commonly, clinicians dose reduce or discontinue BV, vinblastine, or both. We review evidence-based and practical approaches for mana

An Antibody-Drug Conjugate for Multiple Myeloma Prepared by Multi-Arm Linkers.

First-line treatment of multiple myeloma, a prevalent blood cancer lacking a cure, using anti-CD38 daratumumab antibody and lenalidomide is often inadequate due to relapse and severe side effects. To enhance drug safety and efficacy, an antibody-drug conjugate, TE-1146, comprising six lenalidomide drug molecules site-specifically conjugated to a reconfigured daratumumab to deliver cytotoxic lenalidomide to tumor cells is developed. TE-1146 is prepared using the HighDAR platform, which employs i) a maleimide-containing "multi-arm linker" to conjugate multiple drug molecules creating a drug bund
